    Beautiful facility with inconsistent care

I had a mixed experience. The facility is beautiful, very clean, with appetizing hot meals, engaging activities and excellent rehab - the caring, knowledgeable staff helped my grandfather return home happy and healthy and many treated residents like family. However, I also saw inconsistent staffing, slow responses to calls, safety/monitoring concerns, missing items and some unprofessional attitudes from certain shifts/administrators. Overall, great care is possible here but go see it yourself and watch how your loved one is monitored and cared for across different shifts.

    Summary review

Overall sentiment across the reviews for The Home At Hearthstone is highly polarized, with a substantial number of very positive experiences alongside a number of serious negative allegations. Many reviewers praise the facility’s caregiving, therapy outcomes, cleanliness, activities, and food, citing compassionate, knowledgeable staff and successful rehab discharges. Conversely, multiple reviews raise significant concerns about neglect, slow response times, safety risks, and inconsistent management. The pattern suggests wide variability in resident experience that may depend on unit, shift, or specific staff on duty.

Care quality and clinical outcomes: Numerous reviews describe strong clinical care and effective therapy. Several families report that relatives completed rehab and returned home healthy and happy, emphasizing the competence of the therapy and nursing teams. At the same time, other reviewers detail troubling deficits: missed showers, long periods without laundry or bedding changes, delayed assistance, and alleged failure to follow medical protocols. These conflicting reports indicate that while high-quality clinical care occurs in many instances, there are documented episodes of neglect and lapses in basic personal care for some residents.

Staff behavior and staffing issues: Staff descriptions range from “caring, kind, compassionate, respectful, and professional” to “mean, nasty, unprofessional, and incompassionate.” Several staff members and roles are singled out positively (activity aides, maintenance staff like Brandon Foster, and certain nursing leaders), which supports accounts of dedicated employees who provide individualized attention. However, reviewers also report slow response times, poor communication, high turnover, and weekend understaffing (including gaps in nursing administration and social work). These operational challenges likely contribute to inconsistent resident care and family experiences. There are also specific, serious allegations involving named administrators (e.g., reports about a director of nursing laughing during a distressing incident and calls for removal), suggesting that leadership and culture concerns have been raised by families.

Safety and supervision: Safety is a recurring theme in the negative reviews. Critics cite unmonitored residents, fall hazards, aggressive patients on the floor, and locked units that raise access and safety questions. Some comments describe emergency situations where supervision or timely response was inadequate. In contrast, other reviewers explicitly state that the environment felt safe and that staff treated residents like family, providing peace of mind. The juxtaposition of these accounts points to inconsistent supervision and safety practices across different times or areas of the facility.

Facilities, dining, and activities: The facility’s physical environment is frequently praised as beautiful, clean, comfortable, and well-maintained. The memory care unit receives positive mentions as well. Dining is generally seen as a strength in many reviews — three meals a day, hot and appetizing food, and overall good meals are commonly noted. Activity programming is another clear positive: Bingo, movies, special events (notably an elaborate Halloween), and family-inclusive activities are frequently cited and appreciated. These operational positives are consistent across many positive reviews, reinforcing that facility amenities and engagement opportunities are strengths.

Management and patterns: Reviews suggest variability in management performance. While some reviewers commend management and nursing leadership as accommodating and excellent, others accuse administration of negligence, poor oversight, and dismissive behavior. Multiple comments reference weekend shortages and upper-level staff criticism, which may indicate systemic scheduling or resourcing issues. The mixture of glowing recommendations and severe complaints suggests the facility may have pockets of excellence alongside areas needing attention — especially around staffing consistency, leadership responsiveness, safety oversight, and basic hygiene/laundry processes.

Notable specifics and recommendations for families: Several named staff received praise (activity aide Mary, maintenance staff Brandon Foster), signaling that certain employees make a measurable positive difference. Families concerned about safety or past negative reports should inquire about staffing levels (particularly on weekends and nights), supervision protocols for residents with behavioral issues, laundry and hygiene schedules, incident reporting and follow-up procedures, and administrative escalation pathways. Prospective residents and families should also ask about turnover rates, staff training, and which units have the strongest reputations, since experiences appear to vary by unit and shift.

Conclusion: The Home At Hearthstone elicits strongly mixed reviews. Strengths repeatedly mentioned include compassionate and skilled caregivers, effective rehabilitation services, clean and attractive facilities, engaging activities, and generally satisfying dining. Serious weaknesses reported by other reviewers include inconsistent responsiveness, allegations of neglect (hygiene and laundry lapses), safety and supervision concerns, staffing instability, and problematic leadership behavior in specific instances. The overall picture is one of a facility capable of delivering excellent care in many cases, but with documented and recurring operational and safety issues that warrant careful inquiry by families and oversight by management.

    About The Home At Hearthstone

The Home At Hearthstone is a nursing home run by Foundations Health Solutions, LLC since January 2019, and sits as a single-story community in Ohio with 96 certified beds, usually serving about 89 residents each day. Residents get 24-hour skilled nursing care, and there's a team of registered nurses, licensed therapists, and dietitians, who work together to handle both short-term rehab and long-term care. The place has a staffing level around 3.68 nurse hours per resident per day, though the nurse turnover rate's a bit high at 34.9%. Therapists update treatment plans every day, and dietitians create meals tailored for personal tastes and necessary diets, serving three home-cooked dishes daily and handling special requests for diabetes or high blood pressure. There's help with things like bathing, clothing, toileting, grooming, and moving in or out of the bath, plus transportation for doctor's appointments, shopping, and spiritual offerings. Private and semi-private rooms are available, each with modern TVs, and there's a therapy gym, reading room, spa, steam room, fitness area, hairdresser or parlor, outdoor spaces, and recreation areas so residents can relax or spend time together. Residents get daily scheduled social activities, events, movies, and creative programs, with options for music treatment, animal therapy, board games, and outside trips, as well as exercise and wellness routines fit for individual needs.

The facility has a secured memory care unit and special Alzheimer's programs, with locked areas and security systems in place to prevent wandering for those with memory loss, and offers specialized therapies and psychiatric, hospice, case management, dementia, and physical rehabilitation units. Services include wound management, peritoneal dialysis, IV therapy, anodyne therapy, and help planning for discharge. Beauty services, a mobile stylist, dining room, and communal lounges help people feel comfortable and support friendships. It accepts Long-term Care Insurance and supports families with resources, plus guides them through care and insurance issues. There's a focus on providing higher levels of supervision and help compared to assisted living, letting residents keep as much independence as possible, though the place has had 17 reported deficiencies in inspections, including some infection-related problems, reporting issues, and some gaps in respiratory care, infection prevention, and mandated abuse reporting. The Home At Hearthstone carries approval from The Joint Commission, and it keeps restorative nursing programs going. While there's no publicly available information about the facility's reputation, people can expect a mix of skilled care, activities, and support every day, whether for a short-term stay or a longer time.
