    Good therapy but safety issues

I stayed three weeks. The therapy team and several staff were caring, meals (Thanksgiving) and therapy were excellent, and I felt comfortable at times. But my condition worsened - the place seemed understaffed, nurses were hard to reach, call buttons were ignored or poorly placed, meds were missed, and rooms were dingy with spotty cleaning. Mixed experience overall; good therapy and some kind staff, but I would not recommend it for rehab due to safety and staffing concerns.

    Summary review

Overall sentiment across the reviews is highly mixed and polarized: a substantial number of reviewers praise Valley Oaks Care Center for caring, attentive staff, strong therapy/rehab services, and positive events (for example Thanksgiving meals and veterans remembrance), while another significant group reports serious safety and care failures, poor cleanliness, and management/communication breakdowns. These opposing themes recur enough times to indicate inconsistent quality of care and variable experiences depending on time, unit, or staff on duty.

Care quality and safety are the most contentious themes. On the positive side, several reviewers explicitly praised nurses and therapy staff as kind, attentive, and effective—many called out the therapy team by name and reported health improvements and successful rehabilitation. Conversely, a number of reviews raise severe safety concerns: delayed or absent responses to call lights, reports that call buttons were difficult to reach or placed behind beds, medication errors or medications left on trays, and instances where reviewers allege no nurses or aides were present when needed. Some reviewers went further and reported alleged neglect and deaths, and others warned potential residents to avoid the facility. Because these are reviewer reports, they reflect perceptions and serious allegations that should be investigated further, but their presence in multiple reviews signals an important pattern of perceived lapses in timely and reliable clinical care.

Staffing, responsiveness, and staff behavior show a split pattern. Numerous comments applaud caring, attentive, and pleasant staff who provided personal attention, small gestures of kindness (for example notes about nurse kindness or cookies), and a warm welcome. Several reviewers described staff as helpful and professional. In contrast, other reviewers described aides as rude, staff frequently on their phones, requests ignored or delayed, and slow or non-existent responses when assistance was needed. Recurrent comments about being understaffed or not having enough nurses/aides align with the reports of delayed responses and missed care, suggesting understaffing may underlie many negative experiences.

Facilities and cleanliness comments are similarly inconsistent. Some reviewers explicitly said the facility was clean and had no odor, and appreciated features like murals and a nice environment. Others described the facility as dingy and smelly, with rooms barely cleaned and bedding not changed. These conflicting reports suggest variability over time or across units; cleanliness and upkeep appear to be inconsistent from one stay to another.

Dining, activities, and amenities also show mixed feedback. Several positive mentions highlight delicious holiday meals, good food, and staff efforts around events and special meals. Conversely, other reviewers said the food was awful or did not meet resident preferences. Activity programming and in-room amenities received negative notes: reports that activities were not offered, lack of an in-room phone, and limited TV stations. These comments indicate that while special events and meals can be well executed, routine dining preferences and activity programming may be unreliable or unevenly available.

Communication and management are frequent sources of frustration. Multiple reviewers reported difficulty reaching facility staff, being routed to a single contact number, slow responses to calls, and raised concerns to nursing leadership or the director with no apparent change. Several reviews described perceived mismanagement or alleged negligence by administration. These patterns—difficulty contacting staff, unaddressed complaints, and reported lack of corrective action—contribute significantly to the negative impression among dissatisfied reviewers.

A notable pattern is the variability and possible recent changes: some reviews emphasize staff improvement, new caring employees, and excellent therapy services, while others describe deterioration of condition during a stay and explicitly do not recommend the facility for rehab. This suggests experiences may differ by time period, specific units, or staff turnover. Many positive reviews point to specific staff members and therapy teams, while negative reviews focus on systemic problems (staffing, communication, safety) that persisted despite complaints.

In summary, the reviews present a mixed picture of Valley Oaks Care Center: strong points include pockets of excellent, compassionate caregiving, an effective therapy/rehab program for some residents, special events and hospitality (holiday meals, veterans services), and a generally pleasant environment reported by some. Major concerns raised repeatedly are safety and responsiveness (including alleged neglect and medication issues), inconsistent cleanliness, understaffing, poor communication from management, and uneven dining and activity offerings. Prospective residents and families should weigh these divergent reports carefully, seek up-to-date information from the facility about staffing levels, call-system and medication protocols, cleanliness procedures, and complaint resolution processes, and consider an in-person visit and references before making placement decisions.

    About Valley Oaks Care Center

Valley Oaks Care Center sits at 500 Selfridge Street in East Liverpool, Ohio, and serves as a 55+ community with a building that holds 67 certified beds and usually about 42 residents each day, so it's not overly large, but the place feels pretty active with both long-term stays for those who need ongoing help and short-term care for people recovering from illness or injury, and what you'll notice when you visit are the bright murals all over the halls-those came from Craig Wetzel's hand and survived the recent renovations, lending the building a bit of color and local character, and there's also some nice green space outside with parking and areas to walk around. The care programs have specialized choices for skilled nursing, rehab and therapy, memory care, continuing care, assisted living, independent living, palliative care, and a dedicated vent unit for folks needing help with breathing, plus you can find physical, occupational, and speech therapy here. Staff work on personalized care plans and help with day-to-day activities if needed, and there are restaurant-style meals and a calendar full of activities to give people some variety, though the facility did get a reported deficiency with nutrition and dietary management, meaning the menus hadn't always met the full needs or followed what a dietitian reviewed, which affected several people-and while there wasn't any actual harm at the time, it did signal there could be some risks if things don't improve. Two infection-related deficiencies showed up too, highlighting areas missed in infection prevention and control, but with no real harm noted then, just the chance of problems if issues continue. Altogether, the state's inspection found 29 total deficiencies at the last count. The nurse turnover rate is 35.6%, which sits under the state mark of 51.0%, and reported nurse staffing is about 3.41 hours per resident per day, slightly under Ohio's average. The owners are several trusts and individuals, all managed through a corporation, with Eli Gunzburg running things since 2011. Valley Oaks posts protections under the law for LGBTQ people and has rules posted about housing and public accommodations. The apartments offer things like ground floor, elevator, and easy-access features, though details change, and there's an online or phone option to set up a tour-staff are available to answer questions, help families look around, and give an honest feel for the place from the lobby to the outdoor areas, and visiting hours run every day, with 24/7 help if a visitor arrives outside hours. The facility puts some focus on making people feel at home, aiming for a family-like feel, and offers extras like internet, satellite TV, and places to gather or relax. The activities and dining are meant to bring people together, and they also have seasonal rentals, condos, and townhomes alongside the main care center, even if some of the exact rules and pricing aren't shown up front. Documentation says the place puts attention into specialized support for different care needs and family respite, has a vent unit and pulmonary rehab, and tries to create a space where residents feel valued, but, as with any facility, it's wise to look at recent reports and maybe tour in person to get a true sense of how things run day by day.
