    Compassionate staff, concerning management issues

I found the facility clean, welcoming and well-run day-to-day - the nurses and therapy team (Zoe and many others) were compassionate, professional and top-notch, residents stayed active and my loved one received excellent rehab and personal care. However, I also observed staffing shortages and heard disturbing reports of inconsistent end-of-life care, neglect and management problems, so while I'm grateful for the caring staff, I'd advise close oversight and asking hard questions before committing.

    Summary review

Overall sentiment for Village Creek Rehabilitation & Nursing Center is strongly mixed, with a substantial number of reviews praising the facility and an equally concerning set of reviews describing serious problems. Many family members and former residents highlight the facility’s strengths: a clean, well-kept and attractively decorated environment; a warm, welcoming atmosphere; attentive and compassionate staff; and a highly regarded therapy department that supports effective rehabilitation and quick recoveries. Multiple reviewers singled out specific employees for exceptional service (for example, Mary in marketing and Zoe) and described staff as professional, personable and consistent. The facility is frequently described as active and engaging for residents, offering on-site activities such as bingo, ping pong, dominoes and organized outings like monthly trips to Walmart. Private rooms, a pleasant courtyard and a tidy dining room are also repeatedly mentioned as positive physical attributes.

Care quality is a dominant theme in both positive and negative reviews. On the positive side, reviewers report strong nursing support, excellent therapy outcomes, and hospice involvement that brought comfort to families during end-of-life care. Staff stability, teamwork, and a clear customer-service focus are cited as contributors to good resident experiences. Several reviewers explicitly recommend Village Creek for short-term rehabilitation and long-term care based on observed clinical outcomes and personal interactions with therapists and nurses.

However, the negative reviews raise very serious and specific concerns that cannot be ignored. Multiple accounts allege neglect, including instances where residents reportedly went long hours without food and claims that feeding-tube orders from a physician were not followed. There are also allegations that promised safety checks (for example, hourly checks) were not carried out and that some staff falsified records. A subset of reviewers described abusive or barbaric behavior by nurses and aides, poor end-of-life care, and an administrative culture perceived as driven by finances rather than patient welfare. These reports paint a picture of inconsistent care quality — where some residents receive excellent attention while others may experience neglect or worse.

Facility management and transparency are also mixed in the reviews. Some families said administration exceeded expectations and was helpful, but others reported that complaints were ignored and that the administration was money-driven. Staffing levels are cited as a chronic concern by several reviewers, with mentions of limited staffing or shortages that could account for variability in care and missed checks. Cleanliness likewise shows a split: many reviewers praise a clean-smelling, well-maintained facility, while a few report poor cleanliness and unsanitary conditions.

What emerges from these reviews is a facility that can provide excellent therapy, compassionate nursing, and an engaging environment — but one where experiences appear highly variable and where very serious allegations of neglect and record falsification have been reported by multiple reviewers. Because the concerns described are significant, prospective residents and families should weigh both the positive and negative reports carefully. Recommended next steps for families considering Village Creek include touring the unit(s) in person, speaking directly with current families and staff, asking for specifics about staffing ratios and feeding/monitoring protocols, reviewing recent state inspection reports and complaint history, and observing mealtimes and activity periods when possible. Such direct verification can help determine whether the strengths highlighted by many reviewers align with an individual resident’s needs and whether the facility has addressed the troubling issues raised by other reviewers.

About Village Creek Rehabilitation & Nursing Center

Village Creek Rehabilitation & Nursing Center sits in a spot with space for 120 beds and usually cares for about 72 residents each day, where folks get help with skilled nursing, rehabilitation, or long-term care depending on what they need, and you'll find both short-term stays for rehab and longer stays for those who need more help day by day, so some residents are just here between hospital and home, while others depend on nursing care nearly all the time. The building's managed by Nexion Health At Lumberton, Inc. since 2017, though Sweeny Hospital District owns the place completely, and you'll see Nexion Health still oversees much of what goes on, making sure there are nurse practitioners, clinical specialists, and other long-term care staff available. Skilled care workers, including certified nursing assistants and hospitality aides, help with a wide range of services, and you might see some of these staff who've stuck around for quite a long time, which many residents say makes the place feel friendlier and more like home, even with round-the-clock nursing care and daily rehabilitation sessions going on.

The center has a nurse staffing average of 3.21 hours per resident per day and a turnover rate for nurses of 50.8%, which means sometimes you see new faces, but they keep training their team and checking skills, making sure everyone's up to date with what residents need, letting them handle things like trach care and IV therapy when needed, and having wound care tools like ABI testing and Negative Pressure Wound Therapy right there in-house. Telemedicine is something they use to help residents connect with outside providers easily, and private rooms are available for folks needing short-term therapy, making recovery a bit more comfortable. Village Creek offers therapy for those working to get back on their feet, with physical, occupational, and speech therapy, and respiratory services too for folks who need extra help with breathing, and you'll often find residents working with personalized treatment plans set up by the care team, with a focus on keeping them as independent as possible. It's pet-friendly, which some find makes it easier to settle in, and they try to keep the place feeling homey, with activities like bingo and parties or themed events, so there's something to look forward to even while you're here getting care.

Now, inspection reports show 28 deficiencies over time, and these include issues with infection prevention, some nutrition and food service problems, and an environmental item, and those are things inspectors keep an eye on with regular visits, so the place does get cited at times and it's something people should know when choosing a facility. Still, the staff focuses on providing safe and comfortable care, with a wide range of wound care, therapy, and specialized clinical support, and they accept residents any time day or night, trying to meet the needs of folks with many kinds of chronic or sudden illnesses. Residents often say they appreciate the kindness from the care team, and staff point out they enjoy working here, though no place is perfect, and Village Creek keeps aiming for a home away from home where folks can get the care and support they need.
