    Unsafe understaffed unresponsive expensive unclean

I placed my loved one here and was disappointed. Communication between departments was terrible, staff often unresponsive/overworked, and nursing short-staffed. The memory care felt unsafe - unsecured doors/windows, wandering, verbal abuse, a fall with delayed care, and possible missing meds/personal items. Cleanliness and staff quality were inconsistent: some CNAs/RNs were excellent, but others were rude, dishonest, or neglectful. High cost, few amenities, and poor family communication - I would not recommend this facility.

    Summary review

Overall sentiment across the reviews is highly mixed but weighted toward serious safety and quality concerns. Multiple reviewers report extremely troubling incidents in the memory care unit, including unsecured doors and broken window glass with unsecured locks, allowing residents to wander or escape. Several accounts allege verbal abuse, delayed response to falls, and possible physical abuse resulting in fractures and bruising. There are also repeated allegations of missing personal items, suspected theft by staff, and even possible medication discrepancies. These are not isolated minor complaints; they include reports that the facility was reported to authorities and that investigations have been requested.

Care quality is inconsistent. Numerous reviewers praise specific caregivers — nurses, CNAs, and med techs — as attentive, compassionate, and responsive, noting quick communication and timely resolution of concerns in some cases. At the same time, other reviewers describe neglectful or outright abusive behavior, delayed assistance after falls, dehydration, and poor outcomes following incidents. This divergence suggests large variability in care depending on shift, unit, or individual staff members. Understaffing and overworked employees are commonly cited and likely contribute to the inconsistent care and delays described.

Staffing and management issues are a central theme. Reviewers repeatedly mention poor communication between departments and with families, unresponsiveness, and delayed administrative actions such as refunds for prepaid stays. Some families reported being given inaccurate or incomplete information about doctor visits and resident status. While some staff members are described as helpful and supportive during admissions and day-to-day care, others are described as rude, dismissive, or unprofessional, including staff wearing street clothes and lack of appropriate professional conduct. The presence of both high-quality and problematic staff behaviors indicates management challenges in training, oversight, and retention.

Facility condition and cleanliness reports are sharply divided. Several reviewers state the facility is always clean, smells fine, and is well-kept, while others describe dirty rooms, urine odor, rotting walls with mold-like appearance, filthy windows, and even bed bug infestations. These conflicting accounts point to inconsistent housekeeping standards across rooms or units. Maintenance issues such as windows needing cleaning, paint or updates required, and broken glass in sensitive areas (memory care) raise additional safety and aesthetic concerns.

Safety and security problems are particularly acute in memory care according to the summaries. Multiple reviewers describe unsecured doors and hardware failures enabling residents to wander. Such lapses are dangerous for residents with cognitive impairments and were connected in reviews to actual escapes. Several families also reported theft of belongings and possible medication issues, compounding safety worries. Because these are high-risk problems tied directly to resident harm, they are among the most serious patterns appearing in the reviews.

Dining, activities, and amenities receive mixed feedback. Some reviewers report good food and a robust activity calendar, while others report awful food and very few activities. Therapy services exist and are noted positively by some families, but other reviewers mention limited rehab or mobility support. The facility appears to be budget-conscious; some appreciate Medicaid acceptance and practical staffing, while others feel the cost is high relative to the service level. Outdoor seating and library/amenity offerings are described as limited by several reviewers.

Given the divergence of experiences, a clear pattern is that quality is uneven across units and shifts. Several families strongly recommend the facility and feel their loved ones were well cared for, whereas others strongly advise against it due to safety incidents, alleged abuse, theft, and poor management responsiveness. Because the most serious complaints involve resident safety, possible abuse, and regulatory reporting, these should be treated as high-priority concerns.

Recommendations for families considering this facility would be to verify current licensing, request incident and inspection records, observe the memory care door/security measures in person, ask for staffing ratios and turnover rates, clarify protocols for falls and injuries, and confirm how the facility handles personal belongings and medication tracking. Prospective residents and families should schedule multiple visits at different times of day to assess consistency of staff behavior, cleanliness, and responsiveness. The reviews indicate there are capable, caring staff and functioning services in place, but there are also systemic and safety issues that require careful vetting and ongoing monitoring.

    About Aspire of Oak Grove

Aspire of Oak Grove sits over on 2108 SW Mitchell St in Oak Grove, Missouri, and what you notice about this place is the range of health and care services, since they're not one to leave things to chance when it comes to residents' needs, so you've got skilled nursing set up for those who need daily medical care and attention, and then they offer short-term rehabilitation for folks who are dealing with an illness or recovering from an injury, which is a relief if you ever need more help getting back on your feet, and for those with memory issues, they have a memory care community set up with extra security and staff who've trained to help people with Alzheimer's and dementia, even installing systems so no one accidentally wanders off, and over the regular days you'd see physical, occupational, and speech therapy services running-whether you're staying there long-term or just need outpatient rehab, it's the sort of place where they aim to meet you at your level. There's a nutrition specialist on staff too, making sure dietary needs are looked after, and meals come planned out with nutrition in mind, so you're not guessing about what's for dinner or if it's good for you. Transportation's available, so folks can get to appointments and out for community activities if they want a change of scene, and besides the medical side, there are psychological and mental health services for those who need a bit more support, because everyone handles old age different, and sometimes life gets heavy. You can have pets there, and while it's an apartment setup with features you'd expect in a senior living spot, they don't have details on things like pools, doormen, or fancy patios-there's not much shared about the building's layout or special extras like fitness centers or hot tubs for unwinding, and the specifics about rent, what's included, and the types of services in each room aren't spelled out, but it fits under Aspire Senior Living and goes along with what the Missouri Health Care Association lines up. The facility sticks to federal law on fair housing and employment, so no one's turned away or treated differently because of who they are. Aspire of Oak Grove isn't flashy, but it tries to cover the basics-health, safety, and everyday living-without making big promises it can't keep.
