    Caring staff but leadership concerns

I had a great experience overall. The CNAs, housekeeping and dietary were excellent - caring, well-trained and very engaged; the rooms and facility are nice and well-equipped with a true small-town sense of community. My main concerns were leadership: I found the ADON dishonest and the administration unprofessional and burnt-out - they need a new admin team - and I ran into issues with agency nurses' contracts and payments. Despite that, staff were courteous, communicative, and provided individualized, excellent care.

    Summary review

Overall sentiment in the reviews is strongly positive about the day-to-day care and living environment at Riceville Family Care and Therapy Center, with repeated emphasis on high-quality direct care, cleanliness, food service, and a warm community atmosphere. Multiple reviewers specifically single out CNAs and other hands-on caregivers as excellent, noting dedication to elders, individualized attention, and staff who actively engage residents. Housekeeping and dietary services receive consistent praise, suggesting a reliable level of cleanliness and meal service. Reviewers also describe the staff as helpful, communicative, courteous, and friendly, reinforcing the impression of a supportive caregiving culture.

The facility and accommodations are described favorably: reviewers call the location nice and well equipped, and mention well-furnished rooms. Comments about staff being well trained and knowledgeable support the clinical competence found on the floor. The small-town nursing home and clinic setup is noted as a positive feature by some, contributing to a sense of community and a unique environment where staff and residents appear to form close relationships. Multiple reviewers summarize their experiences as great, indicating overall satisfaction with resident outcomes and daily living.

Notwithstanding the strong praise for frontline care, there are consistent and significant concerns about administration and staffing logistics. Several reviewers accuse the administration — specifically the Assistant Director of Nursing (ADON) — of dishonesty, and describe leadership as unprofessional or burnt out. There is an expressed desire for a new administrative team from some reviewers. Related operational issues include reported problems with agency nurses around contracts and payment, which could reflect challenges in staffing continuity or external staffing arrangements. These administrative complaints stand in contrast to the positive descriptions of caregiving staff, and multiple reviews suggest that while the hands-on team is performing well, leadership and administrative stability may be undermining morale or operational smoothness.

Taken together, the reviews paint a facility that delivers strong, compassionate day-to-day care, with dependable housekeeping and dining, a comfortable physical environment, and an engaged staff that fosters community. The predominant strengths are the quality and dedication of direct caregivers, the cleanliness and upkeep, and the individualized attention residents receive. The main pattern of concern lies in management and administrative practices — allegations of dishonesty, perceptions of burnout, and issues with agency nurse contracts/payments — which are serious enough to warrant attention because they could affect staff retention, morale, and long-term consistency of care. In summary, Riceville Family Care and Therapy Center is frequently described as a great place with excellent frontline staff and services, but reviewers repeatedly call out leadership and administrative problems that should be addressed to preserve and strengthen the overall quality of care.

About Riceville Family Care and Therapy Center

Riceville Family Care & Therapy Center sits on Woodland Avenue out in Riceville, IA, and folks around here have counted on it since 1972, back when it started as Riceville Community Rest Home, and these days it's community-owned and non-profit, tending to the northern Iowa and southern Minnesota areas all these years under experienced hands like Continuum Health Care Services Lc, The BRIDGE Organization Inc, and more recently managed by Katie Mayer and Jeffrey Nasstrom. It's a rural place with a warm, family-minded feeling and is known for making personalized care plans for every resident, always through the thoughtful work of skilled staff, and while it has 34 certified beds, it averages about 26 residents daily, which helps keep things feeling a little more personal. Nurses work about 3.48 hours per resident each day, which falls a little shy of the state average but does show that the turnover rate for nurses is a bit better than most-42.9% compared to Iowa's 46.6%. You'll see they've had inspection visits in recent years, like in March 2024 and 2025, with six total deficiencies noted across those, which sometimes mentioned issues in care planning, so they're always under review and working on things, and residents or families can always expect the center to value transparency and community input.

The facility's a steady piece of the area and pays attention not only to needed care like skilled nursing, long-term stays, and out-of-home respite care, but also runs a therapy center that serves both folks living there and those coming in from outside, with services stretching from physical therapy all the way to family care, counseling, behavioral health, and even chiropractic support when needed. For those who need it, dialysis transport is covered, and the center welcomes hospice patients but lets hospice teams take the lead-plus they serve Meals on Wheels three times a week in Riceville along with an on-site Congregate Meal Site on the same days, which seems to keep seniors connected and fed. They may not have locked dementia units, but they screen folks' needs on a case by case basis. Modern touches like spacious waiting areas and comfy rooms make stays a little nicer, and shifts run 24/7 with office hours steady during the weekdays, which gives people a sense of support around the clock. Independent apartments sit close by, though there's a bit of a wait list since part of that space now helps out the therapy program, and background checks are part of how they keep things safe for everyone.

Riceville's always tried to keep things community-minded, welcoming family involvement in care and supporting both chronic disease management and preventative medicine-they won the 2009 Governor's Award for excellence in health care, and in 2010 got called one of the Best Nursing Homes in America by US News & World Report, plus they hold a 4-star Medicare/Medicaid rating. Even though every place has its challenges-inspections now and then turn up things to work on-the focus here sits on patient comfort, thoughtful care planning, experienced staffing, and making sure that every resident feels cared for, with strong community connections that stretch beyond the front doors into both Iowa and the neighboring areas.
