    Clean, caring staff; inconsistent management

I had a mixed experience. The place is clean, homey, and many aides and the therapy team were caring and competent - residents seemed happy and rehab was good - but I also saw serious mismanagement: rude/unhelpful staff at times, poor communication (families not informed, no discharge paperwork), delayed meds/bloodwork/therapy, missing home oxygen orders, ambulance transfers and even a premature return to hospital. Given the long waitlist and higher price, proceed with caution and closely monitor care.

    Summary review

Overall impression: The reviews for The Gardens are highly mixed, with a strong divide between families reporting excellent, attentive care and those reporting serious lapses in clinical care, communication, and facility upkeep. Many reviewers praise the staff, therapy team, cleanliness, and overall environment — describing the place as homey, safe, and well-suited for rehabilitation and long-term care. At the same time, a substantial number of reports describe troubling incidents of neglect, clinical safety failures, and poor management practices. The result is a facility where experiences appear inconsistent: some residents and families receive very good care, while others encounter significant and sometimes dangerous problems.

Care quality and clinical concerns: Several reviewers report high-quality hands-on nursing care and an excellent physical/occupational therapy team that helped with recovery and mobility. Conversely, multiple reviews cite delayed medications, delayed blood work, and postponed or mismanaged physical therapy. More serious clinical incidents are also described: dehydration, urinary tract infections, pneumonia, bruising, improper feeding-tube handling, and alleged neglect during rehab. There are reports of patients being sent back to hospitals, premature discharges, and missing critical home-oxygen orders — all of which are serious safety concerns. These clinical issues suggest variability in medical oversight and inconsistency in following care plans.

Staff behavior and communication: Staff performance and demeanor are a major theme with polarized views. Many reviewers praise caring, attentive aides and cooperative administration, noting responsive nursing staff and helpful office personnel. However, there are numerous reports of disrespectful staff, poor communication with families, failure to inform family members of changes or discharges, missing discharge documentation, and even staff yelling at ambulance drivers. Several families explicitly reported the facility failed to notify them about transfers or medical issues. This inconsistent communication and occasional disrespectful conduct are key negative patterns that undermine trust.

Facilities, cleanliness, and dining: Multiple reviewers describe the facility as clean, homey, and comfortable with private rooms and accessible layouts for wheelchairs. At the same time, others mention cold rooms, sparse furniture, bad odors, and soiled linens being left in bathrooms. Dining reports are mixed: some families praise the food, while others describe meals as trucked in and reheated and note problems such as no water being provided with meals. These contradictory reports indicate that physical conditions and housekeeping/dining standards may vary by unit, floor, or staffing shift.

Rehab and therapy experience: Rehabilitation and therapy receive consistent praise from several reviewers who described an excellent therapy team and good rehab outcomes. Others, however, recount delays in therapy start times or what they perceived as neglect during the rehab process. This suggests the facility can deliver strong therapy services but may have capacity or scheduling issues leading to inconsistent experiences.

Management, admissions, and logistics: Several reviews highlight a pleasant, informative admissions/sales director and reasonable rates, but there are also repeated complaints about long waiting lists (up to six months), poor communication during the admissions process, and unhelpful staff advising prospective residents to keep looking. Management concerns are raised explicitly by multiple reviewers, tied to inconsistent staffing, inadequate oversight during quarantine, and failures around discharge and readmission procedures. These systemic and administrative issues contribute to many of the safety and satisfaction problems reported.

Patterns and takeaways: The dominant pattern across reviews is inconsistency. Positive reports emphasize caring staff, effective therapy, cleanliness, and satisfied families; negative reports emphasize clinical safety issues, poor communication, and lapses in basic hygiene and meal service. The facility appears capable of high-quality care in many cases but also has documented instances of neglect and serious mistakes. Prospective residents and families should weigh both sets of experiences: ask for specifics about staffing ratios and oversight, request copies of discharge and medication plans, clarify how therapy and nursing are scheduled, and visit multiple times (including evenings and weekends) to assess consistency. If considering The Gardens, it's prudent to monitor care closely and maintain active communication with staff and management to reduce the risk of the negative outcomes described.

    About The Gardens

The Gardens sits at 1165 South Brenner Road in Sapulpa, Oklahoma, where country views and quiet surroundings meet a growing suburban area, so you're not far from shopping, banks, or medical clinics, but you can watch wildlife and look out at the peaceful nature all around the place, too. This care facility serves as a Nursing Home and provides both skilled nursing and residential care, so you get long-term care, short-term recovery after illness or injury, and even outpatient therapy if you qualify, with therapy rehabilitation services handled by licensed professionals in physical, occupational, and speech therapy, and they've got a staff of nurses, therapists, and physicians you'll see around regularly. The Gardens also gives you personalized care programs, 24-hour nursing by RNs, LPNs, and CNAs, and there are always services offered for medical management, activities, and daily living support, plus consultant care services like pharmacy and dietary help if your needs change, and physician oversight and health check-ins, which adds a layer of security and regular attention. They've even set up short term respite care, including furnished rooms, meals, activities, and full housekeeping plus laundry, which is good if you only need temporary help. Amenities connect right through the services-comfortable spaces, daily activities, and peaceful landscapes, so even the therapy programs focus on keeping everyone engaged and involved. You'll see they've got a team led by Candace Barnes as the Administrator, and, being part of Phoenix Healthcare, LLC, they use internal audits and tracking to keep up quality care, with a record proving over 20 years of steady outcomes. Payment's flexible, too-Private Pay, Long Term Care Insurance, Medicare, Medicaid, and some private insurances, including Community Care, so there are several ways to cover the costs, and they help residents and families find what works. The Gardens stays open 24/7 for residents' safety and shelter, but the main office runs during the weekdays, 8:30 AM to 5:00 PM, and the place keeps online communication handy, with a website at phoenixhealthcarellc.com and a "Send Email" feature, so families can check in easily or get questions answered. All in all, The Gardens covers a lot of ground with skilled nursing, long-term and short-term care, consultant and therapy services, and a peaceful atmosphere, and while it isn't fancy, it keeps things comfortable and gives practical care options for seniors who need more help day-to-day.
