    Unsafe filthy neglectful care facility

I've been connected with Grace Living Center for over 15 years. A few staff were caring and helped my father, but overall the place is unsafe and filthy: persistent urine/feces smells, dirty carpets, pests, exposed wiring and missing ceiling tiles, and even open wounds/bedsores with maggots. Staffing and management were unresponsive and short-calls ignored, defective emergency cords, CNAs dispensing meds, allegations of nurses stealing medication, delayed hospital transfer for three days. Rooms lack basics (no in-room bathroom or sink, one outlet, shared shower), care quality is poor, and I would strongly advise against placing a loved one here.

    Summary review

Overall sentiment in the collected reviews is predominantly negative, with multiple reviewers describing systemic problems that raise serious safety and quality-of-care concerns. While a minority of reviewers praise individual caregivers and note positive short-term therapy outcomes, the preponderance of reports point to understaffing, unsanitary conditions, poor management responsiveness, and instances of apparent neglect and unsafe practice.

Care quality is inconsistent and, in many reports, deficient. Several accounts describe neglectful or dangerously inadequate clinical care: bed sores, at least one report of an open wound infested with maggots, delayed IV removal, and delayed hospital transfer (one report specifying a three-day delay). There are also multiple allegations concerning medication handling — CNAs dispensing medication and accusations of medication theft by nurses — which, if accurate, represent major lapses in clinical governance and patient safety. Some reviewers explicitly characterized the skilled nursing and rehab care as poor and unsafe; others reported a more positive rehab experience with quick improvement, indicating significant variability in outcomes that may reflect inconsistent staffing or supervision.

Staffing and staff behavior emerge as a central theme. A large number of reviews report chronic understaffing, zero staff availability at times, and limited coverage that impacts basic care and responsiveness. Despite these system-level staffing issues, several reviews single out individual staff members as cordial, caring, and helpful; some families expressed gratitude for specific caregivers and noted long-tenured employees (one report cited more than 15 years). However, there are also troubling reports of unprofessional behavior — staff allegedly stalking residents, talking about residents behind their backs, and an overall lack of trustworthiness. Combined with accounts of unresponsiveness to nurse calls and defective emergency call cords, these behaviors compound concerns about resident safety.

Facility condition and maintenance are repeatedly criticized. Multiple reviewers described strong and persistent odors of urine and feces, dirty carpets, and an overall lack of cleanliness. Physical hazards were noted, including exposed wiring, missing ceiling tiles, and lack of basic safety features such as rails outside rooms. Room accommodations are reported as poor or minimal: no in-room bathrooms or sinks in many rooms, only one bathroom per wing, shared overhead lighting, a single power outlet, and shared showers/tubs among patients. Pest problems (scorpions mentioned) were also reported. These environmental deficits, paired with hygiene issues and poor hand-hygiene practices, increase infection and injury risk for residents.

Management, oversight, and reputation are areas of consistent concern. Many reviewers described upper management as unresponsive or unprofessional, with some saying they would not recommend the facility and would not allow family members to stay there. There are mentions of a name change and a broader bad reputation, suggesting either recent operational changes or attempts to rebrand amid ongoing problems. Reports of unresponsiveness to complaints, lack of follow-through on dietary restrictions, and poor handling of safety incidents (e.g., delayed transfers and broken emergency systems) indicate weaknesses in leadership, accountability, and quality assurance systems.

Dining and therapy were mixed topics. A few reviewers described the food as adequate, but others reported that dietary restrictions were not followed. Therapy and rehab experiences varied: some families noted quick improvement and effective care, while others called the rehab services negative and insufficient. This pattern of mixed experiences suggests that individual staff members can provide good care, but that such care is not reliably delivered across shifts or patients.

In summary, the dominant themes are inadequate staffing, serious cleanliness and maintenance problems, safety hazards, and troubling reports of clinical neglect and medication mishandling. Positive comments focus mainly on individual caregivers and isolated successful rehab outcomes, but these do not offset the numerous and severe negative reports. Based on the frequency and nature of the allegations — including wounds with maggots, faulty emergency systems, medication concerns, and pervasive odors/pests — the reviews collectively suggest significant systemic issues that potential residents and families should weigh heavily. The most common recommendation across reviewers is to avoid entrusting loved ones to this facility until there is clear, documented improvement in staffing levels, infection control/cleanliness, clinical oversight, and management responsiveness.

About The Wilshire Skilled Nursing & Therapy

The Wilshire Skilled Nursing & Therapy is a skilled nursing facility with 56 certified beds and has been under managerial control by Bridges Esop, Inc since December 2020, and it's affiliated with Bridges Health. The building is single-story and accepts Medicaid, and the facility's also called Wilshire Nursing Skilled and Therapy. Staff provide skilled nursing care and intermediate care for very frail residents who can't take care of themselves alone and need regular nursing help, and their care planning fits each person's health needs. The place serves residents who need short-term rehabilitation after hospital stays, with therapy services like physical, occupational, and speech therapy for people recovering from strokes, surgeries, broken bones, joint replacements, or illnesses like heart attacks and respiratory problems. There are programs for wound care, diabetes, catheter and colostomy care, as well as cardiac support. The center has a clinical team made up of competent and highly trained people who attend to the residents' daily needs and help with personal hygiene, medication, and laundry. Meals get planned by dietitians to make sure they're both healthy and good-tasting for seniors, and the staff organize activities and events for residents' comfort. There are features and amenities designed to help the residents recover, feel comfortable, and keep some independence. As of now, staffing levels are about 3.76 nurse hours per resident every day, but the nurse turnover rate stands at 70.8%, which means new people come and go from the nursing staff fairly often. The latest inspection reports listed 12 deficiencies, with some issues found in pharmacy services, infection control, and activities to meet personal needs, including a September 2024 citation for infection control and a pharmacy service citation F0761 relating to medication labeling and storage, as well as deficiency F0679 for activities not meeting every resident's needs. Mildred Chapa serves as the Administrator. The facility houses 56 residents in total.
