    Filthy, unsafe facility with neglect

I had a terrible experience - the building is decrepit, constantly reeks of urine/stool, and is dirty with mold, rust, pests and broken equipment. Staff and management were rude, unprofessional and disorganized: inaccurate records, wrong room numbers, ignored meds/call bells, denied visitation, and even theft or safety incidents. A few caregivers and therapists were genuinely caring, but they couldn't fix the neglect, health/code violations, and overall unsafe conditions. I would not send a loved one here.

    Summary review

Overall sentiment: The reviews paint a facility with a deeply mixed reputation but an overwhelming preponderance of negative reports. While many reviewers single out individual employees and departments for praise — particularly certain nurses, aides, and the rehabilitation/therapy team — the dominant themes are chronic understaffing, severe cleanliness and maintenance failures, safety and medical-care lapses, and management problems. Numerous reviewers explicitly urge regulatory inspection, closure, or avoidance, citing issues that range from unprofessional care to potential health-code and safety violations.

Care quality and clinical safety: A major pattern across the reviews is inconsistent and often poor clinical care. Multiple accounts describe neglect (residents left unbathed for weeks, untreated infections, bedsores allowed to worsen), medication mishandling (ignored meds, medication errors leading to diarrhea or other adverse effects), and critical incidents (dialysis machine mis-set reportedly causing a patient to pass out, transfers to hospital, deaths following apparent neglect). Several reviewers describe delayed responses to call bells, rough handling of patients (slamming, improper transfers), and lack of appropriate monitoring for vulnerable residents (patients left unmonitored, alarm bracelets not working or not responded to). Conversely, the rehab/therapy unit is frequently cited as a bright spot where patients experienced measurable improvement.

Staff performance, culture, and morale: Reviews are highly polarized about staff. Many individual caregivers are described as compassionate, helpful, and hardworking; specific staff members are named and lauded for going above and beyond. At the same time, a significant number of reviews report rude, dismissive, or even hostile staff — often naming particular employees as problematic — and describe a toxic culture and poor leadership. Chronic understaffing is reported repeatedly (some say the facility is short 75% of staff much of the time), contributing to burnout, rough or inattentive care (GNAs sleeping during night hours), and frequent lapses in basic services. Several reviewers allege staff drinking on the job, sleeping, or otherwise failing to meet basic duties. Communication failures are common: families report difficulty reaching nurses, social workers, or administrators, inaccurate records, and poor handoffs between departments.

Facility condition, cleanliness, and pests: Physical plant complaints are among the most consistent and severe. The building is frequently described as run-down and in need of overhaul: peeling paint, rusted fixtures, black mold in rooms, hole in elevator floor, broken elevators, stained and dingy linens, and air mattresses in place of proper beds. Overpowering urine and fecal odors, dirty rooms, unclean bathrooms, and reports of feces or bodily fluids on floors create concerns about basic sanitation. Pest problems are repeatedly mentioned, including rats, roaches, bed bugs, flies, and gnats. Several reviewers explicitly linked these conditions to potential health-code violations and urged closure or condemnation.

Safety, theft, and on-site illegal activity: Safety concerns extend beyond environmental issues to incidents of theft and illicit activity. Multiple accounts allege theft of personal items (brand-new wheelchair, clothing, money), frequent disappearance of residents’ belongings, and staff or resident collusion in theft. Reviews also allege drug use and distribution on site (residents smoking crack or weed, activities coordinator alleged to provide cigarettes for PCP), creating an unsafe environment for vulnerable residents. Concerns about fire and elevator safety, expired safety certifications, lack of an escape route for upper floors, and frequent falls or hospitalizations further compound safety worries.

Dining and housekeeping services: Dining is frequently criticized for poor quality (descriptions include ‘slop’, bland or cold meals, flies at mealtime), missed meals, and failure to adhere to prescribed diets. Housekeeping is reported as inconsistent: some reviewers appreciate daily cleaning staff, while many others report unwashed linens for weeks, dirty rooms, and unsanitary dining areas. Linen shortages, failure to provide gowns, and soiled bedding show systemic supply or management problems.

Management, administration, and finances: Many reviews allege systemic management failures: toxic leadership, poor responsiveness to complaints, HR and administrators described as dismissive or arrogant, and uncoordinated or dishonest communication (wrong room numbers, inaccurate records). Several reviewers describe contentious billing issues or alleged coercive financial practices (keeping social security funds, extra charges around $700), and some indicate regulation or health department involvement with little apparent resolution. Multiple reviewers report a decline in quality after management changes, suggesting problems with leadership transitions and oversight.

Activities, social life, and resident experience: Reports on activities and social engagement are mixed. Some residents and relatives mention enjoyable recreation (60’s music, parties, television, a recreation room) and positive social connections. However, many reviews say there is little mental stimulation, few structured activities, and isolation (including quarantine practices during COVID that felt punitive or secretive). The combination of understaffing and limited programming appears to exacerbate loneliness and decline for long-term residents.

Patterns of extremes and variability: A notable theme is the extreme variability of experiences. While many reviewers issue strong warnings to avoid the facility entirely (citing hygiene, abuse, theft, and safety), others praise specific caregivers, successful rehab outcomes, or express genuine satisfaction and pride in the facility as home. Several reports indicate improvement over time or better experiences in particular units (especially rehab), suggesting that quality may vary greatly by unit, staff on duty, or timing. Nonetheless, the volume and severity of the negative reports — including allegations of neglect-related hospitalizations and deaths, pest infestations, and possible code violations — are substantial and recurrent.

Implications and takeaways: Based on the reviews, the Clinton Healthcare Center exhibits significant systemic issues in maintenance, staffing, safety, and management, punctuated by pockets of strong, compassionate care (notably in rehab and among certain named staff). The most urgent concerns reported are sanitation and pest infestation, theft and illicit activities on site, medical neglect and medication/dialysis errors, and chronic understaffing that undermines basic resident care. Many reviewers explicitly call for regulatory inspection, corrective action, or facility closure. For families considering this facility, these reviews suggest the importance of in-person visits, asking for up-to-date inspection records, clarifying staffing ratios and safety protocols, verifying how complaints are handled, and monitoring individual caregivers and units closely. For regulators or advocates, the pattern of allegations — especially those describing severe neglect, infection control failures, and safety-code lapses — indicates grounds for investigation.

In summary, while individual caregivers and certain units receive strong praise for their dedication, the overall body of reviews signals pervasive and serious problems with cleanliness, safety, staffing, management responsiveness, and consistent clinical care. The pattern is one of high variability with many deeply concerning reports that families and oversight bodies should not ignore.
