    Great activities, but unsafe care

    I loved the warm, lodge-like feel, great amenities (gym, courtyard, salon, bar) and an outstanding activities director - my loved one was engaged and the food often felt restaurant-quality. Many caregivers were kind and attentive, but chronic understaffing and high turnover caused long waits, missed care, medication/communication errors and inconsistent housekeeping. I was alarmed by safety and neglect concerns (poor supervision, hygiene issues, reports of abuse) and by management's spotty responsiveness when problems were raised. If you visit, ask specifically about overnight staffing, incident reporting, hospice coordination and how complaints are handled before deciding.

    Prairie House sits on a 40-acre campus near Ascension St. John Broken Arrow and has a modern, multi-story building with landscaped grounds and a covered entryway, and it brings together Assisted Living, Memory Care, Independent Living, Skilled Nursing, and Senior Services so residents get care that fits what they need, and everywhere you look you see places for seniors to feel comfortable, including roomy studios and suites with kitchenettes and private baths, and common areas like a dining room with restaurant-style seating and light fixtures, bistro, lounge, patio, and movie theater with soft armchairs, and there are community rooms, an arts and crafts studio, a library, and a gym with adaptive exercise equipment for group classes like yoga and tai chi along with free weights, as well as an on-site salon offering haircuts and manicures, a spa, outdoor putting green, and gardens with walking paths and flower beds for fresh air and bird-watching or light gardening, and all the apartments come with cable, WiFi, and options for pets, even in Memory Care suites, which often feature large windows, cozy living areas, and small kitchenettes with granite counters.

    Residents, even those needing more support, can take daily exercise classes and join in wellness programs, yoga, stretching, walking clubs, and special programs led by trained activity leaders, and there's a full monthly calendar that's filled with outings to local museums, shopping, community events, nature preserves, music and cultural events, group games, horticultural activities, educational classes, family and community support groups, plus a theater for movies and special events, and for folks with a spiritual side, religious services take place on-site.

    Prairie House serves food in a Gold Leaf Dining Experience with meals planned by dietitians and Gold Leaf Chefs, always keeping in mind special diets, allergies, or texture needs, and residents can eat in the main dining hall, pick from a café menu, order tray service to their room if necessary, or join in for guest meals, and kitchenettes in the apartments let people make light snacks for themselves if they'd like, too.
