    Unsafe dirty facility neglected residents

    I placed my father here and had to take him home. The building is dirty and rundown - urine, feces and cigarette smells, stained/broken bathrooms, holes in walls, and misleading photos. Nurses and admin were often rude, unresponsive and short-staffed; medications and basic care were missed and safety incidents were reported. A few staff (social worker, PT, some aides and the activity director) were kind and attentive, but they couldn't overcome the neglect and unsafe conditions. I would not recommend this facility.
