    Compassionate care and clean facility

I placed my aunt here and feel very reassured - the nurses, aides and Encore rehab team are caring, knowledgeable and attentive, rooms are bright and immaculately clean, and meals and activities keep residents engaged. Admissions and leadership were professional and helpful (Ken, Sarah and Aimee went out of their way), and the atmosphere feels warm and family-like with truly compassionate, high-quality care. I'm very pleased and would recommend it.

    Summary of reviews

Reviews describe a facility with clear strengths in clinical rehabilitation, daily programming, and a welcoming community environment alongside recurring operational concerns. Many families praised nursing and therapy staff as compassionate and skilled, and several accounts highlight effective rehabilitation outcomes and a therapy team that aided residents' return home. Admissions and administrative staff are frequently described as helpful and informative, and leadership is often credited with creating an inviting, updated interior and an array of activities that include parties, entertainment, and regular social programming.

At the same time, there are substantive concerns about consistency of care. Multiple accounts point to uneven staffing levels and frequent use of agency personnel, which reviewers connect to variability in caregiver familiarity, routine continuity, and responsiveness to resident needs. Relatedly, reviewers describe gaps in clinical monitoring and escalation (for example, missed diagnostic follow-up and delays in addressing acute changes), which have, in some cases, led to serious health issues. These should be understood as operational patterns rather than isolated descriptions: they reflect family-reported variability in how promptly and consistently clinical conditions are identified and managed.

Dining and activities are generally viewed positively: the facility offers a varied daily menu, celebratory meals, and abundant programming from morning through evening. That said, meal-service consistency is uneven in some reports—particularly meal temperature and service continuity. The activities program and social events are repeatedly cited as community strengths that contribute to residents' quality of life.

Facility condition feedback is mixed. Many reviewers note renovated, bright, and well-maintained interiors and comfortable rooms, while others describe specific cleanliness and odor-control problems in certain areas and inconsistent housekeeping following incontinence events. Safety-related concerns — including falls, transfer-safety practices, and unit-level policy enforcement (for example, visitation logistics and privacy protocols) — appear intermittently and merit proactive discussion with management.

Management and accountability receive both praise and criticism. Several families commend visible leadership and improvements in staffing and care delivery; others report frustration with complaint resolution, staff accountability, and the investigation of clinical concerns. A few reviews reference serious clinical lapses; these accounts suggest the facility should be asked directly about its incident investigation process, staffing ratios, agency use, and clinical escalation protocols.

For prospective residents and families: the facility offers notable strengths in therapy, activities, and an engaged admissions and leadership team, but experiences appear to vary depending on unit staffing and shift coverage. Recommended questions to pose during a tour include current staffing ratios and agency use, processes for clinical monitoring and physician availability, housekeeping and incontinence-care protocols, fall-prevention and transfer practices, and how complaints are investigated and resolved. This balanced approach can help families align the facility’s strengths with a given resident’s clinical and social needs.

    About Mount Washington Care Center

Mount Washington Care Center, a Limited Liability Company, has been serving the community since 1979, licensed by the Ohio Department of Health under license number 1708N, and holds an A+ rating as a BBB Accredited Business. You'll find they're open around the clock, every day, offering different levels of care, whether someone needs independent living, assisted living, skilled nursing, memory care, or even more complex services like ventilator care and hospice care. This place is agency free, has a dedicated long-term staff, and is actually five-star rated for quality, with certifications from Medicare and Medicaid, and membership in both the Ohio Health Care Association and the American Health Care Association. A person here gets personalized, individual care plans managed by Registered Nurses and Licensed Practical Nurses, with licensed Physical, Occupational, and Speech Therapists doing rehabilitation therapy, and if anyone needs it there's psychiatric help too.

Residents work with licensed dietitians who keep an eye on therapeutic diets, and they can also get help from a podiatrist, ophthalmologist, audiologist, dentist, psychologist, and even have the chance to attend church services without leaving the facility, which makes things easier. People have the choice of private rooms, pets are allowed, and every room has free cable TV and a phone, plus laundry, housekeeping, furnished meals, and an emergency response system. The place takes care of transportation and has controlled entry for safety, with lounge areas so families can visit comfortably and a hair salon too. Care plans let residents and families stay involved, supported by licensed social workers and staff trained to fit activities for folks at all different care levels, since having a regular schedule of activities and a chance to meet the staff and other residents matters.

Those coming for short-term rehab or respite care have a dedicated area for that, and everything here-from therapy services like speech, occupational, and physical, to monitoring medications and diets-is centered on a multi-faceted approach, which means looking after medical, emotional, social, and spiritual needs. Staff keeps a close eye on quality of life with a big focus on rehabilitation, dignity, compassion, and letting families be part of decisions. You can always find out about pricing and availability for different types of care, and for anyone needing help understanding options, there's a Family Advisor service. The center's got strong roots in the community, has a long-serving team you'll get to know, and carries on doing things in a way that seems focused on comfort, safety, and individualized care.
