    Understaffed, filthy, unsafe, neglectful care

I placed my father in this facility and it was a nightmare. Chronic understaffing, rude/evasive administration, filthy rooms and hallways (moldy smell, flies, feces left, broken furniture), safety problems and bruises, overcrowding, poor hygiene and limited therapy - despite a few loving aides, care was neglectful and my father was overmedicated and declined visibly. Management was unprofessional and possibly fraudulent; we moved him to a cleaner facility closer to family. I do not recommend placing a loved one here.

    Summary review

Overall sentiment across these reviews is strongly negative, with repeated and severe allegations about care quality, safety, cleanliness, and management. The dominant themes are neglect, poor sanitation, medication concerns, and administrative failures. Many reviewers describe situations that they consider dangerous or abusive — including theft, untreated injuries, medication overuse, significant weight loss, and in at least one case a patient death — which have led several families to remove relatives from the facility and warn others against placement.

Care quality and clinical concerns: A central and recurring issue is clinical care and medication management. Multiple reviews allege overmedication and inappropriate use of antipsychotic or sedating drugs, with some families reporting their loved ones were heavily medicated to the point of sedation and weight loss. There are also numerous reports of insufficient assistance with mobility and activities of daily living, suspected underfeeding, and observable physical harm — bruises, black eyes, and other injuries — with at least one report of residents being attacked by others. These accounts indicate failures in supervision, individualized care plans, and safe handling of residents with cognitive impairment or behavioral needs.

Staffing and staff behavior: Staffing levels and staff conduct are described with a starkly mixed picture. Many reviewers report that the facility is too short-staffed, with erratic scheduling, overworked caregivers, and a hostile work environment that impacts continuity of care. Several accounts describe staff as lazy, abusive, or unprofessional, and there are allegations that nurse leadership (e.g., Director of Nursing) made false accusations against family members. Conversely, a subset of reviews praises direct care staff as loving, hardworking, and genuinely caring — suggesting that while some caregivers try hard under difficult conditions, systemic staffing and management issues undermine overall resident care.

Facility environment and maintenance: Cleanliness and maintenance are repeatedly criticized. Reports include moldy smells, visible mold and dust, filthy floors, dirt behind toilets and showers, broken furniture and fixtures (missing shower knobs, broken closet doors with protruding screws, filthy nightstands), and pests such as flies. Reviewers also mention HVAC failures or no air conditioning and thermostat/heat problems. Several families described the hallways as having a persistent stench and rooms as unmaintained to the point of causing allergic reactions. These environmental problems raise infection-control and safety concerns and indicate chronic maintenance and sanitation lapses.

Safety, security, and incidents: Safety concerns are prominent. Allegations include theft of personal items, denial of access to residents' rooms, overcrowded rooms, residents being attacked, and feces left in living areas for hours. Multiple reviewers expressed that the environment felt unsafe and that the facility should be shut down. There are also mentions of threats from staff and that some family members intended to press charges. These reports suggest serious lapses in oversight, reporting, and post-incident response.

Administration, communication, and billing: Many reviews criticize administrative leadership for poor communication, unpreparedness, rudeness, and evasiveness. Specific allegations include improper Medicaid/insurance billing and administrative misconduct, as well as false accusations directed at family members by facility leadership. These issues point to systemic problems in transparency, record-keeping, and regulatory compliance, which compound clinical and operational deficiencies.

Dining, activities, and resident life: Reviewers commonly report insufficient or neglected meals, limited therapy programs, and a lack of meaningful recreational activities. Families noted weight loss and poor personal hygiene among residents, and some described the environment as dehumanizing. Although staffing shortages were cited as a partial cause, the recurring complaint is a lack of engagement and an impoverished day-to-day experience for residents.

Mixed experiences and patterns: While the overall tenor of the reviews is negative and many families had urgent, serious complaints, there are scattered positive comments praising certain staff members’ compassion and dedication. A few reviewers explicitly moved loved ones to cleaner facilities and reported improved outcomes, indicating that available alternatives were sometimes necessary and effective. One reviewer explicitly noted COVID-era challenges as context, implying staffing and operational strains during the pandemic may have exacerbated problems.

Recommendations and notable outcomes: The reviews collectively recommend caution. Several families removed residents immediately, some reported planning legal action, and others stated the facility should be shut down. The persistence and severity of complaints — spanning sanitation, clinical care, safety, and administration — suggest systemic issues rather than isolated incidents. Families considering this facility should seek independent inspections, review state survey and citation history, request detailed care plans and medication reviews, and prioritize direct observation of cleanliness, staffing levels, and staff-resident interactions before placement. Conversely, if a resident is already in this facility and showing decline, families should conduct a prompt, documented assessment and consider immediate transfer if safety or basic care needs are not met.

    About Four Seasons Living Center

Four Seasons Living Center sits at 2800 State Hwy TT in Sedalia, Missouri, and has 239 certified beds with an average of about 233 residents each day, so the place is pretty large and you'll find all sorts of people living there since it serves both seniors and adults with mental illness, since there's care for folks starting at age 18 and up, and the whole thing's managed by Reliant Care Management Company, who's been running it since back in June of 1996. The facility's got several different units: there's a manor unit that holds up to 61 men, mainly for those with dementia, traumatic brain injury, and similar needs; you'll find a Safe Haven unit with 239 dual-certified beds that handles skilled tasks like IV therapies and tube feedings; and there's a residential care unit and an all-female secured area for mental illness, which takes women over eighteen for short or long stays. They also have a specific Alzheimer's unit, with 56 residents and added security like cameras and alarmed doors, and a Medicare unit, which is the only one in Sedalia, where licensed nurses work around the clock and people get therapies like physical, occupational, and speech. You'll also find a physical rehab unit for more complex needs, whether someone is staying for a while or just for recovery.

They provide lots of help with daily things, like bathing, getting dressed, help with moving and transferring between places, and taking medicines. There's also a memory care community that's set up to help people with dementia have some engagement and a higher quality of life, with trained staff and safe, secured areas. There's always staff supervising twenty-four hours, and the building's set up to keep folks safe if they're at risk of wandering. People get meals every day, with options if you need a special diet due to allergies or diabetes, and the place also takes care of housework and laundry for you, so you won't have to worry about cleaning or doing your own wash.

If you need more serious health care, the skilled nursing section helps with things like wounds, insulin shots, tube feeds, tracheotomy care, dialysis, hospice, post-surgery recovery, and even psychiatric care for folks who need mental health services. They've also got physical therapy and case managers, plus staff handling medication management and monthly injections as needed. They offer respite care if someone just needs a short stay, home health if memory care isn't needed, and continuing care for ongoing needs. Visitors can come for scheduled tours to meet the staff and see the place before deciding.

Amenities include furnished rooms, a dining room, beauty and barber shop, an activities area with events like movie nights and games, emergency alert systems, transportation, social services, and planned activities to help residents stay busy and involved. They coordinate move-ins, so new folks get some help settling in. They accept all sorts of payment, like Medicare, Medicaid, private insurance, hospice, and private pay, so families have options to cover the bills.

On the other hand, the center's got a 2.7 review score out of 19 reviews and has been cited by inspectors for some issues, including nutrition and dietary deficiencies about food taste and temperature and storage issues, plus some resident rights concerns and a total of 49 deficiencies spotted in reports, five of those about infection control. The facility's nurse turnover rate stands at 54.5%, and nurse staffing works out to about 1.32 hours per resident each day, which some might want to compare to other facilities. Four Seasons Living Center mainly cares for older adults but also has space for others with special needs, and tries to treat residents with dignity while staying connected to hospitals and doctors in the area.
