    Warm residents; staffing shortages concern

I toured the facility and liked that residents were warm, smiling and well groomed, the place smelled clean, staff were friendly, attentive and professional, activities like bingo kept people engaged, and visible safety measures (new security, screening, masks, disinfectant) were in place. Rehab after a stroke looked solid and several staff (Trisha included) treated residents with dignity. My main concerns are chronic CNA/staffing shortages, teamwork issues and occasional reports of rude or mistreating staff and a weak director - I'd consider this place for a family member, but cautiously.

    Summary review

Overall sentiment across the reviews is more positive than negative, with repeated praise for the hands-on caregiving, cleanliness, and social atmosphere at Ashley Manor Health & Rehabilitation. Multiple reviewers specifically called out nurses and other staff as caring, respectful, polite, and professional. Staff interactions with families are described as helpful and respectful, and reviewers noted that staff make families feel welcome rather than a nuisance. Residents are frequently described as well groomed and well taken care of, and the facility is repeatedly characterized as clean and tidy with a more pleasant odor than other care homes.

Safety and infection-control practices are another consistent strength in the reviews. Commenters mentioned a new security system, a health screening machine, masks being available, and disinfectant onsite — all pointing to an institutional emphasis on safety and infection prevention. This is reinforced by observations that the facility smells clean and that residents appear kempt and comfortable. Rehabilitation care was specifically mentioned in a positive light by a reviewer whose family member received post-stroke rehab, indicating competent clinical and therapy services for some residents.

Social life and resident engagement are highlighted as positives as well. Activities such as bingo and general resident participation paint a picture of an active, engaging environment where residents enjoy themselves. Multiple reviews describe warm, inviting interactions, smiling residents, laughter, and staff who are engaged with residents — elements that contribute to a welcoming atmosphere and a sense of community.

However, there are nontrivial concerns that recur in the reviews. Staffing shortages, particularly an insufficient number of CNAs, are mentioned repeatedly; reviewers express a need for better staffing levels and improved teamwork. Problems with certain staff behaviors are also reported: a few reviews describe staff who were rude to patients or who teased residents about medications. One review indicated the family ultimately removed their grandmother from the facility due to mistreatment. These reports suggest variability in staff conduct and supervision.

Management and environment receive mixed feedback. While some reviewers described the facility as well managed, at least one review described the director as "not the best," indicating concerns about leadership. Additionally, the surroundings or exterior of the facility were described as "not the best" by one reviewer, suggesting that while internal cleanliness is praised, the broader physical environment may need attention.

In summary, Ashley Manor appears to offer solid day-to-day care characterized by cleanliness, respectful and attentive staff, meaningful resident activities, and visible safety measures. The most significant weaknesses identified are staffing shortages, isolated incidents of poor staff behavior and mistreatment, and some leadership or facility-surroundings concerns. Prospective families should weigh the generally strong personal care and engagement against the reported variability in staffing and occasional negative staff conduct; asking direct questions about staffing ratios, supervision, and how complaints are handled would be prudent steps for those considering placement.

About Ashley Manor Health & Rehabilitation

Ashley Manor Health & Rehabilitation sits in Boonville, Missouri, and has 52 certified beds with about 44 residents living there most days, and you'll find they give both short-term rehab and long-term care for seniors who need help with daily living or have special health needs, and they run things with help from JMS Senior Living, a known group of long-term care places, also working with Reach Ltc and professionals like Kellie M Asher, DPT. Chef-made meals use local, seasonal food and can be tailored to a resident's needs and wants, while in-house salon care, free Wi-Fi, and transportation make daily living easier, and rooms and shared spaces are set up for comfort, safety, and independence, with attention to mobility. The facility offers skilled nursing care, therapy for physical, occupational, and speech or language needs, along with memory care, home care, hospice, home health, and support or referral services, and you'll see a therapy and recovery area meant for rehab. An interdisciplinary care team works with each resident to make care plans that fit unique needs, and nursing staff, while present, have hours below the state average-about 3.16 hours per resident per day, and nurse turnover has reached 61.4%, which is higher than the state average. They aim for a professional setting with compassion and support, but reviews from January 15, 2025, and past years have noted some problems-like not enough nursing staff at times, issues in infection control, and failures in keeping things clean and safe for everyone-leading to federal standard violations and 25 cited deficiencies across inspections, with 4 involving infection control, and one infection-related problem noted in the most recent report, so while residents can expect a broad set of care options and plenty of amenities, there are some things with staffing and cleanliness they need to keep working on, and the administrator running the place is Heather Johnson.
