    Understaffed facility with unacceptable care

I moved my dad here and felt torn - the building, activities, location, and many staff are friendly and caring, but overall care was inconsistent and often unacceptable. CNAs and nurses could be kind and helpful, yet the place is clearly understaffed: slow or no response to call lights, delayed baths/toileting, and buck-passing. Medical care was worrying - untimely pain meds, medication mix-ups/wrong meds, doctors slow to respond, hygiene neglect, and safety/cleanliness lapses (odors, bedsoiled sheets). Food and equipment felt minimal/outdated, management communication and billing were chaotic, and unannounced room moves made things worse. I cannot recommend this facility without major staffing and management changes.

    Summary review

Overall sentiment is deeply mixed and highly polarized: a substantial number of reviews describe excellent, compassionate care—particularly in the therapy/rehab services and from specific named staff—while another substantial cluster of reviews alleges serious, systemic failures including neglect, medical errors, and abusive or unprofessional behavior. This split suggests wide variability in resident experience that may depend on shift, unit, recent management changes, or individual staff members.

Care quality: Reviews range from outstanding rehab and measurable recovery progress to reports of severe medical neglect. Positive reviewers frequently highlight rapid functional improvements in rehabilitation, strong discharge planning, and nurses or therapists who actively manage medications and recovery plans. Conversely, negative reports include delayed or missed hygiene care (residents left soiled, showers not provided for extended periods), medication mix-ups, inadequate catheter and wound care, pressure wounds and possible sepsis, dehydration and electrolyte imbalance, and even allegations of death tied to facility care. Several families described needing to intervene or move residents due to perceived declining health or safety risks.

Staff and staffing: Staff are the central point of contention. Many reviews praise individual employees (nurses, aides, social workers, and administrators such as Amy, Jason, Cheryl, and D Anthony) as compassionate, responsive, and highly skilled. The therapy department and some aides receive repeated commendation. At the same time, numerous reports describe chronic understaffing, long call-light response times, CNAs perceived as lazy or unprofessional, staff texting or being distracted, rude front-desk behavior, threats of police when families attempted to access rooms, and episodes of staff refusing entry or service. Those negative accounts often allege that resource shortages lead to corners being cut and resident neglect.

Facilities and cleanliness: Several reviewers report a clean, well-maintained facility, attractive courtyards, and pleasant communal spaces. However, other reviewers mention strong odors, unsanitary conditions, bedsoiled sheets, and an outdated physical plant in parts of the facility. There are isolated but serious claims—such as a storage room being used as a resident room—that raise red flags about facility oversight and regulatory compliance.

Dining and amenities: Opinions on dining are split. Some families praise good food, complimentary dinners, and attentive meal service, while others describe minimal, cold, or unacceptable meals (examples include untouched cold lasagna and hurriedly dumped food). Activities programming is a recurring positive: reviewers commonly note bingo, live music, monthly calendars, and social engagement that keeps residents active.

Management, communication, and administration: A major theme in negative reviews is poor management and communication—frequent administrative turnover, unreturned calls, lack of transparency about clinical changes (such as moving a patient for dialysis without timely updates), unannounced room moves, and lost or mishandled resident belongings. Financial and operational concerns appear in several reports: unpaid vendors, partial or delayed payments, and allegations the ownership is attempting to sell the company. Positive reviews sometimes mention recent managerial improvements and better operations under new leadership, suggesting the situation may be in flux.

Safety and serious incidents: Multiple reviewers report safety incidents that family reviewers found alarming: wheelchair safety incidents requiring multiple staff, low oxygen levels without timely intervention, alleged wrong medications, pressure ulcers, infections, and claims of elder abuse or drugging. There are also reports of residents being moved or discharged in ways that distressed families. These accounts, coupled with mentions of lawsuits and alleged deaths, significantly increase perceived risk for potential residents.

Patterns and variability: The reviews reveal a pattern of inconsistent performance—some shifts, teams, or units provide exemplary care, while others are described as neglectful or unsafe. This inconsistency points to systemic problems (staffing levels, training, management oversight) rather than solely isolated, individual failings. Several reviewers explicitly note improvement over time under new management, while others say things have deteriorated, indicating rapidly changing conditions.

Summary assessment: Liberty Health and Wellness elicits strong praise for its therapy and for many individual staff members, social work support, activities, and certain facility features. However, there are numerous and serious allegations of understaffing, neglect, medication errors, poor hygiene, unsafe incidents, management dysfunction, and financial/operational instability. For anyone considering placement, these reviews suggest due diligence is essential: tour multiple times, ask for recent inspection and staffing data, request specifics about nurse-to-resident ratios and on-shift CNA staffing, verify medication and incident reporting procedures, and evaluate management stability and complaint resolution practices. The mixed nature of the reviews means experiences can vary dramatically; the facility shows both notable strengths and serious, documentable risks that families should investigate thoroughly before committing.
