    Compassionate staff, serious safety issues

    I have mixed feelings. Many nurses and CNAs were kind, professional and made my loved one comfortable in a home-like apartment with friends and good meals, and at times the care felt like a godsend. But communication and safety were seriously flawed - wrong phone numbers, unresponsive staff, missed meds, discharge confusion, understaffing, bruises/skin tears, a fall and even a death - and I was effectively penalized for reporting concerns. Beautiful facility and some wonderful staff, but visit unannounced and be vigilant.
