    Promising start, infections and neglect

I placed my father here for rehab and the early results were excellent - PT/OT/speech made real improvements and many nurses and aides were attentive, kind and communicative. The facility felt home-like with engaging activities, friendly residents and generally clean, fresh common areas. Unfortunately over time he developed C-diff, recurrent UTIs and bedsores, and I saw signs of poor cleaning, inconsistent hand hygiene and breakdowns in nurse/aid communication; promised fixes didn't happen. Medication and personal care were sometimes delayed or missed, misinformation about wounds was given, and hospice eventually became involved. The front desk was often unmanned and COVID visitation restrictions added stress. Because of the infections and decline we moved him to another facility - I'm grateful for several outstanding staff who gave us comfort, but I can't fully recommend this place without serious reservations.

This facility stands as a 146-bed center that's licensed and accredited by the Joint Commission, and it takes in people with different payment options including Medicare, Medicaid, private pay, and insurance, which some families find helpful when making decisions. There are skilled nurses on staff around the clock and there's care for a wide range of health needs like wound care, diabetes, recovery after surgery, cardiac and neurological needs, and therapies like physical, occupational, and speech therapy, all in a dedicated therapy gym. The space offers both private and semi-private rooms, each one with private bathrooms, kitchenettes, air-conditioning, cable TV, telephone, Wi-Fi, and some safety features fitted in, and residents benefit from housekeeping, laundry, and linen services.

Residents can join in games, arts and crafts, library reading, fitness programs, activities in a wellness center, or tend to an outdoor garden, with scheduled daily activities and community outings, and there's also resident-run programs so folks can stay as busy as they wish. Dining options are available with restaurant-style meals, menu choices for special diets, a dining room to gather in, and even choices for people with diabetes. The facility provides transportation for doctor's visits, errands, and community outings, and there's a salon and barbershop on the property for personal care. Senior Care at Denton Post Acute Care specializes in both long-term and short-term care, with services for individuals dealing with memory loss or requiring subacute or skilled nursing, and it works closely with the University Rehabilitation Center for extra support. Veterans will find dedicated services, and there are specific care options for people needing rehab-to-home, vent care, in-house dialysis, or respite care. There's also extra help for families, with move-in coordination, community care planning, honest information about care communities, and regular updates through an online Care Portal. Residents and families can use a Customer Care Line to share concerns or ask for help, either openly or anonymously, which helps make sure any worries get noticed.

What makes this place stand out a bit is the focus on personalized attention, assistance with every part of daily life, and an effort by staff to create a caring, home-like feeling, while giving people choices about what to do, what to eat, and how to spend their days, all backed by a skilled nursing team and a connection to Senior Care Health & Rehabilitation Center-Denton and the wider Creative Solutions in Healthcare family. The community provides transparent, verified reviews and wants to help families find the right match by giving honest guidance and free consultations; they don't list prices up front, but they do provide availability details. Amenities include kitchenettes, washers and dryers, sprinkler systems for safety, game rooms, guest parking, internet, and common areas where folks can spend time together. Medical services offered on-site include therapy, wound care, podiatry, dentistry, optometry, and medication support, with special care units for memory or dementia needs. Staff works to keep everyone engaged with meaningful activities and physical wellness to help each resident feel at home and maintain independence as much as possible, so it's a place where people can get support while having some choices in their day and knowing a nurse is always nearby.
