    Clean facility but concerning care

I have mixed feelings. The place is extremely clean with no odor, my dad liked the food, and their memory care/skilled nursing and intensive therapy helped my mom recover and go home after three weeks - staff often listened, warned us about meds, and even stayed with her overnight - but we also experienced rude and untrustworthy behavior and concerning incidents that felt like neglect/abuse, so I'd recommend visiting often and watching care closely.

    Summary review

Overall sentiment in the reviews for Accura HealthCare of Pleasantville is strongly mixed, with many reviewers reporting excellent, attentive clinical care and rehabilitation outcomes while other reviewers describe deeply troubling incidents of mistreatment and neglect. Positive accounts emphasize strong clinical skills, individualized attention, and a clean, pleasant environment; negative accounts make serious allegations about staff behavior, inconsistent management, and even abuse. These conflicting reports create a polarized picture that warrants cautious interpretation and follow-up verification.

Care quality and outcomes: Several reviewers highlight high-quality skilled nursing and effective rehabilitation services. Specific positives include extensive cognitive and physical therapy, diligent medication management (including being warned about medication side effects like hallucinations), and staff who stayed with a resident through the night and focused on the goal of returning the resident home. At least one family reported that their relative returned home after a three-week stay — a clear example of successful short-term rehab. These reports suggest that the facility can provide competent, goal-oriented clinical care, especially for short-term rehabilitation and memory-care needs.

Staff behavior and culture: Reviews show a split in perceptions of staff. Many comments describe staff as kind, compassionate, friendly, and passionate — with families noting attentive, empathetic interactions and staff who helped residents feel at home. Conversely, other reviewers report rude or negative attitudes, allegations that staff tricked a resident, and claims of a lack of care. The presence of both highly positive and extremely negative descriptions could indicate variability in staff performance across shifts or units, intermittent staffing issues, or changes in management or personnel over time.

Facilities and dining: The physical environment is generally praised in multiple reviews: the facility is described as extremely clean with no odor. Dining is mentioned positively as well, with at least one family noting that their parent liked the food and calling this facility the best among local nursing homes. The memory care unit receives particular praise in some reviews and is described as being among the best, along with notes about active community involvement and an engaged atmosphere.

Serious allegations and safety concerns: A subset of reviews make very serious accusations, including elder abuse, neglect leading to coma, and claims that residents faced life-threatening risks. Some reviewers used extremely strong language (calls to shut down the facility, warnings not to place family there). These allegations are severe and, if accurate, would be cause for immediate regulatory scrutiny and legal action. They stand in stark contrast to the positive rehabilitation and care stories and must be treated as high-priority concerns to investigate further rather than dismissed as isolated complaints.

Management and consistency: Several reviewers complained about changing rules and inconsistent policies, suggesting potential instability in facility management or communication issues with families. The mixture of glowing praise for staff passion and equally strong criticisms about rudeness and deception suggests inconsistency in training, oversight, or culture. This pattern can produce very different experiences depending on timing, specific unit, or which staff members are on duty.

Notable patterns and recommended next steps: The reviews indicate two clear clusters of experience — many families report positive clinical outcomes, compassionate care, and a clean environment, while others report unprofessional behavior and, in a few cases, allegations of severe abuse. Because of this polarization, anyone considering placement should follow up before making decisions. Recommended validation steps include reviewing the facility’s most recent state inspection and complaint history, asking administration about staff training, turnover and staffing ratios, requesting references from current families, touring the specific unit where a loved one would stay, and asking about incident reporting and abuse-prevention protocols. If there is any sign of abuse or neglect, contact local adult protective services and the state licensing agency immediately.

Summary conclusion: Accura HealthCare of Pleasantville receives commendations for clinical rehabilitation, medication vigilance, cleanliness, and compassionate care in many accounts, especially around short-term skilled nursing and memory care. However, the presence of multiple severe allegations and reports of inconsistent staff behavior makes the overall reputation highly mixed. These divergent reports recommend cautious optimism: recognize the facility’s demonstrated strengths but verify safety and management practices thoroughly prior to placement and pursue immediate investigation if any abuse or neglect is suspected.

    About Accura HealthCare of Pleasantville

Accura HealthCare of Pleasantville sits at 909 North State Street in Pleasantville, Iowa, with 46 certified beds and an average of about 42 residents each day, so you're looking at a place that's pretty full most of the time, and while it offers long-term care and skilled nursing with therapy services for things like stroke recovery, cardiac and orthopedic care, diabetes, and pulmonary needs, it's also got dedicated memory care for folks living with Alzheimer's or dementia, plus respite care for short stays and hospice support for folks nearing the end of life, which means they tend to most needs right under one roof, and they do try to make it feel warmer with activities, therapy recreation, and a community-style approach, though it's important to know that inspection reports have pointed out serious concerns-there are 68 documented deficiencies, including four about infection control, and one inspector even found a violation in infection control that was called an immediate jeopardy to resident health, while others noted not enough nursing staff on duty and some physical safety hazards, and another thing is the nurse turnover rate sits at 63.6%, which is higher than the state's average, and the actual nurse hours per resident come up short too, at 3.04 a day when the state's average is 3.8, which may mean less one-on-one attention, and yes, the staff do speak English and work in teams to cover medical, psychological, nutritional, and spiritual needs, following personalized care plans, and they offer lots of therapy-from balance exercises to safety awareness training-but the facility is not accepting new residents right now, and faxing isn't an option, and all of this has gone on under American Healthcare Management Services LLC since 2003, with an affiliation to Accura Healthcare, so it's a place that says it focuses on patient-centered support and family-style care, but inspection records show it faces challenges with staff levels and infection prevention that families should keep in mind.
