    Good daytime care, unsafe nights

I like the caregivers - kind, attentive, and the facility is clean with good meals, activities, and helpful therapy. But nights and weekends are clearly understaffed; I've seen call lights ignored and residents left without help, which felt dangerous and made me distrust staff. Be cautious and confirm staffing before choosing this place.

    Summary review

Overall impression: Reviews for Country Trails Wellness and Rehab Center are mixed but lean positive in many summaries, with several reviewers praising the caregiving, therapy, and activity offerings. Multiple accounts highlight caring, attentive staff and effective rehabilitation that led to health improvements. At the same time, there are significant and recurring concerns—most notably around staffing levels, responsiveness, and inconsistent cleanliness—that create a polarized picture of the facility.

Care quality and rehabilitation: A recurring positive theme is strong hands-on care and an effective therapy team. Several reviewers explicitly report that residents received needed bathroom and mobility assistance, experienced health improvements, and enjoyed the extra rehabilitation services. Caregivers are frequently described as loving, attentive, and caring, with staff going "above and beyond" in many instances. Conversely, there are troubling accounts of poor responsiveness — including reports of call lights being ignored, residents being left on the toilet and soiling themselves, and one very serious 1-star review that alleges neglect with perceived risk to life. These negative reports are stark and suggest that, while many shifts provide adequate to excellent care, there are occasions when care lapses occur.

Staffing and staff performance: Staffing issues are a consistent pattern. Multiple reviewers identify understaffing, especially at night and on weekends. This understaffing is linked in the reviews to slower response times and poorer care on certain shifts. While day-shift staff and therapy teams are often praised, reviewers describe inconsistent staff performance across shifts: some caregivers are "very good" and "highly recommended," while others are distrusted or judged neglectful. A few reviews note that problems brought to management's attention were addressed, implying some responsiveness from administration, but the staffing shortages appear to be an ongoing operational concern.

Facility condition and cleanliness: Opinions on facility cleanliness are mixed. Several reviewers explicitly commend the center as "very clean" with no urine odor and report that past rumors of mold had been debunked. By contrast, other reviewers claim the facility is "nasty," "dirty," and has "mold everywhere," indicating serious cleanliness problems on some occasions or in certain areas. The facility itself is described as older with small rooms, which may contribute to perceived cramped conditions and could factor into differences in cleanliness or patient comfort between rooms or wings.

Dining and activities: Dining impressions are mixed but generally more positive than negative. Some family members report that their loved ones "like the meals" and receive assistance as needed; others say the food is "not appetizing." Activities are highlighted as a strength: multiple reviewers mention many activities, residents "having fun," and positive social engagement. These programmatic elements appear to be a clear positive, contributing to resident satisfaction where interpersonal care is consistent.

Management and patterns to note: Several reviewers indicate management will address problems when raised, suggesting a degree of accountability. However, the frequency of complaints about staffing and responsiveness—especially on nights and weekends—suggests systemic challenges rather than isolated incidents. The existence of a very negative, safety-focused review alongside many positive ones indicates variability in the resident experience; the divergence may be tied to specific shifts, staffing levels, or particular units within the facility.

Bottom line: Country Trails shows many strengths—compassionate caregivers, an effective therapy program, active resident engagement, and multiple positive reports of cleanliness and good meals. Nonetheless, the consistent reports of understaffing, especially nights/weekends, mixed cleanliness reports (including allegations of mold from some reviewers), and at least one severe allegation of neglect are important red flags. Prospective residents and families should weigh the generally positive caregiving and rehabilitation feedback against the documented inconsistencies in staffing and cleanliness, and should ask direct questions about night/weekend staffing levels, recent inspection reports, and how the facility handles missed call-light responses and other care lapses.

About Country Trails Wellness and Rehab Center

Country Trails Wellness and Rehab Center in Grand Saline, Texas, is a large senior living community with 120 licensed beds and space for about 70 residents each day, and you'll find a range of care like skilled nursing, assisted living, independent living, and support for short-term or long-term recovery, all set up so folks who come here get help with daily tasks like bathing and dressing, medication management, and things like wound care or pain management, and what's good here is they offer specific therapy programs for people who need physical, occupational, or speech therapy, whether someone is bouncing back from orthopedic surgery, heart issues, or stroke. They've got a state-of-the-art fitness center, peaceful outdoor areas, comfortable private rooms, and 24-hour call systems with supervision, making sure safety and comfort stay a priority, and they pay close attention to creating a welcoming environment where everyone can work on their wellness, get support from skilled nurses, and have personalized care plans. Folks here can use both inpatient and outpatient therapies, recreational activities, and even telehealth counseling, and the nursing team handles complex needs with help from physicians, psychiatrists, psychoanalysts, and other healthcare professionals, plus there are programs for women, infants, and children in the community, and care options extend to hospice and palliative support too. They offer medical, social, and home health services, manage drug and alcohol concerns, and provide wound care, partnered with area hospitals for extra help when needed. They have an A+ rating from the Better Business Bureau and are known for comprehensive care, though recent inspections did find a few issues, including some infection control and nutrition problems, so it's worth knowing this facility does its best but sometimes faces challenges like any other, with 3.49 nurse hours per resident per day, a nurse turnover rate over 50%, ownership by Live Oak Healthcare, LLC and Trinity Healthcare, LLC, and some deficiencies reported in areas like food storage and infection programs. Still, the focus at Country Trails Wellness and Rehab Center remains on helping residents recover and live as well as possible in a supportive, comfortable setting designed for health, healing, and day-to-day living.
