    Chronic understaffing, neglectful, rude staff

I do not recommend this facility. I saw chronic understaffing and high turnover, rude/unresponsive nurses, and CNAs left without help so residents weren't properly cared for - late or missing meals, often not showered or washed, diapers put on without consent. I witnessed neglectful, unprofessional behavior (including an incident on Sept 9, 2022 and a rude staff member, Diane); only Lily helped with a shower. Rehab care and transfers were poor. We moved him to St. Joseph's and he was much happier.

    Summary review

Overall sentiment: The reviews present a mixed but predominantly negative picture of RiverLedge Nursing Home. While several reviewers praise the facility's physical environment and activity programming, a large number of summaries raise serious concerns about the quality and consistency of direct care, staffing levels, safety, and food/rehabilitation services. The pattern is one of inconsistent experiences: some residents and families encountered respectful, compassionate staff and an active community, while others report neglect, lack of responsiveness, and even abuse.

Care quality and personal care: The most frequent and serious complaints involve basic personal care failures. Multiple reviewers describe residents not being showered or washed, missed or late meals, and diapers being applied without consent. There are reports of residents being left unattended and of staff not responding to needs in a timely manner. These issues suggest lapses in daily hands-on care and in supervision. A few reviews do note compassionate and respectful treatment, but those positives appear uneven and less common than reports of neglect.

Staffing, culture, and management: Staffing problems are a persistent theme. Reviewers repeatedly describe the facility as understaffed, with high turnover and low pay leading to overworked employees and a negative work environment. Several complaints allege rude, lazy, or unprofessional behavior; one review specifically names a staff member ("Diane") as having provided a negative experience. There are also reports that some staff refuse to assist CNAs, indicating teamwork and management issues. The reviews imply systemic workforce problems that directly affect resident care and safety.

Safety and abuse concerns: A particularly serious allegation describes an abuse incident witnessed by a granddaughter on Sept 9, 2022, involving an aide and kitchen staff. This was presented as a direct safety concern. While not all reviewers observed abuse, the presence of this reported incident, along with other neglect-related complaints, raises red flags about supervision, training, and incident reporting. Families and reviewers advised caution and some explicitly do not recommend the facility for loved ones.

Dining and rehabilitation services: Multiple complaints concern the dining experience: late or missing meals, raw or disgusting food, and running out of menu items. These issues point to operational problems in the kitchen and meal delivery processes. Rehabilitation and therapy services are reported as minimal or inadequate by several reviewers; some experienced delays transferring residents to higher-level rehab, and at least one family moved their loved one to another facility (St. Joseph's) and reported better outcomes there. These comments suggest rehabilitation and clinical coordination need improvement.

Facilities and activities: In contrast to the care and staffing criticisms, many reviewers praised the physical setting and social programming. The facility is described as secure, very clean, and welcoming, with a range of posted activities, a game room, chapel, outdoor spaces, bingo, cards, and catered events like picnics. These features appear to be genuine strengths and are consistently mentioned among the positive remarks.

Patterns and recommendations: The reviews indicate inconsistent performance across key domains. Strengths include cleanliness, security, atmosphere, and social activities; weaknesses cluster around hands-on care, staffing and culture, food service, and rehabilitation. The repeated mentions of understaffing and high turnover suggest the negative experiences are not isolated incidents but likely systemic. For families considering this facility, the reviews recommend thorough in-person visits, direct questions about staffing ratios, turnover rates, incident reporting procedures, training (especially empathy and abuse prevention), and how meals and therapy services are managed. For management, the reviews point to priority actions: improve staffing levels and retention, invest in staff training (empathy and professional conduct), strengthen supervision and incident investigation processes, and address kitchen/food service reliability and rehabilitation service access.

Bottom line: RiverLedge Nursing Home shows clear strengths in environment and activity programming, but recurrent, specific complaints about neglectful care, safety, staffing shortages, poor food service, and inadequate rehab services create significant concerns. The facility may provide a pleasant atmosphere for some residents, but the frequency and severity of negative reports—especially those involving basic hygiene, consent, and a reported abuse incident—mean families should proceed cautiously and verify how management has addressed these issues before making placement decisions.

    About RiverLedge Nursing Home

RiverLedge Nursing Home sits at 732 Ford Street in Ogdensburg, NY, and has served as a skilled nursing and rehabilitation center for over 41 years, and while it has gone through name changes and renovations, it's always aimed to support residents with both long-term care and short-term rehab needs, which means people would get help with everyday tasks like bathing, dressing, and transfers, and there's always staff around the clock for things like wound care, medication management, and assistance for those living with Alzheimer's or dementia. The facility has held up to 180 residents across five skilled nursing neighborhoods and offered both private and semi-private rooms, all furnished, with kitchenettes, cable, Wi-Fi, phone service, and places like living rooms, sunrooms, common areas, and a small library, plus outdoor spaces and gardens for some fresh air, and you'd find homestyle dining that allows for special diets, meal prep, and personal choices, so mealtime feels comfortable. There's beauty salon service, daily scheduled activities, both community-sponsored and resident-led, as well as life skills programs for those with developmental disabilities, and support for mental and emotional well-being through behavioral health services when needed.

Residents and families could use counseling support, respite care, and short-term post-hospital rehab, and with 24-hour call systems in place, there's peace of mind when it comes to safety or immediate needs. The staff by United Helpers Management Company have made sure there's a focus on person-centered care, adapting services to what folks require, connecting different types of help as needs change over time, and for a long while, the place has offered a continuum of care, backing up everything from independent senior living to memory care, and even virtual tours for those wanting to see things before moving in. Over the years, RiverLedge built new units and even added a special home for elderly women and men, dealt with fires, and made renovations, trying to keep the space both safe and welcoming, though it's gone through a closure process recently, with most staff and residents moved to other locations, following up on their well-being, and providing training for staff transitions.

RiverLedge has been licensed for both Medicaid and Medicare, always reporting five health standard deficiencies but without fines or penalties, holding a Medicare Safety Rating of 2 stars out of 5 but keeping supervision and healthcare as steady as possible, with support from nearby pharmacies, churches, and hospitals like Claxton-Hepburn Medical Center. The facility has given attention to rehab, memory care, activities, and daily living support, but it's important to note the recent closure means only a handful of residents remain, with most staff-about 75%-relocated to other care sites.
