    Aspire of Washington, which used to be called Pearl Valley, is a for-profit skilled nursing facility with 90 certified beds, and it accepts Medicare and Medicaid, so they're able to serve people with different needs and insurance types, and they have private and semi-private rooms, a spacious courtyard, scenic garden, and a greenhouse that residents can enjoy outdoors when the weather's nice, and the community setting is meant to feel comfortable, with home-cooked meals and snacks, a common dining and living area with Wi-Fi, and some areas that allow smoking, and they're open to anyone who qualifies after an assessment. The facility offers both short-term rehab and longer-term care for people who need more daily help, and the nursing team includes certified nursing assistants (CNAs), licensed practical nurses (LPNs), and registered nurses (RNs), and while nurse staff hours per resident are 3.53 each day, which is below the state average, the facility offers 24-hour care and a lot of specialized services like wound care, pain management, diabetes monitoring, medication, and help for people with breathing problems, plus they provide hospice, palliative, respite, and telehealth support, and connect people with dentists, eye doctors, hearing specialists, podiatrists, and mental health professionals as needed. They also have a lot of therapy services on-site with physical, occupational, and speech therapy-treatments can include everything from help with wheelchairs and splints to memory and cognitive programs, and all care is planned to fit the person's needs, aiming to help folks recover and feel as independent as possible. There's a focus on activity and "Pathways to Active Living" or PALs, which adds social, emotional, physical, mental, and spiritual activities meant to keep residents engaged, with organized groups and events. The facility's staff aims to follow values of caring, accountability, and respect, but Aspire of Washington's ratings on Medicare Nursing Home Compare (as of November 23, 2024) show a history of serious quality problems-it's actually a Special Focus Facility Candidate, which means it's had more oversight because of issues like 87 documented inspection deficiencies, higher than average infection-related problems, concerns about having enough food and fluids, safe pain care, and keeping the environment safe. Their nurse turnover is 60%, higher than both the state average and what many people might expect, and there are noted violations involving infection control and residents' quality of life and care.
