About Avamere Transitional Care And Rehab – Brighton - CLOSED

Avamere Transitional Care And Rehab - Brighton, also known as Brighton Care Center, used to sit at 2025 E Egbert St, Brighton, Colorado, before it closed down. This community managed different types of care over the years, including skilled nursing, rehabilitation, long-term care, respite care, and short-term rehabilitation for people needing extra help to recover and regain strength before going home, and they even had a temporary COVID-19 Recovery Unit in 2020, so folks found a place here when they needed somewhere to heal up with good supervision. The facility offered both studio rooms and furnished options, and it let people choose from all-day dining, restaurant-style food, and special meals for allergies or diabetes. People found a range of programs and activities, with arts rooms, a barber/salon, walking paths, gardens, a dining room, and transportation for errands or group trips, all put together to let residents keep active and social.

Specialties included medication management, staff help with bathing, dressing, transfers, and 24-hour supervision, and the staff worked with each resident, their families, and medical providers to make sure everyone in the care plan was on the same page, so care stayed focused on the person's real needs and wishes. The community employed people who spoke English and had backgrounds in skilled nursing and memory care, and for rehabilitation, there was in-house therapy using new equipment and therapeutic approaches that helped folks get back some ability or independence after injuries or illness. For a while, the community held a 5-star Medicare rating and a 4-star staffing rating, with an A+ score and partnerships, though it wasn't accredited by the BBB.

Over 20 years since opening in 1998, the building served the Brighton area, and the management, led by Mr. Mike Schnable, kept the place busy with different levels of care, from Independent Living and Assisted Living to Memory Care, Long-Term Care, and Home Health Care, including services like home care and hospice. When open, Avamere Brighton let residents and their families choose what worked for them, whether it was emergency alert systems, community activities, move-in help, or daily assistance for tasks. However, this community doesn't accept new patients and remains closed, so people now must look elsewhere for these kinds of services and care.
