    Caring staff but unsafe, inconsistent

I had a mixed experience. The therapy team and several nurses were awesome, caring, and helped my loved one improve, and a few staff formed genuine bonds. But too many staff were inattentive, rude, or unqualified - poor dementia care, rough handling, missed communication, missing belongings, and safety/neglect concerns. The facility is old, often hot or smelly with heater/AC problems, housekeeping is inconsistent, and meals/activities are hit-or-miss. It seems budget- and profit-driven: it tries hard in places but isn't appropriate for high-need rehab or dementia patients unless you get one of the good caregivers.

About Heritage Park Rehabilitation and Skilled Nursing Center

Heritage Park Rehabilitation and Skilled Nursing Center sits at 2806 Real Street in Austin, Texas, and provides care for an average of 183 residents a day out of 197 certified beds, and you'll find they've got a skilled nursing staff and a physician on-site to help folks who need constant care, medication, help with meals, or personal hygiene. They offer a range of services, so you see things like physical therapy, speech therapy, occupational therapy, post-surgical care, orthopedic care including for joint replacements, tube feeding, wound care, and even cardiac and pulmonary care, and they admit patients seven days a week and help people with stroke recovery, respiratory care, and provide respite care for families needing a break. The facility has memory support for residents with Alzheimer's or dementia, and they've set up behavioral health help too. There's an infection control deficiency from a recent November 2024 inspection, and three infection-related areas were flagged, plus an environmental deficiency around pest control, which means there were some issues noted even though nobody got hurt, and there's a full pest control program in place for prevention and management. The facility's last inspection showed a total of 45 deficiencies. Regency Integrated Health Services started managing the place in December 2022 and Regency IHS Of Heritage Park, LLC took over in November 2024, so there have been some management changes. Nurse turnover sits at 35.9%, which is lower than the Texas average, though they've got 3.09 nursing hours per resident per day, which is a bit under the state average. Heritage Park is part of the Plus Care Network and Wellsential Health, and you'll also see them listed with the Texas Independence Health Plan network and connected clinics like North University Clinic & Express Care, East Austin Clinic, and several others. They offer home health services for people who want to stay as independent as possible at home, plus hospice services for comfort and quality of life. Residents can use amenities like WiFi, cable TV, a beauty salon, a secure memory support unit, private or semi-private rooms, recreational activities, transportation, and an outdoor courtyard. There's outpatient therapy, infusion services, and even mobile clinics, so they cover a lot of ground when it comes to care. While the center is focused on providing a supportive community for recovery and daily living, prospective residents and families may want to consider all inspection findings and services when evaluating the facility.
