    Compassionate staff excellent therapy clean

I love it here - five stars. The staff are compassionate, long-tenured and great at communicating with family; therapy/rehab (in- and outpatient) helped after my stroke. The food is excellent, rooms and grounds are very clean (AL has large rooms; nursing rooms are smaller), bird feeders and a neat pavilion make outdoor time lovely. Plenty of activities, TV in every room, free Wi-Fi, on-site beautician, Medicare/Medicaid accepted. Only downside: overnight staff can be minimal. I'm very happy and would highly recommend.

    Summary review

Overall sentiment in the reviews is strongly positive. Multiple reviewers specifically praise the quality of care, the compassion of the staff, and the facility's cleanliness. Families report good communication from registered nurses and other departments, and several reviewers emphasize gratitude for staff support during significant health events (for example, rehabilitation after stroke). The facility’s rehabilitation services (inpatient and outpatient) and local therapy options are noted as effective and accessible. Long-tenured staff and reports that staff are improving over time are cited as contributing to continuity of care and a stable environment.

Staff quality and interpersonal care are recurring strengths. Reviewers consistently describe staff as friendly, competent, compassionate, and attentive. Many express high trust in the caregiving team and say they would highly recommend the facility. At the same time, the most commonly reported operational concern focuses on overnight coverage: several reviews say overnight workers tend to do the minimum, which suggests a perceived difference in responsiveness or engagement during night shifts compared with daytime staff.

Facility and accommodations receive mixed-but-leaning-positive feedback. The building is characterized as older but well-maintained; reviewers frequently note that the facility is clean and has been recently remodeled in places. Amenities such as TVs in every room, free Wi‑Fi, and updated private and double rooms are appreciated. A notable pattern is the distinction between the nursing home and the attached Assisted Living: nursing home rooms are described as small, whereas the Assisted Living portion offers much larger rooms. Outdoor features (awnings, a pavilion, bird feeders outside windows) and on-site beauty services add to the perceived quality of life.

Dining and activities are highlighted as strong points. Multiple reviewers explicitly state that residents "love the food," and they mention meal options and flexible dining as positives. The activities program is active and varied—bingo, outings, live entertainment, and other engagement opportunities are repeatedly cited. Reviewers report that residents participate in activities and appear happy with the social and recreational offerings.

Administration and policy details mentioned in reviews further support a positive picture: the facility is Medicare/Medicaid certified, does not charge for bed hold, and operates as a not-for-profit. These operational characteristics, combined with transparent communication from staff and the availability of rehabilitation and beauty services on site, contribute to reviewers’ overall satisfaction and frequent recommendations.

In summary, Prairie Manor Care Center is portrayed by reviewers as a clean, compassionate, and well-run facility with strong clinical and rehabilitation services, good food, active programming, and many amenities that support resident comfort and family communication. The most consistent concern is the perceived minimal engagement from overnight staff, and the nursing-home room sizes are noted as small compared with the attached Assisted Living. Those trade-offs aside, the dominant theme is high satisfaction—residents and families repeatedly recommend the facility and praise the staff and services.

    About Prairie Manor Care Center

Prairie Manor Care Center sits in a small town and has been caring for older adults since 1966, and you'll find it right next to Heather Haus, which is a connected assisted living building with 20 units, so folks often move between the two when their needs change, and everything feels more like a little neighborhood than a big institution, which lots of people seem to like, since it's clean, home-like, and meant to be friendly. The skilled nursing facility offers 36 to 40 beds, depending on the year, and you'll find both private and semi-private rooms for people who need a lot of help, such as 24-hour nursing, medication management, and personal care like bathing, dressing, and feeding, all under the care of licensed staff, therapy teams, and assistants who have training, and they always try to be helpful and welcoming, sticking to the motto "Quality Care with a Small Town Touch."

Staff provide lots of services, such as physical therapy, speech therapy, and occupational therapy-right on-site for both those staying at Prairie Manor and also folks who just need outpatient therapy for recovery, muscle strengthening, or issues like trouble swallowing, memory, or balance. There's a dedicated memory care unit for those with dementia or Alzheimer's, where they use special safety features like secure doors and fall prevention tools, and offer routines that help keep people comfortable, engaged, and safe from wandering or confusion, including activities that are social or mental, which have even won some awards over the years for quality and engagement. There's hospice and palliative care for those nearing end of life, and long-term care for folks who need ongoing attention, making sure comfort, privacy, and respect for each person's choices remain front and center, no matter the condition.

Meals come from a kitchen with a chef and planning team that focuses on nutrition, taste, and variety, so food isn't overlooked, and events, outings, movie nights, and trips to fairs or malls try to keep people busy and happy, with transportation arranged for many planned activities, and religious services and social visits allowed at any time of day or night, since they have a 24-hour guest policy. There's a real effort to make life pleasant, from clean rooms and modern amenities (like Wi-Fi and common areas) to staff who work hard to keep things safe, friendly, and welcoming, with a feel that comes from being run by local churches but operated day-to-day with a good amount of independence. Prairie Manor offers care for the elderly, disabled, or anyone needing skilled nursing or assisted living services in Blooming Prairie, Minnesota, especially for those who want a smaller place with a personal touch and support with daily living tasks or ongoing therapies.
