    Strong rehab, inconsistent long term

I had a mixed experience. The therapy team, social workers and many nurses/aides were excellent - skilled rehab, compassionate caregivers, helpful admissions and insurance assistance, good activities and mostly decent food. However, nursing and management were inconsistent: understaffing, rude or unprofessional staff, medication/bathing/feeding errors, lost belongings, billing/collection problems, and serious cleanliness/safety concerns reported. Overall I'd trust them for short-term rehab but would be cautious about long-term or memory-care placement.

    Summary review

Overall sentiment across reviews for Sagebrook Nursing and Rehabilitation is strongly mixed, with a clear pattern: many reviewers praise the therapy departments, social workers, and individual caregivers, while others report alarming lapses in basic nursing care, sanitation, and professional conduct. Multiple families credit the facility with excellent PT/OT that enabled successful rehab discharges home, strong therapy programming in the on-site gym, and compassionate, helpful social workers and admissions staff who navigate insurance and placement effectively. Named employees (for example Travis, Brandee, Monica, Bonnie and others) are frequently cited as standout helpers, and many reviewers describe clean, comfortable rooms, good activities, and a supportive atmosphere for short-term rehabilitation needs.

However, interwoven with those positive experiences are numerous, serious negative reports that cannot be ignored. A sizable subset of reviews describe inconsistent or neglectful nursing care — including missed medication doses, delayed wound care, bedsores developing during stays, and patients left in soiled clothes or diapers for extended periods. There are multiple allegations of understaffing (particularly on night shifts), heavy reliance on agency staff with high turnover, and poor follow-through on special care needs (e.g., low-sodium diets implemented late, delayed showers and hygiene care). These problems appear to create wide variability in patient experience depending on unit, shift, or specific staff on duty.

Sanitation and safety concerns are a recurring and serious theme in some reviews: complaints range from dirty rooms and bathrooms, mold, cockroaches and bug larvae, to feces found in beds and patients urinating in hallways. A few reviewers report such severe unsanitary conditions that they moved loved ones immediately. There are also claims of medication theft and alleged illegal drug handling by a night nurse, which raises safety and regulatory concerns that reviewers said they planned to report. Infection control is another issue raised — including reports of residents contracting COVID-19 while at the facility and inconsistent retesting or isolation practices.

Staff professionalism and communication show a split pattern. Many reviewers praise staff warmth, clear updates, leadership involvement, and strong communication from therapists and social workers. Conversely, others recount rude or unprofessional behavior from administrative staff and nurses: shouting at patients, berating, discriminatory or threatening interactions, and even removal of belongings. Families also report problems with billing and debt collection, including some instances of aggressive collection efforts after a resident's death and charges disputed at discharge, which undermines confidence in administration. Several reviewers indicated that the facility functions well for short-term rehab goals but is not suitable for long-term or memory-care needs.

Dining and activities are similarly mixed: numerous reviewers list engaging activities and outings and say residents enjoyed the food, while others found meals inedible or poorly managed diets for special needs. Therapy is consistently one of the facility’s strongest attributes across reviews, with multiple accounts of patients regaining independence, improved cognition, and successful transitions home. This consistency suggests therapy staffing and programming are organizational strengths.

Key patterns and practical considerations: the variability in care (excellent therapy and compassionate staff versus reports of neglect and unsanitary conditions) suggests inconsistent staffing models, possible unit-level differences, and shifts in management or personnel over time. Prospective families should verify current staffing ratios, ask about agency staff usage, inspect cleanliness and infection control practices, confirm nursing oversight and wound care procedures, and request clear written therapy plans and medication administration protocols. Documenting care issues early and keeping open lines with social workers (many are praised) can help. Because of the reported extremes — from exemplary rehab and loving end-of-life care to allegations of abuse, medication theft, and contagious outbreaks — the facility may be a strong option for short-term, therapy-focused stays with vigilant family involvement, but families considering long-term placement or memory care should exercise caution and perform careful on-site evaluation and follow-up.
