    Compassionate professional care and recovery

I'm very satisfied with the care my mom received - the nurses, CNAs, therapists and social worker (Melissa) were compassionate, professional, and truly went above and beyond. Thanks to the excellent PT/OT she was discharged walking and even went home without a wheelchair. The facility is spotless, homey and well run, with delicious restaurant-style meals, engaging activities, easy visiting, and supportive administration. I highly recommend this place.

    Summary of reviews

Reviews of Greenery Center for Rehab and Nursing are highly mixed, with a clear split between families who describe strong clinical and hospitality performance and those who report operational and care‑quality concerns. Positive accounts consistently highlight effective rehabilitation services, caring staff, active programming, and recent facility improvements. Negative accounts raise issues about cleanliness, responsiveness, medication and safety processes, and variability in staff conduct. Prospective residents and families will find both encouraging examples of successful discharges and reports indicating systemic gaps that merit inquiry.

Care quality and clinical services: Many reviewers praise the facility’s physical and occupational therapy programs, noting well‑equipped gyms, certified therapists, and rehabilitative outcomes that enabled home discharge without assistive devices in some cases. Nursing and CNA staff are frequently described as compassionate and professional, with specific staff and social‑work personnel singled out for responsiveness. Conversely, there are recurring concerns about medication management (delays and errors), wound care and infection‑control practices, and inconsistent help with activities of daily living. These issues suggest variability in clinical oversight and follow‑through across different shifts and units.

Staff, operations, and management: A prominent theme is a core of dedicated, friendly staff and accessible leadership, including social-work and admissions staff who facilitate transitions. At the same time, reviewers describe uneven staffing levels, use of temporary workers, and inconsistent responsiveness to call bells and resident needs. Communication with families varies: some report clear, proactive updates and open administration, while others report infrequent care conferences and difficulty reaching staff by phone. Renovation and new‑management efforts are mentioned positively, but several operational problems—particularly around staffing continuity and front‑desk/phone responsiveness—remain areas to verify.

Dining, activities, and resident life: Dining receives mostly favorable comments for menu variety and restaurant‑style meals, and the kitchen staff is often called personable and accommodating. There are also comments about processed food and limited availability of certain therapeutic diets, which points to variability in menu accommodation. Activity programming, including bingo, arts and crafts, Bible study, and transportation to outings, is a consistent strength and contributes to a family‑like atmosphere. Noise control in corridors and inconsistent announcement of events are occasional detractors from the resident experience.

Facilities and safety: The facility includes recently remodeled areas, particularly a west wing described as clean and modern, along with outdoor patios and tidy common spaces. At the same time, reviewers note very small resident rooms in some units, limited maneuvering space for wheelchairs, and sanitation concerns in parts of the building and during some shifts. Safety‑related operational gaps are reported (for example, inconsistent use of bed alarms, rails, or transfer supports and delayed responses), indicating the need to confirm current protocols and monitoring practices during a tour.

Bottom line and recommendations: The Greenery Center offers robust rehabilitation services, engaged activity programs, and many dedicated staff members; these are reasons families have had positive experiences. However, consistent themes around sanitation practices, infection‑control and wound care, medication management, staffing variability, and communication indicate operational weaknesses that could materially affect resident well‑being. Families should balance the facility’s therapy strengths and recent renovations against these concerns by asking specific questions during a visit: current staffing ratios and use of temporary staff, infection‑control and wound‑care protocols, medication administration procedures, availability of therapeutic diets, room dimensions for mobility needs, and channels/frequency for family updates. Verifying these items in person and requesting recent quality or inspection documentation will provide a clearer picture of current performance.

About Greenery Center for Rehab and Nursing

Greenery Center for Rehab And Nursing sits in Canonsburg, Pennsylvania, in a spot that's only a few minutes from major hospitals and offers quick access to Interstate I-79 and Pennsylvania 519, so getting there is pretty straightforward for most families, and once inside, you'll notice the place works hard to provide skilled nursing, rehab, memory care, and palliative care, but it also handles things like pain management, advanced diabetes care, and hospice care, so people with all sorts of needs end up here. The facility runs as a 140-bed, for-profit partnership owned by Greenery Holdings LLC, and they've managed the place since May 2016. They focus on developing personal care plans shaped around each person's goals, which matters because everyone needs something a little different.

Folks at Greenery Center can get physical, occupational, and speech therapy in a modern gym with an ADL suite, and you find things like post-surgical rehab and stroke rehabilitation services too, while daily activities, religious services for different denominations, and things like a full-service beauty shop or salon offer a bit of comfort and socialization, which keeps things lively for residents. They even organize monthly outings for things like shows, shopping, or trips to the Meadows Casino, and rooms come with smart TVs, telephones, phones, and free Wi-Fi, plus a dining room set up for eating together followed by activities.

You'll see they've got a dietician handling all kinds of dietary needs, and there are visits from on-site doctors, an audiologist, a podiatrist, an optometrist, a dentist, and wound care specialists who come regularly. Laundry services get handled, and piped-in oxygen, BIPAP, and CPAP therapy are available when required. The center provides memory care programs for those with more specific cognitive needs and palliative and respite services for short-term stays, so families looking for specialized options can find them.

The staff is mostly highly trained, and their approach is holistic, aiming to address all aspects of health, but reports show that the place has had several documented deficiencies, including infection control and complaints about safety and resident rights violations, and these are filed in public inspection reports. Their nurse turnover rate is reported at 63.3%, which is something to consider, as well as their nurse staffing hours per resident per day coming out to 3.48 hours. They only have staff who speak English, and as of now, they aren't accepting new patients, which might affect availability for those looking to move in soon. All in all, Greenery Center for Rehab And Nursing offers a wide range of services, modern features since their remodel in 2019, and a mix of long-term and short-term care in a place with a history of both compassion and some reported issues, so reading public inspection reports and doing a bit more research can help families get the full picture.
