Paying for assisted living in Alabama

Most Alabama families pay for assisted living out of pocket, at least at first. Common sources include savings, monthly income such as Social Security or a pension, proceeds from selling a home, long-term care insurance, and help from family. Costs vary widely depending on the community, the size of the apartment, and how much daily care a resident needs. You can compare current pricing for communities across the state in the pricing tables below.

Because assisted living is largely private-pay, it helps to plan early and ask each community exactly what its monthly rate includes and which services cost extra.

Does Alabama Medicaid's Elderly and Disabled Waiver cover assisted living in Alabama?

Alabama Medicaid does not pay for room and board in an assisted living community, and the state does not have a Medicaid program designed specifically to fund assisted living. However, the Elderly and Disabled (E&D) Waiver — a Home and Community-Based Services waiver run with the Alabama Department of Senior Services — can pay for certain care services that help people who would otherwise need a nursing home remain in a community setting.

    Covered services may include:

    • Personal care and help with daily activities
    • Homemaker and companion services
    • Respite care for family caregivers
    • Adult day health and case management

To qualify, an applicant generally must be an Alabama resident, meet Medicaid's income and asset limits, and require a nursing-home level of care. You apply through the Alabama Medicaid Agency; a caseworker reviews both financial and medical eligibility.

Does Medicare cover assisted living in Alabama?

No. Medicare does not pay for assisted living room and board or for ongoing custodial care (help with bathing, dressing, and other daily activities), in Alabama or any other state.

Medicare may still cover medical care a resident receives while living there — for example, doctor visits, hospital stays, prescription drugs under Part D, and short-term skilled nursing or rehabilitation after a qualifying hospital stay.

For free, unbiased help understanding what Medicare covers, contact Alabama's State Health Insurance Assistance Program (SHIP), offered through the Alabama Department of Senior Services at 1-800-AGE-LINE (1-800-243-5463).

VA Aid & AttendanceA pension supplement for wartime veterans and surviving spouses who need help with daily activities. It adds a monthly amount on top of a VA pension that can be applied toward assisted living.
Long-term care insuranceIf a policy is already in place, it may cover much of the monthly cost. Check the daily benefit, elimination period, and any assisted living restrictions before relying on it.
Reverse mortgage (HECM)Homeowners 62+ can borrow against home equity, with no monthly payments due until the home is sold. Most useful when one spouse stays in the home.
Life insurance conversionSome whole-life policies can be sold or converted into a long-term care benefit while the policyholder is still living.
Home sale or bridge loanProceeds from selling a home — or a short-term bridge loan while it's on the market — can fund care during a transition.

Assisted living laws and regulations in Alabama

Assisted living communities in Alabama are licensed and regulated by the Alabama Department of Public Health (ADPH) under the authority of the State Board of Health, through its Bureau of Health Provider Standards. The state licenses two categories — standard assisted living facilities and specialty care assisted living facilities, which serve residents with conditions such as Alzheimer's disease and other dementias.

State rules set minimum standards for staffing, staff training, resident admission and discharge, medication assistance, food service, fire safety, and the physical building. ADPH conducts inspections and surveys, and it investigates complaints. Records of a facility's license status and inspection or survey history are public.

Before choosing a community, confirm it holds a current license and review its inspection history using the official ADPH Health Care Facilities Directory. Checking a community's record is one of the most important steps a family can take.

You can read the full rules in Alabama Administrative Code Chapter 420-5-4 — Assisted Living Facilities, maintained by the Alabama Legislative Services Agency.

Frequently asked questions about assisted living in Alabama

Alabama Medicaid does not pay for room and board in assisted living, and the state has no Medicaid program built specifically for assisted living. However, the Elderly and Disabled (E&D) Waiver, a Home and Community-Based Services waiver, can cover certain care services such as personal care, homemaker help, respite, and case management for eligible residents who need a nursing-home level of care. You apply through the Alabama Medicaid Agency.

Costs depend on the community, the apartment size, and how much daily care a resident needs, and they vary from one region of Alabama to another. Most families pay privately at first using savings, monthly income, long-term care insurance, or proceeds from selling a home. See the pricing tables below for current rates at specific communities.

Assisted living provides housing, meals, and help with daily activities like bathing, dressing, and medications for people who are largely independent. A nursing home (skilled nursing facility) provides round-the-clock skilled medical and nursing care for people with more serious health needs. Alabama licenses these as separate facility types with different rules and costs.

To receive help through the Elderly and Disabled Waiver, a person generally must be an Alabama resident, meet Medicaid's income and asset limits, and require a level of care similar to what a nursing home provides. Both financial and medical eligibility are reviewed. Contact the Alabama Department of Senior Services at 1-800-AGE-LINE (1-800-243-5463) or the Alabama Medicaid Agency to begin.

Use the Alabama Department of Public Health's Health Care Facilities Directory to confirm a community holds a current assisted living license and to review its record. You can search by facility type, county, and city. License and inspection information is public, and checking it is one of the most important steps before choosing a community.

No. Medicare does not pay for assisted living room and board or for ongoing custodial care anywhere, including Alabama. It may still cover medical services a resident receives, such as doctor visits, hospital stays, Part D prescriptions, and short-term skilled care or rehab after a qualifying hospital stay.
