    Neglectful unsafe care, avoid facility

    I regret placing my loved one at Shadyrest Home (Pryor, OK) - overall an extremely negative experience. Caregivers were often absent, the RN was unavailable for hours, and I witnessed serious safety/care failures: a clogged, unaddressed feeding tube, water bag not refilled, improper flushing/positioning, and long delays in response. I also saw unprofessional and disturbing behavior (even Avon reps taking orders from a dementia resident) and felt staffing and corporate leadership were inadequate. To be fair, some staff were courteous, housekeeping and the building were clean, and the cooks/food were good - but the neglect and safety concerns make me advise avoiding this facility.
