    Neglect, poor communication, unsanitary conditions

I cared for my mom here and was deeply disappointed: staff were often unprofessional or rude, communication was poor, and intake/discharge was disorganized. Call bells and nurse responses took hours, home health and nurse/doctor visits were delayed or never happened for months, and we saw neglect (left without oxygen, missed baths), unsanitary conditions (mold, bedbugs, water damage, trash not emptied) and a privacy breach. A few aides and the new Director of Nursing were kind and responsive, but overall I would not recommend this facility.

    Summary review

Overall sentiment across the review summaries is predominantly negative, with a number of recurring and serious concerns about care quality, facility conditions, and administrative operations. Multiple reports describe long nurse call bell response times and poor staff responsiveness, which families and reviewers link to neglectful outcomes such as residents not receiving timely assistance, not getting baths, and even being left without required oxygen for hours. Several reviewers explicitly stated they would not recommend the facility and characterized care as poor or worse.

Staff performance is portrayed as mixed but leaning negative. While a few reviews praise individual aides and note that some staff are "amazing," caring, and helpful, the majority describe unprofessional, rude, or disrespectful behavior. Reports include verbal and mental abuse, staff ignoring residents, difficulty locating clients when needed, and poor communication with families. There are also operational complaints about a disorganized intake and discharge process, delays setting up home health services and nurse appointments, and a perception that insurance was billed for services that were not delivered. One review cites a privacy breach by an administrator contacting a relative, prompting consideration of formal complaints.

Clinical care concerns are prominent. Several summaries state residents went long periods without provider visits or adequate rehabilitation, with one noting no doctor visits for over six months and another reporting only minimal rehab. Critical safety incidents were alleged, including a patient left without oxygen near the nurses station for hours. Family members reported that some residents were bedbound on oxygen and not receiving appropriate attention. These accounts raise substantial questions about clinical oversight, monitoring, and adherence to care plans.

Facility and hygiene issues are repeatedly called out. Specific problems mentioned include missing trash containers in bathrooms, trash not being emptied for days, food being tossed in the trash rather than served, as well as more severe environmental concerns such as bedbugs, mold in bathrooms, water damage to ceilings, bags over windows, and generally broken-down facility elements. Together these descriptions suggest lapses in housekeeping, infection control, and physical plant maintenance that could affect resident safety and comfort.

Management and leadership present a mixed picture. Some reviews note positive changes or individuals in leadership — specifically a new Director of Nursing who is described as responsive, kind, and caring — and families who experienced improved communication and faster answers to questions. However, these positives are set against repeated comments about disorganization, poor communication, and administrative failures (for example, in coordinating post-discharge home health), indicating inconsistency in leadership effectiveness and operational follow-through.

Common patterns across the reviews point to systemic issues rather than isolated incidents: recurrent slow response times, staff professionalism and training gaps, maintenance and sanitation shortfalls, and lapses in clinical oversight. While there are isolated reports of good care and responsive leadership, the volume and severity of negative reports—especially those involving neglect of medical needs and environmental hazards—are serious. Prospective residents and families should weigh these concerns heavily and consider seeking independent verification of improvements (e.g., recent inspection reports, evidence of remediation for pest/mold issues, staffing and training changes, and confirmation of reliable clinical coverage) before making placement decisions.

    About Greenfield Healthcare and Rehab

Greenfield Healthcare and Rehab sits over in Erie, PA, where they offer a place for seniors who need skilled nursing, rehab, and long-term care, and folks living there get care 24 hours a day from the nurses and staff, and you'll notice there's all sorts of meals that get planned to fit special diets like for those living with diabetes or heart problems, so if you need certain foods, they'll do their best to help with that, and besides meals, there's a focus on making the place feel comfortable, almost homelike, with cozy rooms, spaces for recreation, and social events or activities so residents can interact if they want or just spend time however they like. The building's got about 133 certified beds, and on an average day, around 79 people call it home, which means there's usually room to get to know one another without feeling too crowded or too empty, and if someone's been in the hospital and needs rehab, or maybe long-term help, Greenfield offers services aimed at improving health, such as physical therapy, speech therapy, and occupational therapy, and therapists and nurses are available for those specific therapies as needed, and Greenfield provides specialized medical care for people dealing with serious issues like pain, wounds, stroke recovery, cardiac or respiratory care, tracheostomy, diabetes management, and orthopedic recovery.

You'll find advanced care available, including help for folks coming in for short-term rehabilitation after surgery, or those who might need hospice care, memory care, or respite care, and along with the staff's efforts to look after residents' well-being, they have programs and social gatherings to keep everyone involved if that's wanted, so being in a community where something's always going on is part of their model. They accept both Medicare and Medicaid, which matters to many families, and Greenfield is a for-profit corporation that's managed by Elizabeth Kachel, Charles Rohrbach, and Abraham Smilow, with links to Ghc Snf Opco Holding Company LLC and other entities like Hillel Tropper 2016 Irrevocable Trust, and Abraham Smilow also has a connection in both management and ownership. Even with all these services, you'll want to be aware that inspection reports from CMS show Greenfield has had some problems over time-there have been forty deficiencies found in their reports, and three of those were infection-related, meaning the inspectors found concerns about infection control that did not meet federal standards, and there were other issues like not always having proper doctor reviews, not documenting everything as required, and not honoring every resident's rights all the time, so families should know this part too.

On staffing, Greenfield reports nurse hours per resident each day at 3.21, which falls below the Pennsylvania state average of 3.9, meaning nurses might be stretched a little thinner, and the nurse turnover rate stands at 61.4%, higher than the state's average of 47%, so some residents might notice staff changing more often than in other places. Still, the care team keeps focused on providing support, especially for those who need help regaining mobility or living skills, and they aim for recovery with respect and a positive outlook, but it's good to remember the facts about staffing and inspection history when comparing options. The place offers regular activities, good food that tries to meet everyone's health needs, and a setup that tries to make people feel respected as individuals-so for someone in Erie or nearby needing skilled care and a range of rehab options, Greenfield Healthcare and Rehab provides many services under one roof, with a clear effort to deliver both medical and daily support, though it's wise to consider all inspection and staffing details before making a choice.
