    Improved care after prior problems

I've had a mixed experience. Many nurses and staff were wonderful, caring and professional-my mother was well cared for, her room was clean, PT/OT and food were excellent, and communication was good. However, prior management (including an unprofessional DON) caused safety and cleanliness problems (bed bugs, slow call responses, even a hospitalization); since new leadership arrived I've seen real improvement and problematic staff were removed, but I remain cautious.

About Premier Armstrong Rehabilitation and Nursing Center

Premier Armstrong Rehabilitation and Nursing Center sits in Kittanning, Pennsylvania, and you'll also hear folks call the area Applewold, and it's a for-profit facility run by Pollak Holdings, owned by Elie and Theodore Pollak, and Elie Pollak's held managerial control since May 2021. The center's got 113 certified beds and usually cares for about 86 residents a day, so it's not too crowded, though staffing's a concern because inspection reports say nurse hours per resident per day run at 3.46, lower than the state average of 3.9, and sometimes there aren't enough nurses or a licensed nurse on every shift, which got marked as a deficiency. There've been 84 total deficiencies-including four about infection-and a major one for not always ensuring a safe, clean, and homelike place for residents, and those are worth knowing. The building's got private and shared rooms, each furnished with air conditioning, private bathrooms, and phone, cable, internet, and Wi-Fi, plus communal lounge areas with flat screen TVs and spots for families to gather, so there's space to visit and rest. Premier Armstrong serves folks who need short-term rehab after a hospital stay and those needing long-term nursing support, and they have skilled nursing, physical therapy, cardiac and neurological care, stroke rehab, wound care, daily personal care, medication management, and coordination with outside healthcare providers, so people with all sorts of medical issues end up here.

Staff include licensed nurses, certified nursing assistants, and physical therapists, and they help with moving around, personal care, and checking residents' health night and day. Security's in place with handrails, a wandering prevention system, an emergency response setup, and smoke and heat detectors, aiming to keep everyone safe, especially those with memory problems or wandering risks. Residents get three meals a day, prepared and served, and have help with housekeeping, laundry, and getting to appointments, plus there's transportation arranged for medical visits and other needs. Amenities include private suites, shared rooms, private bathrooms, air conditioning, telephone, cable TV, internet, Wi-Fi, and a call system that works 24 hours, and some rooms have big spaces for family gatherings. The place is licensed and accredited, including a license for MS Physical Therapists and US DHHS Skilled Nursing Facility status, though there's no list of specific accrediting organizations provided. For more than 25 years, the center's been recognized for its quality care, though their inspection record does show ongoing challenges with meeting some important state and federal standards. Premier Armstrong tries to offer a secure, supportive setting for seniors who need medical care and daily help, and its specialty programs and comprehensive approach give families some peace of mind, though being aware of the deficiencies and lower staff levels is important for making a decision.
