Aurora Health and Rehab sits over at 1200 McCutchen Rd. in Rolla, Missouri, and people will notice right off that it's a large place with 116 certified beds, always open day or night, and while the country kitchen stands out as one of the shared amenities, at the heart of the facility is a wide range of care, from independent living and assisted living to memory care, skilled nursing, home health care, and hospice, with every service under one roof, so families won't have to look elsewhere if their loved one's needs change. The staff runs the Rehab To Home program that helps folks regain strength and independence after rehab, and there's a special Certified Wound Care Nurse for wound treatment, which not every place has, while the Activities Department regularly works with Senior Companions of Phelps County, bringing in friendly visits and social engagement that helps residents not feel isolated. The nurse team here has many decades of experience with both acute and chronic illness, and they put together personalized plans for each resident, working with a nurse-to-resident staffing ratio that some might find on the lower side at 3.13 nurse hours per resident per day, and with nurse turnover at 65.7%, care consistency isn't always guaranteed, though this is fairly common in the industry these days. Aurora Health and Rehab offers skilled rehabilitation and accepts both Medicare and Medicaid, and the clinical staff provides detailed health services tailored to each resident's situation. The place is a for-profit operation and part of Vertical Health Services, with ownership and management that's changed hands a few times, currently controlled by McCutchen Rd Consulting LLC since mid-2023. For those who want to visit, the staff can help set up tours and check if rooms are available, and they'll answer both general and employment questions. Inspection records show some challenges here, with 57 total deficiencies found over time-including 8 related to infection control-and the Centers for Medicare & Medicaid Services rates Aurora Health and Rehab much below average in overall performance, health inspections, and staffing, but marks it as average for quality measures, so while the care options and extra features stand out, it's wise to carefully look at the inspection results and current reports when making decisions.

Welcoming team; inconsistent care quality
I felt welcomed from day one - Jen, Dawn (and even Shadow the dog) and the therapy team were warm, professional, and gave me confidence in the medical and rehab care. The facility often feels clean, homey and roomy, with excellent PT/OT/SLP and many caring nurses and aides who go above and beyond. That said, care is inconsistent: weekends/evenings can be understaffed, and I saw missed meds, missed showers, call lights ignored, occasional hygiene and cleanliness problems, and some broken or smelly rooms. Overall I'd recommend for rehab and skilled therapy but advise close oversight and asking specific staff/shift questions before committing.
