    Compassionate staff and excellent rehab

I'm very pleased with the compassionate, knowledgeable nursing and CNA staff and an outstanding rehab/therapy team that helped my loved one improve. The older building is clean and well maintained, activities (bingo, arts & crafts, on-site salon) are plentiful, and meals offer good options. Admissions and administration were responsive and caring - I would recommend this community.

    Summary of reviews

Reviews for Pontchartrain Health Care Center are sharply divided, with a subset of families describing consistently strong clinical rehabilitation, wound care, and a compassionate frontline staff, and another subset describing serious operational and clinical concerns. The facility’s rehabilitation program, including physical and occupational therapy, receives frequent praise for effectiveness and measurable progress. Several accounts highlight an on-site nurse practitioner and a capable wound-care team that led to successful outcomes. Nursing assistants and some licensed nursing staff are often described as caring and attentive, and families commonly mention helpful admissions staff, engaging activities (bingo, crafts), and on-site salon services that contribute to a family-like environment.

At the same time, an important pattern across reviews is variability in clinical oversight and day-to-day responsiveness. Multiple descriptions indicate delays in medication administration, long waits for nursing attention, and inconsistent nursing follow-through. These operational gaps have been associated with weight loss, skin-integrity concerns (pressure-related injuries), and other clinical complications in a number of accounts. Reviewers also point to equipment and safety-management problems — missing or broken rails, delayed access to mobility aids, and other maintenance-related safety issues — that can affect residents’ mobility and comfort.

Communication and administrative processes are recurring weaknesses. Families report inconsistent communication from staff and social workers, difficulties reaching reception or getting calls returned, delays in paperwork and belongings retrieval, and confusion about residency or referral requirements. There are also concerns about personal-property controls and account/billing handling. Facility infrastructure is described as aging in places: small, shared rooms in some wings, visible maintenance needs (ceiling tiles, wall upkeep), and variable cleanliness — while some areas are noted as clean and odor-controlled, others are described as having sanitation concerns.

Dining and activities elicit mixed feedback. Several reviewers praise flexible meal choices, menu alternatives, and appreciated kitchen staff; others describe inconsistent food quality and dissatisfaction with meals. The activities program and on-site services (beauty shop, social engagement) are generally seen as positive contributors to resident well-being.

Overall, the facility appears to offer strong rehabilitative care and demonstrably compassionate staff in many instances, but those strengths coexist with operational inconsistencies that have led some families to report significant care and safety concerns. Prospective residents and families should consider in-person visits, ask specific questions about medication administration protocols, staffing ratios and weekend leadership coverage, infection-control and skin-integrity monitoring, equipment and maintenance practices, and policies for family communication and belongings management. Doing so may help identify whether the facility’s current mix of clinical strengths and operational challenges fits the resident’s needs.

The building has dedicated spaces for rehabilitation, whirlpool units for therapy, and a Secure Care Safety System for added peace of mind. Transportation helps residents get to appointments. The nursing home runs an active Facebook page and has an online presence, so families can keep up with events and stay in touch. The facility has mixed reviews - 1.6 stars based on seven reviews - and a 5.1 out of 10 community rating, making it the fifth highest rated community in Mandeville on some lists. Staff aim to provide individualized treatment plans and encourage family involvement to support recovery and improve the chances of returning home after an illness or hospital stay.
