    Good bedside care, systemic failures

The direct care here was good - friendly, attentive staff who helped with baths and meals, ran exercise/music activities, provided therapy and transportation, and generally kept families informed. However, management was unresponsive (unreturned calls, transfer problems), and I encountered serious safety and quality issues: bed rails wrapped in pipe insulation and tape, delayed assistance, a bed fall, medication errors, missing belongings (including my grandmother's wedding rings), poor food, and rude staff (one called my husband "trash"). I also experienced a culture of intimidation, alleged abuse reports, retaliation, lack of transparency and leadership cover-ups; my mother died there during the COVID lockdown and I would have moved her if I could. Good bedside care but systemic safety, leadership and trust failures - I cannot recommend it.

    Summary review

The reviews for Care Center of Dequincy present a highly mixed picture with distinct positive experiences alongside serious and recurring negative allegations. Multiple reviewers praise aspects of direct care: several families report that residents receive good personal care, staff help reliably with baths and meals, and residents participate in exercise and music activities. Therapy services and transportation to appointments are mentioned positively, and some reviewers note that staff are up-to-date, supportive, and keep families informed. The facility's location is described as convenient for visits, and in some cases residents are reported to be comfortable and well-cared for.

Counterbalancing those positives are repeated, specific, and concerning complaints about management, safety, and professional conduct. A consistent theme is unresponsive management — reports of unreturned calls and problems related to transfers or discharges. Leadership is accused of lack of transparency, scapegoating, coverups, and misconduct, creating an intimidation culture with alleged retaliation against staff and families. Several reviewers explicitly state that accountability is lacking and raise the possibility of regulatory or closure risk.

Safety and basic operational issues appear repeatedly in the negative summaries. Reviewers report instances of neglect and delayed assistance, including a bed fall incident and more general poor staff responsiveness. There are alarming reports about unsafe physical equipment (bed rails wrapped in pipe insulation and tape), medication errors, and abuse reports. Families describe thefts (including a grandmother's wedding rings), lost or displaced personal items and clothing, and a broader pattern of items going missing. Such allegations, combined with reports of deaths during the COVID-19 lockdown, heighten concerns about resident safety, oversight, and infection control during crises.

Staff behavior is portrayed in both positive and negative lights. While some staff are described as friendly, attentive, and communicative, other accounts depict rude treatment and even overtly abusive language — one reviewer reported a staff member calling their husband "trash." These polarized descriptions suggest there may be significant variability in staff performance and conduct between shifts or specific employees, and reviewers also allege retaliation and intimidation that could suppress reporting of poor behavior.

Dining and daily environment receive critical notes from multiple reviewers: poor food quality is mentioned more than once and contributes to an overall sense of inconsistent day-to-day living standards. On the other hand, activity programs (exercise, music) and therapy are strengths noted by families, indicating that some programming and rehabilitative services are functional and well-regarded.

Overall sentiment is conflicted: while some families are satisfied with direct caregiving, therapy, activities, and communication, there are multiple serious systemic concerns about management responsiveness, safety practices, accountability, and staff conduct. The frequency and severity of negative reports — including theft, medication errors, abuse allegations, unsafe equipment, and leadership issues — are significant enough that prospective residents and families should investigate further. Recommended next steps for those considering this facility include asking for documentation of safety checks, staff training and turnover rates, incident and corrective-action records, medication administration audits, infection-control policies (especially regarding COVID-19 responses), and references from current resident families. For current family members, escalating concerns to regulatory bodies and requesting formal incident reports and management responses would be prudent given the nature of the allegations.
