    Clean facility, but dangerous neglect

I moved my father here and wish I hadn't. There are some wonderful nurses, CNAs and physical therapists and the activities can be fun, but the positives don't make up for systemic failures. The place is chronically understaffed, phone lines and administration were unresponsive, and family calls often went unanswered. Staff communication was poor, meds were mishandled or withheld, basic hygiene was denied, and supplies were frequently unavailable. I found residents in unsanitary conditions (feces/urine on bedding and shower floor) and my loved one developed bedsores and serious infections that led to amputation. Management was rude and indifferent, the social worker only acted too late, and after my father died we received no apology or condolences. Clean, bright facility and great therapy - but unsafe care, frequent neglect, and terrible communication: I strongly advise looking elsewhere.

    About Paradigm at the Prairies

Paradigm at the Prairies is a skilled nursing facility with 150 licensed beds and an average of about 98 residents each day, so there's room for both long-term stays and short-term rehabilitation for folks who need help after leaving the hospital but aren't quite ready to go home. This place is Medicare and Medicaid certified, and also has approval from the Centers for Medicare & Medicaid Services, which means they can take those kinds of insurance. The nursing staff includes certified nursing assistants, licensed vocational nurses, and registered nurses, though the nurse hours per resident per day are 2.82, which is below the state average, but their nurse turnover sits lower than the state average at about 42.5%. The services there cover a lot, with skilled nursing, physical therapy, activities, dietary and housekeeping, organized group activities, and social work, plus help with things like home care, home health, hospice, and referrals to other care if needed. They offer adult care home services, assisted living, independent living, memory care, and intermediate care for very frail residents who need nursing care. The Paradigm Healthcare group runs this place and several others in Texas, with management by El Campo Nursing & Rehabilitation LLC and Aharon Shkop since January 2024. The facility focuses on building an empowered team for better resident support, and management says they're committed to compassionate and innovative care, helping to create a supportive atmosphere. You'll find the usual amenities you'd expect to make residents and visitors feel at home, along with organized activities, and things like a resident satisfaction line and access to a long-term care ombudsman for help with complaints. Past inspections have found problems with infection control, urinary care, and waste disposal, so that's something to keep in mind, but the place does address care issues and has several support programs. They do let families tour the facility, and you'll get a range of care options from short-term rehab all the way to long-term nursing, so anyone looking for a range of help, whether that's physical therapy after a hospital stay or full nursing support for ongoing needs, will find lots of choices here along with the support you'd expect in a larger care facility.
