    Neglectful care despite some kindness

    I had a mixed experience at Ascension Oaks: some staff (administrator Brett, the social worker and rehab team) were kind, activities were engaging, and parts of the building and rooms were clean. However I personally witnessed troubling neglect and unsafe care - residents left soiled or dehydrated, staff distracted (smoking, on phones), broken equipment, bruises/infections, theft of shoes/clothes, and little effective action from management. Because of these serious issues, I cannot recommend placing a loved one there.
