    Warm, caring dementia care community

I'm very pleased with this family-like, homey community - the staff are friendly, caring, and go above and beyond, providing strong, person-centered dementia care. The facility is clean and well-kept, nurses and CNAs are attentive, and there are varied activities that keep residents engaged. I noted occasional staff inexperience and some food concerns, but overall I highly recommend it.

    Summary review

Overall sentiment across the reviews is strongly positive, with many reviewers expressing high satisfaction and willingness to recommend Harvest Acres Nursing and Rehab. Recurring strengths are attentive, compassionate care and a welcoming, family-like atmosphere. Multiple reviewers highlight that nurses and CNAs are attentive and that staff take pride in their work; residents and families often feel heard, encouraged, and cared for. There is a clear emphasis on person-centered care and many comments indicate that the staff go above and beyond to make residents comfortable.

Care quality and staffing are commonly praised: reviewers repeatedly mention compassionate staff, strong dementia support, and staff who provide individualized attention. A notable theme is the facility’s focus on memory care — several reviews point to a new, dementia-focused direction intended to maintain a calm, engaged lifestyle for residents. That initiative is described as timely and much needed, with staff described as working hard to implement it. At the same time, a minority of reviewers report issues with staff experience levels and occasional frustration directed at residents; these comments suggest some variability in staff training or consistency that prospective families may want to inquire about.

Facility and environment are another strong area. The building and grounds are described as clean and well maintained, inside and out, with housekeeping and cleaning staff singled out for praise. Reviewers note a comfortable living environment and describe visits as pleasant, reinforcing the impression of a homelike setting. The sense of community and ‘extended family’ feeling appears frequently, with many reviewers using words like loving, homeliness, and family atmosphere.

Dining and activities receive mixed but generally positive feedback. Several reviewers mention balanced, healthy meals and small treats for residents, while a robust schedule of varied activities and fun events is a consistent positive. However, there are explicit complaints from some reviewers about food quality, describing meals as unpalatable. This contrast indicates that dining experiences may vary by day or by reviewer expectations and is an area worth checking during a visit.

Management and compliance show both strengths and caveats. Multiple reviewers say staff and management are professional and respond to residents’ concerns in a timely manner, and that the community is making deliberate moves toward improved memory care programming. Conversely, a few reviews mention that the facility has been cited for infractions; while details are not provided in these summaries, such notes suggest prospective residents should review recent inspection reports and ask management about corrective actions taken. The characterization of the facility as a small-town operation may convey close community ties for some but could also signal more limited resources compared with larger, urban providers.

In summary, Harvest Acres Nursing and Rehab presents as a caring, community-oriented facility with strong dementia-focused care, attentive staff, good cleanliness, and engaging activities. Most reviewers are highly satisfied and would recommend the community. Notable concerns include some reports of staff inexperience or frustration, inconsistent food quality, and mentions of regulatory citations. Prospective residents and families should weigh the many positive reports about staff compassion and the homelike environment against these isolated concerns, and consider an in-person visit, a review of current inspection records, and specific questions about staffing stability, training, and dining to make an informed decision.

About Harvest Acres Nursing and Rehab

Harvest Acres Nursing and Rehab stands in Keota, IA, as a for-profit nursing facility offering both long-term care and skilled nursing, and the place takes Medicare and Medicaid if folks need that. The building holds up to 35 beds and has an average of about 26 residents each day, so it's not too big and not too noisy, either. The staff here provides specialized health care, including skilled rehab services, wound care, and support for those with complex needs, and there's always a nurse on duty all hours of the day. Even though the nursing staff is described as compassionate and highly qualified, staff levels run below the state average, and nurse turnover is higher than usual at 56.5% compared to the state's 46.6%. Residents get about 2.89 hours with a nurse each day when the state average is closer to 3.8.

Quality measures at Harvest Acres rate above average, but the facility's health inspections are much below average, and it's listed as a Special Focus Facility Candidate because of serious quality history, including 43 inspection deficiencies, five about infection control, and some citations for not responding or reporting possible abuse, and not protecting residents from neglect or exploitation. The overall government rating sits at much below average.

The facility fits several types of needs, including folks needing long-term nursing care, folks needing memory care, and those needing short-term rehab to help them get well and go home. Harvest Acres handles physical, occupational, and speech therapy, tailoring each plan to help folks regain their abilities, and there's a focus on making transitions home when possible. They offer hospice and respite care, too, with support for comfort, pain management, and dignity at the end of life. For memory care, there's a secure wing for people with Alzheimer's or dementia, with safety features, small group activities, and one-on-one attention. Amenities include private rehab suites, private dining, lounges, a beauty salon, linen service, and housekeeping. The building offers respiratory support, such as oxygen therapy, CPAP, and Bi-PAP, and certified nurses handle wound care on-site.

Harvest Acres operates Monday through Friday from 10:00 to 17:00. Ownership is tied to Campbell Street Services, but further details aren't provided. The facility's motto is "Residents First," and the focus lies in maintaining privacy, dignity, and comfort for those with different levels of care needs, even if the facility's inspection and staffing issues should be noted when making decisions.
