    Unreliable care despite friendly staff

I appreciated the knowledgeable tour, renovated rooms, plentiful activities, pleasant courtyard and mostly friendly, accommodating staff - it often felt home-like and well organized. But repeated cleanliness problems, noisy hallways, shared rooms, poor food/diet management (cold/inappropriate meals, diabetic diet ignored), medication errors (under-medicated by day, over-medicated at night), clothing mix-ups and an unresponsive director/administration forced me to lose confidence. Staff can be caring, but because of those serious care and communication issues I had to transfer my mother and cannot recommend this facility.

    Summary review

Overall sentiment across the review summaries is mixed, with a clear split between reviewers who praise the facility's environment, activities, and certain staff, and others who report significant care and administrative problems. Many reviewers commented positively on physical aspects of Twin Oaks Nursing & Convalescent Home: the facility is frequently described as clean and fresh, with large, comfortable rooms and included amenities such as private phones and DIRECTV. Several mentioned recent renovations and a home-like, welcoming atmosphere. Outdoor and communal spaces receive positive notes — an attractive wall decor, a well-maintained garden, and a beautiful courtyard with a fountain are repeatedly highlighted, along with pleasant indoor temperature control and reasonable visiting hours.

Activities and social life are consistently cited as strengths. Multiple reviewers praised a robust activities program (examples given include pep rallies, stage productions like 'The Birth of a King,' and a Mardi Gras Ball), regular volunteering opportunities, and occasions that encourage reminiscing and resident participation. These social offerings appear to contribute to a positive, family-like culture for some residents and families; reviewers describe the environment as fun, well organized, and supportive of engagement among residents.

Care quality and staff behavior are areas of considerable divergence. Several summaries report caring, knowledgeable, and accommodating staff, and some reviewers commend strong management and efficient operations. However, other reviews raise serious concerns about the consistency and reliability of care. Reported issues include under-medication during daytime hours and over-medication at night, insufficient assistance with toileting and eating, and instances where family members felt the administration was unresponsive or unprofessional. There are direct accounts of a resident being transferred away from the facility because of care problems, indicating that for at least some families the level of care fell short of expectations.

Dining and nutritional management emerge as recurring trouble spots in multiple summaries. Problems include meals being served cold — in particular when a resident requires assistance to eat — and failure to follow prescribed diets, notably diabetic meal plans. Reviewers cited inappropriate menu items for residents with dietary restrictions (cupcakes, pasta, potatoes), which raises concerns about monitoring and staff training in dietary needs. While some reviewers mentioned medication and nutritional monitoring as a positive, others contradicted this with concrete examples of mismanagement, suggesting inconsistency in execution.

Operational and administrative themes are similarly mixed. One set of reviewers reports strong management, efficiency, and a welcoming leadership presence, while others describe an unengaged director, unprofessional administration, and staffing attitudes that ranged from friendly to problematic (including mention of at least one staff member with a notable negative attitude). Logistical problems such as clothing mismanagement — mislabeled garments, clothes lost or mixed with others — and shared rooms/noise issues (residents yelling in hallways) were also recorded and contribute to concerns about day-to-day operational oversight.

Taken together, the reviews point to a facility with many appealing features — good amenities, an active social program, attractive grounds, and in many cases compassionate staff — but also notable and recurring care and administrative problems that have led some families to advise against the facility and, in at least one instance, to transfer a resident. The pattern suggests variability in experience: some residents and families report very positive outcomes, while others experienced lapses in medication administration, meal/dietary management, personal-care assistance, laundry/clothing handling, and responsiveness from leadership. Prospective residents and families should weigh both sets of feedback carefully and, based on the themes identified here, consider verifying specific operational details (medication schedules and monitoring, diabetes/diet handling, assistance at mealtimes and with toileting, laundry policies, and staffing consistency) during a tour or through direct conversations with management to determine whether the facility's strengths align with their care needs.

About Twin Oaks Nursing & Convalescent Home

Twin Oaks Nursing & Convalescent Home sits right in La Place, Louisiana, and's been taking care of folks since 1975, and you can see that from the way the staff keeps the place comfortable and clean. The building at 506 W 5th St has safety features like a full sprinkler system and handicap access, so people who need a wheelchair can get around, and rooms come with things like kitchens or kitchenettes, cable TV, washers and dryers, and are kept up with regular maintenance. Residents get access to a dining room, fitness center, wifi, a game room, salon, guest parking, and there's always someone to help with cleaning.

The staff provides help with walking, wheelchairs, bathing, toileting, dressing, grooming, laundry, and there's always someone nearby for personal care. Twin Oaks runs 24-hour skilled nursing, wound care, medication support, podiatry, occupational therapy, and transportation services. Meals are provided every day, and there are plenty of activities like social events, arts and crafts, education, and health programs. The facility accommodates both long-term residents and short-term guests who are recovering after a hospital stay, with a dedicated rehabilitation unit in its own area for folks working on getting back home.

They offer skilled nursing care for short-term rehab and long stays, and have therapy services up to 7 days a week for faster recovery. You see lots of family involvement because they encourage relatives to help out with care. Twin Oaks can take payment from Medicare, Medicaid, private insurance, managed care, and private pay, and they also accept long-term care insurance.

Twin Oaks got a score of 3.5 out of 6 reviews in one listing, but only a 1 out of 10 from a single review on another, so feedback is a bit mixed. The mission's to offer good medical care and rehab, and while the building's safe and the environment's pretty nice, every place has its good and bad days. Twin Oaks focuses on skilled nursing and rehabilitation, with a team of trained staff helping both seniors who plan to stay and those aiming to go back home after getting stronger.
