    Compassionate post-surgical rehab and care

I placed my uncle here after brain surgery and I'm very grateful - compassionate, respectful staff provided excellent post-surgical rehab (OT/PT/speech), comfort and hospice care with dignity. The facility is clean, well-maintained and new-feeling, communication was strong and family visits were welcomed. Overall a very good, reassuring place I'd recommend.

    Summary of reviews

Experiences at this facility are mixed, with distinct clusters of positive post-acute and long-term care outcomes alongside operational concerns that affect some families. Many accounts highlight strong clinical rehabilitation capabilities — weekday OT/PT/speech services, effective post-surgical rehab, and coordinated transitions for patients discharged from hospitals. Several families specifically praised compassionate nursing and aide care, dignified end-of-life and hospice support, and staff who communicated well and provided individualized attention.

At the same time, there is a pattern of operational weaknesses that families described as affecting day-to-day care. Staffing levels and turnover were frequently mentioned, and reviewers linked these shortages to delays in assistance, inconsistent delivery of ADL support, and variability in the tone or responsiveness of front-line staff. Communication and administrative systems also appear uneven: examples included missing paperwork, fax and record-transfer failures, and difficulty reaching staff by phone. These issues created uncertainty during admissions and care transitions for some families.

Housekeeping, sanitation, and pest-control emerged as specific areas of concern for some reviewers. While other families described clean, well-maintained common areas and a newer building addition, a subset reported inconsistent cleaning of resident rooms and shared bathrooms and flagged the need for more reliable pest-control measures. Meal service likewise showed variability: several reviewers praised food quality, but others described late lunches or interrupted meal delivery.

Facility amenities and community aspects are generally viewed positively. The facility has a newer addition, larger event rooms, and an environment that many families described as friendly and conducive to visits and volunteer activities. Family engagement through visiting, music, and volunteer-led programs was noted as a benefit that contributed to resident well-being.

Management and oversight present a mixed picture. Some reviews praised responsive leadership and good communication; others pointed to turnover in key roles (including social work and nursing leadership), payroll/operational concerns, and gaps in clinical supervision. These management-level inconsistencies appear to correlate with the variability in frontline performance, housekeeping, and administrative reliability.

In summary, prospective residents and families should weigh the facility’s demonstrated strengths in rehabilitation, compassionate bedside care, hospice coordination, and updated physical plant against recurring operational issues: staffing instability, uneven communication and record-keeping, inconsistent housekeeping and pest-control, and occasional interruptions in meal-service continuity. Those prioritizing intensive rehab and supportive bedside staff report positive outcomes; those for whom consistent housekeeping, dependable communication, and uninterrupted ADL assistance are critical may want to confirm current staffing, administrative processes, and sanitation/pest-control protocols before placement.

    About Pine Haven Care Center

Pine Haven Care Center sits in Pine Island, Minnesota, and it's been serving seniors since 1964, giving folks a place that feels warm and homelike. The center offers different types of care, including assisted living at Evergreen Place, which has 24 apartments you can decorate to make your own; these apartments come in one- and two-bedroom styles, with private and shared options, full kitchens, large windows, individual climate controls, and sizes that range from 350 to 636 square feet, depending on the floor. Assisted living services at Evergreen Place help with daily activities like bathing, dressing, and medication, and there's always someone on staff, with a registered nurse available 24/7, and you can use extra help for things like meals or laundry for an added fee, and there's also scheduled transportation, meal delivery, a communal dining room with a fireplace, a lounge for gatherings, and complimentary laundry.

The campus includes Pine Haven Care Center, which has 70 skilled nursing beds, so it can care for folks who need short-term rehab or long-term help, and it provides memory care for people with Alzheimer's or other dementias, using therapies and activity programs tailored to memory support. Physical, occupational, and speech therapy happen right there, too, and people recovering from injury or illness can use these services to work on getting better. Home care services with trained aides help out with companionship and non-medical things, and there are residential care homes for seniors who prefer living with a caregiver in a smaller, neighborhood house.

Residents get nutritious meals prepared with quality ingredients, and if someone needs or wants spiritual support, chaplain services are available. The staff has a wide team, including an Interim Administrator, Interim Director of Nursing, Social Services Director, Environmental Services Director, Dietary Director, Activities Director, Occupational Therapist, Physical Therapist, Housing Director, and Human Resources Manager, so there's always someone to help with health, happiness, and daily needs. The care team builds person-centered and unique plans for each resident, always involving family in the process. Pine Haven keeps people's independence and freedom of choice in mind, while supporting dignity and encouraging friends and family to stay involved.

The campus offers respite care for those who only need a short stay, palliative care for serious illnesses, hospice coordination, and social work support for advocacy and well-being. Activities and events happen every day, guided by a campus coordinator and volunteers, and the schedule changes depending on what the residents want. There's also a beauty shop at the adjoining care center, garages for rent, pet-friendly rules with some restrictions, and all utilities included except telephone, along with basic cable TV, weekly light housekeeping, and a 24-hour emergency system. Pine Haven Care Center is part of Pine Haven Community and works as an equal opportunity provider and employer, not discriminating based on handicap. The Pine Haven Foundation supports its programs, always focusing on comfort, quality of life, and celebrating what makes each person unique.
