    Excellent rehab; issues with care

I had a mixed experience. The nurses, aides, therapists and administration were compassionate, hands-on and communicative - the therapy team helped my mother regain strength, range of motion and swallowing and get home ahead of schedule in a clean, small, homey setting. However, staffing shortages produced long call-bell delays and unresponsive overnight staff, there were lapses in cleanliness/infection control, a medication was discontinued that left her critically ill, and we dealt with lost laundry, missing equipment and inconsistent food (often excellent home-cooked, sometimes awful or not diabetic-appropriate). Overall: excellent rehab and caring people, but you must advocate and watch for staffing/quality issues.

    Summary review

Overall sentiment about Harmony Hills Healthcare & Rehabilitation is strongly mixed but leans positive when considering clinical care and rehabilitation outcomes. The most frequently cited strengths are compassionate, attentive staff and an effective therapy program. Numerous reviews praise nursing teams, therapists (PT/OT/SLP), and a visible, involved administration (several reviewers name the administrator and social workers positively). Many families reported excellent rehab outcomes: patients regained mobility, improved range of motion, and left confident to return home. The small size and "family-like" atmosphere are repeatedly highlighted as contributors to personalized care, frequent updates, and a sense that residents are "not a number." Several reviewers also noted strong coordination between departments, effective discharge planning, and availability of post-discharge support and resources.

Staff quality and interpersonal aspects are among the facility's clearest strengths. Reviewers repeatedly described staff as kind, respectful, and going above and beyond — from nurses and CNAs to activities staff, social workers, and therapists. Housekeeping and maintenance receive frequent praise in many accounts, and a substantial number of reviews emphasize cleanliness, pleasant smells (absence of industrial odors), bright rooms with big windows, and well-kept common areas such as sun rooms and lounges. Multiple reviewers call out specific staff members (administrators and social workers) by name for responsiveness and helpfulness. Activities programming (music groups, bingo, celebrations, crafts) is frequently mentioned as engaging and contributing positively to resident morale.

Despite the many positive accounts, there are consistent and significant negative themes that cannot be ignored. The most common concerns relate to staffing levels and responsiveness: several reviewers reported chronic understaffing, long call-bell delays (particularly at night), aides being overworked, and high staff turnover. These staffing pressures are linked in some reviews to slow response to resident needs, missed medications or delayed medication administration, and inconsistent delivery of care. A smaller but serious subset of reviews documents medication errors or omissions, and one review described a medication discontinuation associated with a critical decline. Safety-related reports include isolated but alarming claims such as residents left on bedpans for extended periods, medications left in hallways, and urine or mold in showers.

Cleanliness and infection control are areas of stark contradiction in the reviews. Many reviewers praise the facility as spotless and odor-free, while others report dirty floors, unclean bathrooms, overflowing trash, and specific infection incidents (scabies, C. diff, MRSA). These conflicting accounts suggest variability over time or by unit/staffing shift. Related operational concerns include a small and crowded physical-therapy room with limited equipment for some patients, cramped or shared bathrooms in certain rooms, and occasional problems with laundry and missing personal items.

Dining and nutrition quality are also described inconsistently. A significant number of reviews compliment home-cooked meals, daily soups, and improved food quality, while other reviews characterize the dining experience as poor — cold food, prepackaged items, or low-quality menus (examples cited include hot dogs and baked beans). At least one review noted a failure to provide physician-ordered supplements and limited beverage options, and some mentions indicate the kitchen faces space constraints that can affect meal preparation. The facility has been cited by state inspectors for dining-related issues in at least one review, which aligns with the mixed feedback.

Management and culture are another area where opinions diverge. Many reviewers praise the administrator as engaged, personable, and hands-on, describing transparent communication, frequent family updates, and leadership presence on the unit. Conversely, other reviewers report a blame culture, unhelpful or "snooty" administrative responses to complaints, inconsistent follow-through, or perceived corporate profit motivations. These mixed perspectives indicate that managerial performance may vary between shifts or over time, or that individual experiences differ depending on which staff are on duty.

Other recurrent points: reviewers frequently note the facility's scenic grounds and pleasant common areas, and some praise the center as pet-friendly and social. The facility's small size is viewed mostly positively for personalization and familiarity, but it also contributes to resource constraints (limited therapy space, fewer aides available). Cost concerns are raised by some families, with at least one review describing private-pay rates as "outrageous." Several reviews reference state citations or Department of Health actions, underscoring the need to weigh positive personal experiences against documented compliance issues.

In summary, Harmony Hills Healthcare & Rehabilitation appears to provide high-quality, compassionate clinical and rehabilitative care for many residents, with notable strengths in therapy outcomes, nursing compassion, personalized attention, and engaging activities. However, there are repeated and significant reports of understaffing, inconsistent cleanliness and infection-control practices, variable dining quality, occasional medication and safety lapses, and uneven management responsiveness. These mixed signals suggest the facility can deliver excellent care, particularly when staffing and leadership are strong, but prospective residents and families should proactively investigate current staffing levels, recent inspection results, infection-control records, dining practices, and consistency across shifts. Visiting in person, asking for recent state inspection reports, and speaking directly with administrators about staffing, therapy equipment, and quality-improvement actions will help clarify whether the on-the-ground experience aligns with the overwhelmingly positive accounts or the concerning negative reports.

About Harmony Hills Healthcare & Rehabilitation

Harmony Hills Healthcare & Rehabilitation sits in Wexford, Pennsylvania, in a mansion-turned-rehabilitation center offering both nursing care and rehab services for seniors. The building holds 55 certified beds and splits into two units, with 25 long-term care beds downstairs in the main mansion and 30 subacute beds upstairs. Staff nurses work on site, and therapy staff are there six days a week, while the facility provides skilled nursing, intermediate care, and dementia care. Residents get services like long-term care, subacute rehab, respite care for short-term stays, wound vac therapy, ostomy care, trach care, physical, occupational, and speech therapy, and hospice and palliative care, all focused on a patient-centered approach that tries to treat people with respect, dignity, and empathy.

The building features semi-private rooms, laundry services (both staff-provided and self-serve), Wanderguard for safety, a fully equipped gym, and a practice kitchen for therapy. The center's management and ownership include Limestone Enterprises LLC, Yisroel Moskowitz, Elliott Gestetner, and a few others, with many sharing responsibility over operations. It's a for-profit corporation with indirect ownership by Gem Family 2020 Ngcg Nevada Trust. On average, about 48 residents live there each day, and nurse staffing comes out to around 4.50 hours per resident daily, though the nurse turnover rate is above 68%. It's close to local hospitals like AHN Wexford, UPMC Passavant, UPMC Cranberry, St. Clair Hospital, and UPMC Mercy, which helps with access to medical care.

People can pay using private pay, private insurance, long-term care insurance, Medicaid, Medicare, managed care, or several other ways. The home gets inspected and must meet federal rules to help stop infection from spreading among residents, but it had 36 deficiencies in inspection reports, including problems with pharmacy drug labeling and storage, informing residents about arbitration agreements, nutrition and dietary standards, and some infection issues. None of the deficiencies caused actual harm, but they have been noted as having potential for harm, affecting groups of people at different levels.

While Harmony Hills emphasizes creating a comfortable, homelike setting with a warm and cozy feeling, and the staff tries for compassionate and family-oriented care, the facility also works under strict nursing home standards and gets reviewed by inspectors, so families can see a recent picture of how it runs. Harmony Hills serves as both a short-term rehab spot for people moving from the hospital or as a long-term place for those needing skilled nursing care, and the facility keeps a focus on meeting the daily needs of residents with support and proper medical oversight.
