    Spotless facility, warm staff, excellent

I placed my aunt here for rehab and long-term care and couldn't be happier - the staff are warm, professional, and truly treat residents like family. The facility is spotless and welcoming, meals are tasty and nutritious, therapy is excellent, and activities keep residents engaged; nurses and aides go above and beyond. Only caveat: I've heard occasional complaints about office responsiveness/staffing turnover, but our experience was outstanding.

    Summary review

Overall sentiment across the reviews is strongly positive but mixed — the majority of reviewers praise The Hills Nursing and Rehabilitation for compassionate, individualized care, a strong rehabilitation program, and an exceptionally clean, welcoming facility. Many family members and residents attribute marked improvements in physical and cognitive functioning to the therapy teams and nursing staff; multiple accounts describe successful rehab-to-home transitions and restored mobility. Reviewers frequently highlight a warm, family-like atmosphere where staff know residents by name, provide end-of-life compassion, and ‘‘go above and beyond’’ standard duties.

Staffing and direct-care quality are central positive themes. Numerous reviews single out compassionate nursing staff and caregivers, skilled therapists, and clinical specialists such as wound care nurses. Case managers and specific staff members are praised for strong communication, coordination, and responsiveness. Several reviews name staff (for example, an activities director, case workers, and administrators) as exemplary, reinforcing the perception of hands-on leadership and long-tenured administrators who take pride in the facility’s operations. The team-based care, personalized attention, and visible teamwork are repeatedly cited as drivers of resident well-being and satisfaction.

Facility condition, cleanliness, and dining also receive consistent positive marks. Many reviews describe the building as spotless, odor-free, and freshly maintained — clean rooms, hallways, bathrooms, and dining areas are emphasized. The kitchen and dietary staff are repeatedly praised for flavorful, home-style meals that are both appetizing and nutritious; some reviews specifically mention a nationally recognized dietary manager and kitchen staff pride. The dining experience and presentation are often described as above average compared with other facilities.

Activities and community life are another strength noted by reviewers. There are frequent comments about engaging, personalized activities run by an enthusiastic activities director, a strong sense of community among residents, and creative pandemic adaptations (FaceTime availability, window visits). These social elements are credited with improving mood and cognitive engagement for many residents. Reviewers also note small touches — snacks available, welcoming landscaping, and inviting interior décor — that contribute to residents feeling at home.

However, there are notable negative themes and important caveats. A number of reviews report staffing shortages, high turnover, or periods when key roles (such as the activities director) were vacant; these issues are tied to declines in consistency of care and programming. Several reviews raise serious concerns: unresponsiveness or dismissiveness from some office staff members, delayed medical attention leading in one report to severe UTI and sepsis, and allegations of financial impropriety (missing money, checks allegedly written to nurses). At least one reviewer reported filing a formal complaint with corporate. Other complaints include delayed or missing meals, cold or low-quality food service in isolated instances, and reports of neglect such as delayed diaper changes. These negative accounts contrast sharply with the many positive reviews and suggest variability in experience dependent on staffing, management responsiveness, or particular shifts/teams.

Management and corporate oversight are described inconsistently. Many reviewers praise an accessible and hands-on on-site administrator and long-serving leadership that take pride in the facility. Yet other reviews report poor management practices, high turnover in administrative roles, and perceived corporate indifference, especially around billing disputes or when residents and families raised formal concerns. Specific business-office staff were named unfavorably in several accounts for being unresponsive or slow with refunds. These mixed perspectives point to uneven experiences depending on which office personnel families interact with and how corporate-level issues are handled.

In summary, the dominant impression from the reviews is that The Hills Nursing and Rehabilitation provides high-quality, compassionate clinical care, strong rehabilitation services, engaging activities, and a very clean, home-like environment. These strengths have led many families to highly recommend the facility and credit staff with significant improvements in loved ones’ health and quality of life. At the same time, reviewers should be aware of recurring but less frequent concerns: staffing instability, isolated serious clinical or administrative incidents, billing disputes, and occasional lapses in service. Prospective residents and families would benefit from asking specific questions about current staffing levels, recent turnover, billing policies, and how the facility addresses complaints to ensure they match the generally positive experiences described by most reviewers while guarding against the reported exceptions.

About The Hills Nursing And Rehabilitation

The Hills Nursing And Rehabilitation has 110 certified beds and focuses on recovery and well-being with a range of services like skilled nursing, assisted living, rehab care, memory care, and vent care, and the place also has in-house dialysis services which helps when someone needs more medical support without having to travel. The center is owned by West Wharton County Hospital District and Decatur I Enterprises LLC manages it, with Gary Blake and Malisa Blake being involved since June 2022, and it's a part of the Creative Solutions In Healthcare network. Residents get about 2.98 nurse hours per day, and staff include caregivers who've had specialized training to help people with different health needs, and while the home's known for giving high quality nursing care, there have been 19 deficiencies found on inspection reports, including two related to infections, and some quality of life and care planning issues marked as immediate jeopardy, so while the staff does work to improve, careful attention is needed. Folks living here receive 24-hour care, help with medication, personal care plans, and therapy services for wound care, physical, occupational, and speech needs, and the place welcomes both long-term and short-term stays, even helping with stroke recovery, cardiac illnesses, surgery recovery, and diabetes. The building has features for comfort and safety, offers meals three times a day that people usually enjoy, provides field trips, recreational and spiritual activities, and has amenities like beauty and barber services, housekeeping, laundry, and transportation for appointments. There's also help with podiatry, dentistry, optometry, hospice, and veterans affairs. The staff aims to create a supportive, home-like feel, giving attention to physical, emotional, and social needs, and there's an online portal for families to stay connected and get updates, plus tools to locate other network facilities if needed, and options to contact staff through forms or a customer care line if there's any concern. The community tries to encourage participation in daily life, offers a variety of activities, faith services, and makes sure everyone's needs get addressed through teamwork and personal plans, but it's important to note the inspection history and staff turnover rate of about 32.5%.
