Assisted Living in Missouri

• Age 65 or older, blind, or disabled
• Missouri resident and U.S. citizen or qualified non-citizen, with a Social Security number
• Income and assets under the limits below. Your home, one vehicle, and household goods do not count as assets. People who are blind have higher income limits: $1,330 single and $1,804 for a couple
• If your income is over the limit, you may still qualify through a spenddown, where you pay part of your medical bills each month before coverage starts

Apply online, by phone, by fax to 573-526-9400, or at a local Family Support Division Resource Center. If you are 65 or older, also send the Supplemental Form. Have proof of identity, citizenship, income, bank balances, life insurance cash values, and any property you own ready, since the Family Support Division will ask for them.

• Age 65 or older, or age 21 or older and blind or disabled
• Live in a licensed assisted living or residential care facility
• Missouri resident and U.S. citizen or qualified non-citizen
• Income less than the facility's monthly base rate. For couples, each person's income is tested on its own
• Eligible for MO HealthNet, with assets under $6,220.50 (single) or $12,441 (couple)

Fill out the MO HealthNet Application Addendum and check the Supplemental Nursing Care box. Submit it online or at your local Family Support Division Resource Center. You should hear back within 45 to 90 days.

• Age 63 or older for the Aged and Disabled Waiver
• Eligible or likely eligible for MO HealthNet
• Score 18 or higher on the state's nursing facility level of care assessment, done in person by the Department of Health and Senior Services
• Monthly income under $1,737 and assets under $6,220.50. No spenddown is required

If you already have MO HealthNet, call 877-304-7939. Otherwise, apply for MO HealthNet first. You, a family member, or a friend can also send a referral to the Department of Health and Senior Services online or by phone.

• Age 65 or older, or younger with a qualifying disability or end-stage renal disease
• Part A has no monthly premium if you or your spouse worked and paid Medicare taxes for at least 10 years

Apply online through Social Security, by phone, or at a local Social Security office. If you already get Social Security when you turn 65, you are usually signed up automatically.

• Enrolled in Medicare Part A and Part B
• Live in the plan's service area
