    Immaculate, safe, caring family-style home

I placed a loved one here and have been very pleased - the small, family-style home is immaculate, safe and odor-free with a sunny patio and mostly single rooms. The owners and staff are professional, attentive and caring (they helped with recovery from encephalitis, were kind in the final days, even handled funeral arrangements), meals and mobility assistance are provided, admission was fast and Medicaid is accepted - I would recommend this community.

    Summary of reviews

The reviews for Tender Heart Assisted Living present a mixed picture, with a cluster of consistent positives alongside several significant operational concerns. Many accounts emphasize rapid admission, Medicaid acceptance, a small family-style atmosphere, and attentive, hands-on caregiving. Multiple reviewers described a clean, odor-free interior, mostly single-occupancy rooms, accessible bathing options, daily meals (including positive holiday meals), outdoor common areas, and instances of end-of-life support and bereavement assistance. These elements suggest the facility can provide a homelike environment with personal attention and basic clinical support for residents who need mobility assistance and close supervision.

Care and staff: Care-quality impressions are polarized. Numerous comments praise individual caregivers and owners as professional, patient, and compassionate, with examples of symptom recovery support and attentive final-days care. At the same time, other accounts raise concerns about staff conduct and responsiveness; these describe inconsistent professionalism and tone in staff–family interactions. The aggregate indicates variability in staff performance and interpersonal communication, meaning experiences may depend heavily on specific caregivers or shifts.

Dining and activities: Dining is generally provided daily and reviewers pointed to pleasant communal meals (including a positive holiday meal), but some noted a tendency toward carbohydrate-heavy menus and uneven nutritional balance. The facility offers communal spaces and outdoor patio areas that support socialization — reviewers mentioned friendships formed in common rooms and regular movement of residents to shared areas, which can be beneficial for engagement and mobility.

Facilities and safety: Several reviewers commended the cleanliness and overall home-like feel of the building; accessible bathing arrangements and mostly single rooms were highlighted as positives. However, there are substantive operational concerns relating to pest-control practices and to safety/regulatory compliance: at least one account references health-department inspection issues, and others describe unresolved pest-control problems. These items point to gaps in environmental maintenance and oversight that warrant careful inquiry by prospective families.

Management and communication: Reports about management are mixed. Some families described owners as understanding and involved, while others described defensive or discourteous responses to complaints. There are indications of weak complaint-resolution processes and limited transparency around serious incidents; an allegation involving staff conduct resulted in outside protective-services involvement in one instance. In addition, some families experienced restricted or contested visitation and disagreed with facility decisions at end of life, suggesting that visitation policies and family communication practices may be inconsistent.

Notable patterns and guidance: The overall pattern is one of unevenness — strong, individualized care and a home-like environment in some cases, contrasted with operational and oversight vulnerabilities in others. Prospective residents and families should verify current regulatory status, inquire specifically about pest-control protocols, staffing ratios, supervision and training, incident reporting practices, visitation policies, and menu planning. Visiting the facility during multiple times of day, meeting care staff and management, and asking for recent inspection reports will help assess whether Tender Heart’s strengths align with a particular resident’s needs and whether the facility's operational weaknesses have been adequately addressed.

    About Tender Heart Assisted Living

Tender Heart Assisted Living sits over on Tulane Drive NE in Albuquerque and has state licensing to care for up to 14 residents. The facility's got options for studio, single-room, and two-bedroom setups, with private baths and features aimed at privacy and safety like sprinkler systems, safety bars, and easy access all throughout. Residents can bring personal touches to their rooms and enjoy on-site amenities like a dining room, game and activity areas, salon and barbershop, fitness center, indoor and outdoor spots for relaxing, and gardens and patios that folks like to use for gardening or just sitting outside. Meals come home-cooked, and snacks are always on hand, with staff taking care of laundry, housekeeping, and regular maintenance.

There's a strong focus on both independent living and memory care, with programs for Alzheimer's or dementia including specialty activities, trained caregivers, and memory care areas built to be safe and easy to navigate. The home offers help with walking, bathing, grooming, personal hygiene, and medicine reminders or administration. Residents with more complex needs can get hospice, end-of-life care, and rehabilitation therapies right there, plus mobile lab tests, X-rays, and on-site physician care, so there's not much need to leave for basic health needs.

Tender Heart Assisted Living takes pride in family input, keeping a review and feedback system open so folks can speak up. The management's involved daily and responds quickly if any issues come up, and the ownership's local with over 30 years' experience, so they know how to address things and make changes if something's not working for a resident. Assisted living staff are specially trained in dementia care, and activities span arts and crafts, games, outings to local spots, bowling trips, and social gatherings for a strong sense of community, which seems important when folks are living together like this.

Transportation gets arranged for appointments or outings, and there's free scheduled rides. Physical therapists come in for rehab when needed, helping folks recover or keep up their routines. The BlueStep HQ management system keeps track of medicines, daily health, and other details, so information gets communicated clearly to the staff on each shift.

The community emphasizes personal attention, with help offered for big daily tasks or little things-something as simple as getting a resident into a wheelchair or encouraging someone to join a group activity. Seniors who live at Tender Heart seem to appreciate the calm house-like setting with plenty of privacy but also enough going on to feel connected to other people. The home's focus is to help residents stay as independent as possible while keeping peace of mind for families who trust others to care for a loved one.
