    Does Alaska Medicaid (the ALI Waiver) cover assisted living in Alaska?

    Alaska Medicaid does not pay for the room-and-board portion of assisted living anywhere, but it can help pay for the care a resident receives.

    The main pathway is the Alaskans Living Independently (ALI) Waiver, a Home and Community-Based Services waiver run by the Division of Senior and Disabilities Services. It is for adults 21 and older who need a nursing-facility level of care but choose to live in the community instead. The ALI Waiver can pay for residential supported living in a licensed assisted living home, along with services like care coordination, respite, chores, meals, and transportation.

    To qualify, a person generally must:

    • Be financially eligible for Alaska Medicaid, and
    • Meet the nursing-facility level-of-care standard through a state assessment.

    The ALI Waiver has a limited number of slots, so a waiting list can form. For a less intensive option, the Medicaid Personal Care Services (PCS) program can help with daily activities for people who do not need nursing-home-level care. To get started, contact an Aging and Disability Resource Center or the Division of Senior and Disabilities Services; apply for Medicaid through the Division of Public Assistance.

    Does Medicare cover assisted living in Alaska?

    No. Medicare does not pay for assisted living room and board or for ongoing custodial (personal) care, which is the help most assisted living residents need day to day.

    Medicare may still cover related medical care, such as:

    • Doctor visits and hospital stays
    • Short-term skilled nursing or rehabilitation after a qualifying hospital stay
    • Medicare-covered home health and hospice when eligible

    Assisted living laws and regulations in Alaska

    In Alaska, assisted living communities are licensed as assisted living homes by the Department of Health, Division of Health Care Services, through its Residential Licensing unit. State statutes and regulations (found in Alaska's Administrative Code, Title 7) generally require a license for homes serving three or more unrelated adults, or any home that receives state or federal payment for services.

    State rules set standards for resident admission and assessment, residential services agreements, staffing and training, medication management, safety, and residents' rights. Residential Licensing inspects homes, issues notices of violation when standards are not met, and investigates complaints about care, safety, abuse, neglect, and financial exploitation.

    Inspection results and complaint records for licensed homes are public information. Before choosing a community, families can confirm that a home is currently licensed and ask about its inspection and complaint history. Verify a home's license through the state's official assisted living licensing page, which publishes a current list of licensed homes, or by contacting Residential Licensing directly.
