    Neglect, dehydration, theft, unsafe care

I liked that many staff were kind, attentive, and the activity director was pleasant and helpful, but my overall experience was alarming. My bed-bound loved one was often left without water in their room, became dehydrated and later developed sepsis, missed baths and basic care, and personal items (clothes, a razor) went missing. Rooms are small, the lounge is rundown despite remodeling, and I encountered rude staff and management - I cannot recommend this facility until safety, theft, and basic care failures are fixed.

    Summary review

The reviews of Woodlands Skilled Nursing & Therapy present a mixed picture with clear strengths in communal spaces, certain staff roles, and individualized praise from some families, but also serious, recurring concerns around basic care, safety, and facility upkeep. Positively, several reviewers call out an inviting dining room and lobby with large windows that provide pleasant views (notably birds during meals), which contributes to a welcoming atmosphere for residents during dining. An activity director is repeatedly described as pleasant and helpful, and multiple accounts praise individual staff members as kind, caring, and responsive—specifically mentioning attentive assistance with feeding, bed changes, and responding to requests. There is also evidence the facility is undergoing remodeling, which some reviewers mention as a potential improvement. A subset of reviewers even explicitly recommend the facility based on the attentive aspects of care they observed.

However, these positive notes are balanced by several serious and specific concerns. Physical space issues are mentioned: resident rooms are described as small and a common lounge area as old and unkept. More alarmingly, multiple reviews allege lapses in basic personal care and safety. Examples include missed baths and failure to ensure adequate water availability in patient rooms; one review explicitly links poor water access to dehydration that progressed to sepsis. There are also broader allegations of caregiver neglect and reports that some patients were bed-bound and in need of more attention than they received. These issues point to potential systemic problems in care delivery or staffing levels/oversight.

Security and personal-property concerns are another repeated theme. Several reviewers report theft or unexplained loss of personal items, including clothes and a razor. These complaints raise safety and management issues beyond routine care quality—families are concerned about the protection of residents’ belongings and the facility’s processes for tracking and safeguarding personal items. Compounding these problems are reports of rude behavior from some staff members and management, which contrasts with the praise other reviewers offer for individual caregivers. This mix of courteous, attentive employees and reports of rudeness or poor managerial behavior suggests inconsistency in staff training, culture, or supervision.

Taken together, the reviews reveal a facility with promising elements—pleasant communal dining, caring individuals (notably the activity director and some clinical staff), and active remodeling—that nevertheless struggles with critical operational failures for some residents. The most significant red flags are missed basic care tasks (bathing and hydration), an allegation of resulting severe medical harm (sepsis), reported caregiver neglect, and theft of personal items. These are not minor complaints and indicate areas that would warrant prompt attention from facility leadership: hydration and basic care protocols, staffing and supervision, personal property security, and resident-family communication. Prospective families should weigh the positive interpersonal reports and improving physical spaces against these serious care and safety concerns, and should ask facility management for concrete, recent evidence of corrective actions, protocols for hydration and bathing, inventory/security practices, and staff training/turnover before making placement decisions.
