    Good therapy but safety concerns

I had a mixed experience. The building is older but generally clean, meals/dining are pleasant, and therapy/rehab staff were effective - a few staff stood out (RNs Tammy, Kathy, Linda; LPN Ryan; Activities Dir. Shana) and showed real compassion. However staffing is thin with high turnover, care is inconsistent, and I saw serious safety concerns: medication errors, lost items, rude or inattentive staff, and very limited dementia training - not good for Alzheimer's residents. Overall, good therapy and some wonderful caregivers, but because of the safety and dementia-care issues I would not choose this as my first option.

    Summary review

Overall sentiment for Clear Creek Care Center is highly mixed with strong, repeated praise for frontline caregivers and rehabilitation services balanced against serious and recurring concerns about staffing, safety, and administrative responsiveness. A large portion of reviewers explicitly commend the CNAs, many nurses, and therapy staff for being compassionate, attentive, and skilled. Multiple reviews highlight individualized and effective PT/OT programs, successful rehabilitation outcomes, friendly activities staff, good discharge planning, and hands-on support for hospice and veterans. When the staff and leadership are engaged, reviewers describe a warm, home-like atmosphere, clean interiors, comfortable dining rooms, varied home-cooked meals, and a small-community feeling that eases recovery and end-of-life care.

However, an equally significant portion of reviews recount troubling safety and quality problems. Repeated themes include chronic understaffing, slow or inconsistent responses to call lights, missed or incorrect medications, and incidents involving theft and privacy (HIPAA) violations. Several reviewers reported serious clinical lapses — wound-care failures, medication dosing errors (including a blood-thinner mistake), delayed transfers to higher-level care, and infections that in some cases progressed to severe outcomes. These incidents were particularly damaging because reviewers also described management responses as dismissive or ineffective, with some saying complaints were not properly investigated or resolved. A number of reviews claim medications were improperly stored or mixed together, and at least one reviewer alleged corporate-level issues (Vivage/Optum) had negatively influenced care practices.

Facilities and amenities receive both praise and criticism. Many reviewers describe the interior as very clean, updated, and well-maintained, with comfortable partitioned double rooms, warm common areas, attractive dining rooms, and pleasant outdoor space. The food is frequently praised as hot, home-style, and varied; some reviewers appreciated dining-room touches such as tablecloths and daily specials. Conversely, other reviewers reported cold meals, poor food quality, laundry problems, and occasional unsanitary incidents (blood or vomit left on linens). The building’s exterior and some rooms are repeatedly described as dated (1970s architecture), with small rooms, shared bathrooms, limited closet space, and concerns about air circulation, mattress comfort, and lack of side rails on some beds.

Regarding dementia and memory-care needs, multiple reviews explicitly warn that Clear Creek is not well suited for residents with Alzheimer’s or advanced cognitive impairment. Several families reported a lack of dementia training among nurses and inconsistent behaviors toward residents with memory loss (including reports of sedating residents or using drugs to manage behavior), and reviewers cautioned that staff interaction is limited for residents who need more engagement or specialized care.

Staffing patterns and administration are a major dividing line in the reviews. Where leadership and therapy directors were described as proactive and responsive, families reported positive coordination, inclusion in care decisions, and good discharge planning. Where leadership was perceived as distant or defensive, reviewers recounted unresolved problems, lost items, and a sense that corporate priorities or cost-cutting were eroding care quality. Several accounts emphasize poor night/weekend coverage, slow weekend administrative response, and episodes when short-staffed crews left residents waiting for basic needs.

Activities and social engagement receive many favorable mentions: active programs, trips, celebrations, and specific staff (named Activities Director) who were singled out as blessings. These strengths underline that the facility can provide a rich social and rehabilitative environment when staffing and management align. Conversely, multiple reviewers described residents as depressed, under-stimulated, or neglected when staffing was inadequate.

In short, Clear Creek Care Center exhibits a polarized profile: when staffed and led well, it delivers compassionate, skilled care, strong rehabilitation outcomes, good food, and a family-like environment at reasonable cost. When understaffed or when management does not respond effectively to problems, the facility has serious safety vulnerabilities — medication errors, theft, inadequate wound or infection care, falls, and poor complaint handling. Prospective residents and families should weigh these trade-offs carefully, prioritize direct questions about current staffing ratios (including night/weekend coverage), medication management procedures, dementia-care training, incident reporting and investigation practices, and recent state inspection or complaint history. A thorough tour at different times of day, conversations with current families, and specific inquiries about security, laundry procedures, and how the facility handles medication and escalation should be part of decision-making given the inconsistent reports.

    About Clear Creek Care Center

Clear Creek Care Center sits in a quiet, residential area of Westminster, Colorado, at 7481 Knox Pl, and has been there for over 20 years as a family-owned facility. Managed by Vivage Senior Living and overseen by an administration team led by Charity Linenberger, the center serves as a nursing home offering both long-term care and short-term rehabilitation. Skilled nursing care is one of the main offerings here, with on-site therapists and clinical staff who help residents recover from health events like strokes, fractures, hip and knee replacement surgeries, heart conditions, and respiratory issues. People get tailored care plans built specifically for them, with regular check-ins by a group of trained healthcare workers to make sure needs get met as medical conditions change.

Residents can take part in community activities and events, staying involved in things going on nearby and even learning about nationwide news. The care center values each person's interests and abilities, finding ways to help folks keep a sense of purpose each day. Services focus on the whole person and not only the medical needs, offering tools like health libraries, a symptom checker, and learning centers for behavioral health recovery. For those needing extra support, there are programs for chronic disease management, short-term stays, rehabilitation, and respite care.

The staff provides help with getting and learning about assistive devices, and they connect residents with resources on health care programs, including those funded by state, federal programs, and the Affordable Care Act. There's even involvement with state and federal legislative activities so residents and families can look up bills, search for legislators, and check up on legislative calendars. In case of emergencies, help is available right away. Although Clear Creek Care Center isn't taking new patients right now, the center continues to provide care to those already living there, with a focus on comfort, safety, and personalized service for every individual.
