    Poor care communication management issues

I placed a loved one here and, sadly, my experience was mostly negative. A few staff genuinely cared and the place has seen some improvements, but communication and management are poor: reception often doesn't answer, staff are frequently on their phones, and residents were at times neglected (left in soiled diapers, unbathed). I also heard reports of missing tip money and saw high turnover and disrespect from leadership. Because of these issues I cannot recommend this facility.

    Summary review

Overall impression: The reviews of Centennial Gardens for Nursing and Rehabilitation are deeply mixed and highly polarized. A significant portion of reviewers praise the facility for warm, attentive care, good rehabilitation results, and a welcoming atmosphere; these reviews highlight dependable, caring nurses and aides, a friendly front desk, successful discharges, and improvements since new ownership. Conversely, a substantial number of reviews contain serious, specific allegations of neglect, abuse, theft, and administrative failures. The combination of glowing endorsements and harsh condemnations suggests large variation in experience depending on timeframe, unit, or individual staff on duty.

Care quality: Positive reports repeatedly emphasize compassionate caregivers, staff who ‘‘put smiles on residents’ faces,’’ clinicians trained to manage multi-level patient needs, successful rehab outcomes, and comfort for long-term residents. Families reporting good experiences mention respectful treatment, helpful activities, and effective clinical discharge planning. However, the negative reports raise very serious concerns about basic caregiving fundamentals: multiple reviewers allege residents were left in soiled diapers for hours, were not bathed, had matted hair, or otherwise suffered neglect. There are also allegations of maltreatment, screams for help, and at least one report connected to a delayed notification after a resident’s death. These are not isolated minor complaints but fundamental safety and dignity issues, so they materially affect the facility’s risk profile for prospective residents.

Staff performance and communication: Many reviewers single out individual caregivers and nursing assistants as caring and responsive, while others complain of staff frequently using phones, chatting, or being ‘‘lazy’’ and unresponsive. Communication is another bifurcated theme: some families describe staff as responsive to questions and concerns, while others report poor communication overall, receptionist phone lines unanswered, language barriers, and delayed or absent notifications about critical events. Allegations of favoritism, management tolerance of poor behavior, and insufficient oversight compound concerns about consistency of care. There are also multiple reports alleging theft—tips pocketed at the front desk, nurses stealing from clients, and fraudulent billing—alongside claims that management posts or pays for suspiciously positive reviews; these raise governance and trust issues that families should verify independently.

Facilities, environment, and resident mix: Several reviewers note interior improvements, a welcoming atmosphere, and comfortable spaces. Others describe a less favorable physical environment—bare rooms missing amenities such as TV, radio, or phone, an uninviting exterior, and areas described as a ‘‘dump.’’ A notable operational concern is the reported mixing of patient populations: at least one reviewer said their room was on a ward with mental health patients exhibiting yelling, banging, and room entry at all hours, which can create noise and safety issues for other residents. This suggests variability between units or a lack of clear separation of acuity/types of care, and prospective residents should confirm which unit they would be placed in.

Management, staffing, and culture: Reviews indicate a volatile organizational picture. Some praise ‘‘new ownership’’ and improvements, while others say new management has been disrespectful to long-tenured staff and that leadership treats workers poorly. High nursing staff turnover and staffing shortages are recurring complaints and are often tied to reports of inconsistent care. Several reviewers also question the authenticity of positive reviews and allege management influence over posted ratings. Taken together, these signals point to potential leadership and culture problems that could affect continuity of care and accountability.

Patterns and recommendations: The dominant pattern is extreme variability—some families describe Centennial Gardens as an excellent nursing and rehab option with caring staff and good outcomes, while others relay serious allegations of neglect, abuse, theft, and mismanagement. Because the negative claims involve safety, hygiene, and possible financial misconduct, they warrant immediate attention. For families considering this facility, verify the following before placement: recent state inspection reports and deficiency history; staffing ratios and turnover metrics for the specific unit; policies and documentation for abuse/neglect reporting and how the facility communicated adverse events; copy of the billing statement and an explanation of any third-party charges; a guided tour of the exact room/unit where the resident would stay (including amenities and noise levels); and direct conversations with current residents’ families on that unit. If any allegation of abuse or theft is suspected, families should contact adult protective services and the state long-term care ombudsman to confirm whether complaints or investigations exist. Given the severity and frequency of negative reports alongside positive testimonials, due diligence and careful, unit-level questioning are essential before deciding on Centennial Gardens.
