    Neglectful care, poor hygiene, unsafe

    I placed a loved one at this small Itasca home and had mixed feelings. The administrator seemed loving and a few caregivers gave 100% and showed real care, but too many staff were neglectful and unfeeling and the administration was largely unhelpful. I saw poor hygiene (feces under nails, strong odors), plumbing issues, medication mismanagement, lost dentures we were billed for, and medical information withheld-one resident with an esophageal condition reduced to puréed meals. A resident died of COVID; I reported incidents to APS and health officials and plan to testify in court calling for a state investigation. Residents can look active and the home-style cooking was fine, but I cannot recommend this facility for dependable, safe care.
