    Caring staff but poor administration

My wife lives here and I’m torn: nursing and rehab staff are caring, COVID precautions were handled well, and housekeeping keeps rooms clean and residents well‑groomed. Communication and management are frustrating — staff can be difficult, I’ve seen a missed doctor’s appointment and unapproved meds given, and pricing became confusing after a takeover with transfers to regular pricing without clear notice. Food is terrible and quality has dropped; private rooms are pricey while shared rooms are common. There are decent activities (bingo, gardening, chair exercise, bean bag toss, nail care) and my wife is happy, but beware the administrative issues.

    Summary review

Overall impression: Reviews present a mixed picture of Sombrillo Nursing & Rehabilitation Center. Multiple commenters consistently praise the hands-on caregiving, regular nursing presence, and available rehabilitation services, and many note that housekeeping keeps rooms clean and residents well-groomed. At the same time there are repeated, specific concerns about dining quality, management changes and pricing practices, communication gaps, and a few serious clinical/operational lapses. Taken together, the sentiment is: strong direct care and basic facility upkeep, but important issues around food, administration, communication, and some safety/medical process lapses that prospective residents and families should probe further.

Care quality and staff: Several reviewers emphasize that staff are caring and provide regular nursing and rehab care. Therapies are offered and some residents participate actively in therapy programs. Positive comments include residents being clean, well-groomed, beds made and bedrooms kept tidy — indicating reliable day-to-day personal care. However, staff consistency and communication are recurring weaknesses. Reviewers note that while staff are friendly when present, they can be difficult to deal with and do not always communicate clearly. More serious problems were raised such as a missed doctor's appointment and an instance of unapproved medications being given. These items suggest variability in operational reliability and potential lapses in clinical coordination that should be investigated when considering placement.

Facilities and cleanliness: Cleanliness and housekeeping receive generally positive mentions. Rooms are reportedly cleaned daily, and reviewers explicitly say bedrooms and beds are kept clean. The facility appears to have mostly shared rooms, with few private-room options; private rooms are available but described as expensive. This pattern is important for families to consider depending on privacy needs and budget constraints.

Dining and daily living: Food quality is a clear negative theme — reviewers repeatedly describe the food as terrible. Additionally, at least one reviewer feels overall quality dropped after a takeover, while prices remained the same. Food dissatisfaction combined with perceived quality decline after ownership/management changes are notable red flags for overall resident experience and value for money.

Activities and social programming: The facility offers a variety of activities — specifically bingo, gardening, chair exercise programs, bean bag toss, and nail care — and some residents participate in therapy-based programs. These activities indicate an active programming schedule that supports engagement and recreation, which is a strength for resident quality of life.

Management, pricing, and visitation policies: Management and administrative practices are another area of concern. Reviewers report pricing confusion, an unexpected transfer to regular pricing without clear notice, and a sense of paying the same price despite perceived declines in quality. Communication problems from staff and administration amplify these frustrations. Visitation and access restrictions related to COVID are mentioned; while safety precautions are acknowledged, restricted access and confused or unclear communication about those policies have negatively affected families’ experiences. One reviewer called it the "best nursing home in the state" and another reported that his wife was happy there, indicating that experiences can vary significantly.

Patterns and recommendations: The dominant positive patterns are consistent personal care, rehabilitation services, cleanliness, and a reasonable activity program. Dominant negatives are poor dining, administrative/pricing opacity, communication breakdowns, occasional clinical/process errors, and limited private-room availability. For families considering Sombrillo, it would be prudent to: 1) verify current ownership/management and any documented changes to services since the reported takeover; 2) ask specifically about medication administration protocols and how doctor appointments are scheduled and tracked; 3) tour dining and sample meals if possible or ask about menu options; 4) confirm pricing structure in writing, including any potential transfers to different rates and private-room costs; 5) inquire about staffing levels, shift continuity, and how family communication is handled; and 6) ask about current visitation policies and how pandemic-related restrictions are implemented and communicated.

Summary judgment: Sombrillo appears to deliver solid, compassionate hands-on care and maintains clean living spaces with active programming, but there are credible and recurring complaints around food quality, administrative transparency, communication, and a few operational safety concerns. Prospective residents and families should weigh the strengths in clinical and housekeeping care against reported deficiencies in dining and management, and perform targeted due diligence on pricing, medication safety, and staff communication before deciding.

About Sombrillo Nursing & Rehabilitation Center

Sombrillo Nursing & Rehabilitation Center sits in the heart of Los Alamos, New Mexico, and overlooks Pueblo Canyon, so there's a nice view if you like watching the changing seasons, and all around you'll find a warm, relaxed atmosphere, where people stay either for a short while after a hospital visit or for longer if they need more support. The center can accommodate up to 64 residents and has private, furnished rooms with TVs and telephones, which does make stays feel homier, whether you're there just to recover or for an extended residence, and you'll see their team of Registered Nurses, Licensed Vocational Nurses, Certified Nursing Assistants, and specialists working 24 hours a day, every day, making sure no one goes without what they need, and all that care gets overseen by a licensed physician while therapists for physical, occupational, and speech needs come in as the doctor orders.

Sombrillo offers services for people who need help after surgery, for those recovering from strokes or orthopedic and cardiac events, and for anyone dealing with respiratory or neurological issues, and they provide palliative and supportive care as needed to ease pain or make life more comfortable, with special attention paid to wound care, pain management, diabetes, and complications that can arise with age or after hospitalization-plus you'll get infusion therapy like IV antibiotics if the doctor orders them. They have programs to help folks recover enough to return home safely, even working with Home Health and other resources to make sure transitions go well. If you need help with daily activities like bathing, dressing, or getting your medicine right, their assisted living and skilled nursing services will cover that, too, with staff trained in dementia and Alzheimer's care in case that's needed.

The food menu is overseen by a certified dietitian and changed up often; they serve meals all day, and they'll make adjustments for food allergies, diabetes, or special faith-based diets, so no one's left out come mealtime, and the dining room is part of the big, shared living areas that also include a spacious garden, homelike lounges, community rooms, an arts room, walking paths, and even on-site aviaries and aquariums. They bring in hairdressers and barbers, and for people who like to socialize or stay active, the staff arranges movie nights, arts programs, and activities based on what residents like or can do, so there's usually something going on if you want company.

Safety matters too, so there are emergency alert systems, 24-hour supervision, call buttons, wheelchair access, and help for anyone who can't get around on their own; housekeeping, laundry, and transportation services come standard, and they handle all kinds of medication management. The recreation and social program is pretty varied, and social services staff help residents and families with transitions, arranging needed care or programs-plus there are events and educational workshops for families and community members.

Sombrillo is part of the Los Alamos Retirement Community (LARC), which is a nonprofit group, and membership in state health care associations shows they're keeping up with care standards and training. The facility takes Medicaid and Medicare, and has dedicated professionals and an administrator named David Lemons who oversees operations. You'll find a focus here on dignity, respect, and helping each person reach their best possible level of independence, with care that stretches from helping people get back on their feet to providing long-term support if needed, and nobody pretends it's perfect, but you do get a sense of commitment from the staff and a real effort to foster positive relationships and make each day feel meaningful, whether in recovery, long-term care, or in making this place feel a bit like home.
