    Filthy rundown facility with neglect

I placed a loved one here and it was awful. Rooms, mattresses and clothes were filthy, smelled of urine, and I saw bugs; the building is run-down, not wheelchair-friendly and needs a full overhaul. Staff were often rude or absent (worse on weekends), patients left in soiled bedding, meds and care were delayed or withheld, and families weren't properly notified - food was cold/scant, showers and activities infrequent. Administration was unresponsive; a few staff were kind, but overall I would not trust them with anyone I love and think the facility needs investigation/closure.

    Summary review

Overall sentiment from the collected reviews is heavily polarized but leans strongly negative. A substantial number of reviewers report serious, persistent problems with cleanliness, basic care, staffing, and facility maintenance. These negative reports are severe in some cases, alleging neglect, abuse, medication withholding, theft, delayed emergency responses, and even deaths in the facility’s care. Intermixed with these negative accounts are a smaller but meaningful number of positive reviews praising specific staff members, an administrator, and some aspects of care and safety. The pattern suggests pronounced inconsistency — some residents and families experienced attentive, trustworthy care, while others describe dangerous or neglectful conditions.

Care quality and resident safety are the most frequently cited concerns. Multiple reviews allege neglectful behavior such as patients left in soiled bedding, infrequent showers, failure to provide medications, and delayed or inadequate response to emergencies (including allegations of delayed CPR and deaths). Several reviewers explicitly call for health department intervention or closure, and at least one review references a historical death (2010) as indicative of longstanding problems. At the same time, a subset of reviewers report “excellent” or “amazing” care and state that their family members were treated safely, demonstrating a stark split in reported experiences.

Staff conduct and staffing patterns are another recurring theme. Many complaints describe rude staff, staff who hang up on family members, theft, and use of personal cell phones during shifts. Weekend coverage is repeatedly criticized, with reviewers reporting gaps in care and unhelpful weekend staff. Conversely, some reviews highlight individual staff as friendly, helpful, and family-oriented, and a few praise a trustworthy administrator. This variability suggests inconsistent hiring, training, supervision, or shift-to-shift standards.

Facility condition, housekeeping, and infection control are commonly criticized. Reviews mention filthy rooms and mattresses, strong urine/feces odors, roaches and insects, peeling paint, old flooring, mold or peeling molding, and generally uninviting, run-down common areas. While one or two notes mention a room being clean or small positive touches like flowers, the dominant narrative describes a facility in need of substantial physical rehabilitation and more rigorous cleaning and pest-control protocols.

Operational issues extend to dining, activities, and medical supplies. Food quality is frequently called out as poor, with meals arriving not hot and with limited portions. Reviewers also note a lack of meaningful activities and social programs, which can impact residents’ quality of life. Several reports indicate shortages of medical supplies and medications and criticize the director of nursing (DON) or administration for not being proactive or responsive. COVID visiting restrictions are mentioned as having compounded family stress and limited oversight during the pandemic period.

Notable patterns and practical advice: the reviews indicate long-term, recurring complaints (some reviewers say little has changed in over a decade), suggesting systemic problems rather than isolated incidents. However, the presence of strongly positive reviews indicates that experiences can vary considerably, possibly by unit, shift, or individual staff members. For families considering this facility, the reviews collectively recommend thorough due diligence: review state inspection reports and complaint histories, visit in person (including weekends and evenings), ask about staffing ratios and turnover, verify medication management and emergency protocols, observe cleanliness and odor, and speak directly with recent families. Given allegations of serious neglect and calls for regulatory action in some reviews, prospective residents and families should weigh both the negative patterns and the isolated positive reports carefully before deciding.

In summary, the review set paints a picture of a facility with deep, recurring concerns about cleanliness, care consistency, staffing behavior, and physical plant condition, punctuated by occasional reports of competent, caring staff and management. The dominant impression from the reviews is that quality and safety may vary dramatically depending on timing and personnel, and that significant oversight, improvement, and transparency would be necessary to address the most serious allegations raised by multiple reviewers.

    About DFW Nursing & Rehab

DFW Nursing & Rehab sits in Fort Worth, Texas, and offers skilled nursing care, memory care, and both short-term rehabilitation and long-term care services. The facility runs with 98 certified beds and has about 74 residents each day, providing a place where people get help with daily needs like dressing, bathing, and medication management. Nurses are on-site around the clock, with an average nurse turnover lower than the state average but fewer nurse hours per resident per day than many Texas facilities. Staff follow individualized treatment plans designed by doctors with input from staff, residents, and family. The facility's leadership includes Brian Scroggins, Kendall Broussard, and Charleston Dfw Operations LLC, in place since 2014 and 2015, and it's part of the Charleston Healthcare Group.

The center did get cited for some serious issues, like not reporting suspected abuse or neglect in a timely way, not protecting residents from abuse or neglect, and not keeping the environment entirely free from hazards-these were marked as immediate jeopardy deficiencies and brought a fine of $69,770 and a payment suspension after a complaint in February 2025. They've also had infection-related deficiencies. Despite these citations, no residents came to actual harm during those events, but there was potential for more than minimal harm. DFW Nursing & Rehab is a for-profit facility, approved for Medicare and Medicaid, and accepts most private insurance.

Residents have access to modern amenities, private rooms with bathrooms, home-cooked meals, special options for different diets, cable TV, Wi-Fi, and comfortable living spaces that recently saw renovations. There's a salon and barbershop, daily housekeeping and laundry, a business room, a fitness room, outdoor spaces for relaxation, regular outings, movie nights, and a spa/sauna. Residents can join in on scheduled entertainment, religious services, and various activities to stay social and active. There's special security for residents with dementia who might wander, a 24-hour call system, regular physician visits, and transportation to appointments.

DFW Nursing & Rehab supports people needing IV therapy, hospice care, rehabilitation with Synergy Therapy Services, and those who are non-ambulatory. Staff help with transfers and daily care needs so people can live as independently and comfortably as possible. The facility puts effort into offering a warm and home-like environment, with staff focused on being caring and attentive to residents' needs, always working to keep everyone safe, supported, and well cared for even if things aren't perfect-because keeping people comfortable, cared for, and secure should always come first.
