    Clean facility, caring staff, mixed

I visited and was impressed by the sparkling clean, bright, and well-sanitized facility and the warm, hardworking staff - Amalyn, Stephanie and CNA Chelsea stood out. Communication was responsive, meals and activities were nice, and residents generally looked well cared for. That said, I observed instances of dishonesty and poor treatment from some staff, so I'm grateful for the many positives but feel there's room for improvement. Overall I'd give it 3/5.

    Summary review

Overall sentiment across the provided reviews is predominantly positive, with repeated emphasis on cleanliness, warm staff interactions, and a pleasant atmosphere. Many reviewers describe the facility as "sparkling clean," bright, sanitized, and smelling pleasant, which contributes to a non-depressing, welcoming environment for residents and visitors. Multiple comments praise the visible presence of staff and a personal touch in resident care, indicating that routine upkeep and environmental standards are consistently noticed and appreciated.

Care quality and staff performance emerge as the strongest themes. Numerous reviewers praise hardworking, caring, and compassionate employees — including specific shout-outs to Amalyn, Stephanie, Chelsea Helms, and unnamed nurses — who are credited with going above and beyond for residents. Families reported appreciation for CNAs, nursing staff, and contributions from kitchen, housekeeping, and medical staff, suggesting a team-based approach to resident wellbeing. Reviewers noted that staff often assist with paperwork, offer responsive communication, answer questions, and help make residents comfortable, which points to both practical and emotional support being provided.

The facility’s dining and activities are also noted positively. Several reviewers mentioned that residents enjoyed meals and that the food was good. Activities and a generally upbeat atmosphere were highlighted as contributing to residents’ quality of life, reinforcing the impression of a place that supports social engagement and a positive daily experience. Praise for a professional visitor system and responsive communication further supports a sense that operational aspects such as visiting, inquiries, and family interactions are handled competently.

Despite the overall favorable trend, there are notable concerns and mixed experiences that should not be overlooked. A minority of reviews allege poor treatment of residents, lack of honesty, and disrespectful behavior by some staff members. One summary explicitly accuses staff of dishonesty and calls out deceptive five-star rating activity. Another reviewer gave an overall three-star rating and noted room for improvement. These comments indicate inconsistency in care and conduct: while many families report excellent and attentive care, a few have had troubling experiences that suggest variability across shifts, teams, or individual employees.

In sum, the dominant narrative is that Clayton Nursing & Rehab provides a clean, bright, and generally compassionate environment with many staff members who are attentive, helpful, and committed to residents’ wellbeing. Dining, activities, and operational communication are frequently praised. However, management should take the critical comments seriously: reports of poor treatment and allegations of dishonesty, though less common in this dataset, are serious and point to uneven performance. Addressing these concerns through training, oversight, and transparent response to family complaints would help align all experiences with the many positive accounts reflected in these reviews.

    About Clayton Nursing & Rehab

Clayton Nursing & Rehab sits under the management of Genesis Healthcare and holds a capacity for 45 certified beds, keeping an average of about 30 residents each day; it's a for-profit facility, structured with Genesis Omg Operations LLC as the owner, and its leadership's been steady, with folks like Carolyn Kear and Mark Van Wormer managing since June 2024, and the main administrator with more than 18 years of service under her belt. This place accepts Medicare, Medicaid, and several private insurances, though right now, it isn't taking in new patients. The facility got inspected not too long ago, October 31, 2024, and it's had some health citation history, including issues around nutrition, medication, infection control and other care-related matters, but recent CMS reports give it a 5-star overall rating, with particular strengths shown in quality measures, staffing, and health inspections, sitting at 4 stars for staffing and health inspections and 5 stars for quality. There's a focus on keeping the environment safe, making sure walkways and common areas stay free of hazards, and they've got a wide spread of care like long-term and short-term rehab, skilled nursing, palliative care, pain management, dementia care, wound care even for tough cases like stage 4 bed sores, and they pay attention to allergies and dietary needs. Residents can get IV therapy, orthopedic rehab, discharge planning, and there's physician and registered nurse care, plus staff who know how to help people with different needs, using a 3.29 nurse hours per person per day average. Amenities go beyond basics, with in-room dining, private or semi-private rooms, private bathrooms, and special balconies or patios for memory care residents, and you'll find peaceful gardens or a courtyard, computer with internet on site, cable TV, wireless internet, and phone service, plus movies, newspaper and mail, laundry, housekeeping, and a smoke-free setting. Residents have access to salon or barber services, recreation, religious and cultural activities, activity rooms, and common lounge spaces to visit, plus interpreter services and staff that speak English and other languages. The alarm system helps keep folks with cognitive troubles safer, and there's 24-hour response for emergencies, as well as security measures throughout. Therapy services include contract rehab, physical, occupational, and respiratory therapy, as well as case management, care planning, on-site X-ray, dental, and vision care, pain management, pet therapy, and colostomy care. The facility stands out for allowing visiting pets with vaccination records and offers interpreter support, coordinated transportation, personal care, mail services, and help for people with mobility needs, so everyone's got what they need to keep going about their lives, with room to enjoy gardens, private outside space, and a peaceful environment that's kept as safe and home-like as possible. State assistance is accepted, and laundry facilities are free for residents, with meal support and nutrition services carefully managed, though deficiencies earlier included two related to infection. People have quiet corners here and the ability to do whatever activities suit them-either relaxing or joining in on group events. Right now, the place isn't accepting new residents but takes care in tailoring services to each person, aiming for comfort and respect without much fuss or hype.
