Southern Specialty Rehabilitation and Nursing sits under the management of Lubbock I Enterprises, LLC, with Gary Blake and Malisa Blake involved, and it's run for profit. The place can serve up to 144 people, but usually tends to about 69 residents at a time, and while it's part of Creative Solutions in Healthcare, it's got people of all ages coming through, not just older folks, so there's a real mix. The facility provides short-term and long-term skilled nursing, rehab care, ventilator services, memory care, dialysis right inside the building, and there's support for things like speech therapy, wound care, spinal cord and brain injury, and recovery from strokes, surgery, or heart and diabetes problems; folks can get help with podiatry, dentistry, optometry, hospice, and medicine, and daily needs like meals, laundry, housekeeping, and transportation get handled too. Southern Specialty runs a rapid recovery unit, brings in a team for vent care with 24/7 respiratory therapists on hand, and supports veterans, making plans for each resident so care matches the person. There's a nurse turnover rate of 64.4%, which runs higher than Texas' average, but the place has 3.77 nurse hours per resident per day, which is a bit above the state's 3.4 hours, and staff keep up services like pulse oximeter monitoring, IV support, and help with dignity around continence care. Folks can get involved with activities like Bingo, volleyball, arts and crafts, and holiday events, and there's special attention to environmental sustainability worked into daily routines as well as an online portal for families to keep up with how their loved ones are doing, and a customer care line for feedback or concerns. Meals get tailored to local tastes with a focus on nutrition, and there's a White Glove Program that aims for extra cleanliness and infection control, although the facility has had its share of citations in recent years: in the standard April 15, 2025 inspection, three deficiencies showed up-one for infection control-and over time, 26 deficiencies got reported, including five tied to infection control, and there are past citations for not buying or serving food the right way, issues with incontinence and catheter care, and not always handling infection prevention as it's supposed to. Even so, staff stay focused on making daily life as home-like and respectful as possible, supporting residents with everyday living and social needs.

Strong rehab, but hygiene issues
I had a mixed experience: the therapy, many nurses, and some administrators (notably Dana) were outstanding-helpful admissions, strong rehab (weaning off a ventilator), friendly staff, and an often clean, updated building-yet I also saw chronic understaffing, poor hygiene (urine/feces odors, residents unbathed or left soiled), ignored call lights, inconsistent or rude management, and billing/transfer problems. Overall I'd recommend only cautiously-check current leadership and staffing and monitor care closely.
